1 A B C HEALTH & SAFETY CHECK LIST Yes/ No ROAD TANKER WAGON ACCESS TO SITE Can a large articulated vehicle drive on and off site, without obstructions? Can the vehicle turn on site to enter and exit forwards? If not, someone must assist the driver to reverse. Are there any height restrictions? (11ft 6 inches, height of the RTW) Is there a satisfactory surface to and from the delivery point where the vehicle (RTW) parks? Is there any pedestrian activity that the driver should be made aware of? If yes, then driver should be assisted Is there any site transport activity that the driver should be made aware of? E.g. forklift truck, cranes, and other traffic. If yes, then driver should be assisted. DELIVERY ACCEPTANCE PROCEDURE Do you have a written Delivery Acceptance Procedure in place? Is there an authorised person(s) available to receive the delivery and be in attendance with the driver at all times? (WHO? Name or Job) (Not applicable with approved unattended deliveries) Are the person(s) responsible for receiving the delivery trained to do so? Where tank inlets lead to more than one tank, is the person(s) in attendance receiving the delivery capable of setting the valves correctly? Has a risk assessment been carried out on deliveries to the site in question? (Please provide date of Risk Assessment) As required by Health & Safety legislation is a copy of the risk assessment available on site? OIL STORAGE TANK (S) DETAILS How many oil storage tanks are located on your premises? List grades. Are oil storage tanks located above ground level? (See guidance notes) Is the tank surrounded by a containing wall (bund) capable of holding the capacity of the largest tank +10%? C OIL STORAGE TANK (S) DETAILS CONTINUED Would an overflow from the vent be contained within the 16. bund? If not, where would it run to? Are tanks & remote fill points (if any) marked with the 17. product grade? Is the tank equipped with calibrated tank gauging? Is the tank fitted with a high level alarm and can this be tested? Yes/ No

Does your facility conform to the Oil Storage Regulations ? D OIL STORAGE TANK (S) ACCESS Does the driver have a safe area to work around the RTW 23. and storage tanks? Is the receiving storage tank and vent pipe(s) in full view from 24. the RTW delivery point? If any deliveries are to be made outside day light hours, are the 25. delivery premises sufficiently lit to cover the storage tank, the tank vents and the offloading area? Are the tank inlets within 6 metres of the delivery offloading 26. area? Are tank inlets accessible from ground level? (If not please 27. provide details if above ground level) 28. Is there suitable containment at the tank(s) inlets? E.g. drip tray Are tank(s) arranged with fixed hose connections & 29. control valves? (See guidance notes) What are the size(s) of the delivery point connection? E Does each tank inlet connection lead to one tank only and labelled accordingly? In event of a spillage are there any drains, open water, rivers, canals or other watercourses within 10 metres of the tank? ADDITIONAL COMMENTS Are there any times when deliveries should not be made for safety or any other reasons? If so please specify. Are there any site-specific issues not covered? GUIDANCE NOTES A ROAD TANKER WAGON ACCESS TO SITE 1) If obstructions are of a temporary nature, can the customer ensure clear access on delivery date? If a large tanker cannot drive on or off easily the delivery may have to be made using a small tanker, this will affect load size. 2) Reversing on the public highway should be avoided. Any person directing a large goods vehicle needs to be trained. Wear a reflective waistcoat if directing the vehicle. 3) Minimum height for the RTW to access the site is 11ft 6 inches.
3 4) Unmade surfaces or mud may cause problems with vehicle movement. The driver is instructed to refuse to go to the delivery point if ground/conditions are not considered suitable. Vehicles delivering to quarries may be required to have yellow flashing lights. 5) The driver should be made aware of any pedestrian activity in close proximity or around the delivery area. 6) Mobile Plant and vehicles should not be working in the offloading area while the vehicle is discharging. B DELIVERY ACCEPTANCE PROCEDURE 7) A written procedure must be in place for acceptance of the delivery on your site. 8) A person must be available on site and be in attendance throughout the delivery in order to ensure the safe delivery of fuel. A written agreement must be in place between the customer and Total for unattended deliveries. 9) A person who is responsible for receiving the delivery must be competent to receive the delivery and be fully aware of the Total delivery procedure. 10) Delivery drivers will not open or close any values other than the flow control valve at the delivery point. The person representing the customer must be competent to open and set the valves correctly. 11) A Risk Assessment must be carried out for the receiving of fuel in the delivery area. This is a legal requirement under the Heath and Safety work act ) A copy of this Risk Assessment for receiving fuel in the delivery area must be on site for the driver to see, if need be. C OIL STORAGE TANK (S) DETAILS 13) Please state how many oil storage tanks that you would require Total to deliver fuel into. 14) Please note we do not pump into underground storage tanks 15) Any uncontrolled overflow or spillage could lead to pollution, followed by prosecution of both the delivering company and the
4 customer. The Oil Storage Regulations specify that the tank must be surrounded by a bund capable of holding 110% of the tank capacity. In order to contain any major spillage, bund should be product tight. 16) Tank vents or overflow pipes should direct any oil in to the bund. If the driver can see the tank vents, he can minimise the amount overflowing by stopping the flow rapidly. Note that Mushroom type vent caps will spray any overfill over the surrounding area. 17) All tank inlets must be clearly marked with the product stated, as many mistakes have been made in the past where product grade labels on tanks are missing or damaged. 18) Book-stock records are an unreliable method of checking tank contents. Tanks must be fitted with a calibrated tank gauging system such as a dipstick or electronic gauge. 19) Overfill protection alarm systems if in place, should have a system of checking the operation. 20) It is recommended that a secondary containment system (interceptor) be in place to collect any spillage. 21) A regular inspection programme should be in place to test and recalibrate gauges as necessary and to ensure that any other defects are identified and rectified. 22) It is recommended that you obtain copies of the Oil Storage Regulations and ensure compliance. The Oil Storage Regulations (2001) apply to the storage of oil on any premises, with certain exemptions. Total is not responsible for working for compliance, but may refuse to deliver if you do not comply. D STORAGE TANK ACCESS 23) The driver needs to be able to work in a safe area without the risk of injuries though out the delivery i.e. trips, slips and falls 24) If the driver cannot see the storage tank vent pipe, this is termed as a blind delivery and an authorised person (see note 8) will have to over see the vent pipe during the delivery. Please state if there is more than one vent per tank and confirm visibility from offloading area. 25) Lighting should give adequate vision at the near side of the parked vehicle, the storage tank and the tank vents if deliveries are made at night. 26) Large road tankers do not carry long lengths of hose. A small hose reel equipped vehicle may have to be used.
5 27) The driver should not be required to make 3 inch hose connections above normal working height. A small hose reel type vehicle may have to be used if inlets are above normal working height but fixed access must be provided. 28) A suitable containment must be fixed at the offloading point to capture any drips or spills 29) Fill pipes without a valve and non-return valve may allow oil to run back when the hose is disconnected, causing a pollution problem. All inlets should be fitted with an end cap. D STORAGE TANK ACCESS CONTINUED 30) Tankers carry 3-inch hoses and appropriate fittings for 2-4 inch connections. 31) Inlets that feed into more than one tank give a risk of overfilling of the tank. Installations where tanks are filled through other tanks (interconnected tanks) are particularly at risk. All inlets should be identified and labelled. 32) Storage tanks close to drains, open water, rivers, canals or other watercourses pose an environmental risk in event of a spillage, please refer to the Oil Storage Regulations In the event of a spillage the Environment Agency also has powers to prosecute under other legislation.
