1 REQUEST FOR STATEMENT OF QUALIFICIATIONS FOR WATER SOURCE RAW WATER TRANSMISSION MAIN FOR CITY OF SIDNEY SIDNEY, OHIO PROPOSALS TO BE RECEIVED AT THE OFFICE OF THE PUBLIC WORKS DIRECTOR 201 WEST POPLAR STREET SIDNEY, OHIO UNTIL 3:00 P.M., E.S.T. THURSDAY, SEPTEMBER 25, 2014

2 NOTICE TO PROFESSIONAL CONSULTANTS The City of Sidney requests a Statement of Qualifications and other supporting information from qualified professional consulting firms or individuals to provide the following item(s) of work for the plan development of a raw water transmission main from a new proposed water source located in Washington Township to the City of Sidney Water Treatment Plant located at 880 E. Court Street, Sidney, Ohio. Survey and design of proposed 47,000 foot+/- project route from a re-pump station located in the vicinity of Lehman Road, easterly to the City of Sidney s Water Treatment Plant located at 880 East Court Street, Sidney, Ohio as shown on the included map. Plans to be designed to ODOT, City of Sidney, Shelby County, and OEPA standards and specifications. Plans shall include maintenance of traffic, erosion/sedimentation plans, and pavement restoration plans where necessary. Preparation and submittal of all required permits and required exhibits on behalf of the City including, but not limited to: ODOT, CSX Railroad, ODNR, OEPA, Army Corps of Engineers, FEMA, and Shelby County. Fees to be paid by the City. Itemized Engineer s Estimate utilizing ODOT specifications. Each item must be quantified, no incidental or lump sum items will be allowed without prior permission from the City of Sidney Engineering Department. Supplemental specifications all project specifications required for completion of this project that are not covered by ODOT design criteria or the City of Sidney standards must be prepared by the consultant and provided to the City in an electronic format. Plans must include a minimum of the following: o Grading Plan with existing and proposed contours. o Cross sections and earthwork computations. o Typical design sections with material composition. o Utility plan with pipe profiles. o Erosion control plan. o General and detailed notes sheets. o Crossing plans for I-75, Great Miami River, and CSX Railroad. o Cross sections where needed for clarity of intent. Coordination with CH2MHill for system curves and hydraulics. Coordination with utility companies, including but not limited, to electric, telephone, cable and gas. Soil borings including one at each side of I-75 and the Great Miami River crossings and others as needed and including final report and recommendations. Pipe material analysis and recommendations. Necessary survey work including but not limited to field reconnaissance, traverse of existing monumentation, calculation of route centerline, establishment of benchmarks, staking controls and complete topographic survey along the project route including the location of all land features and underground and overhead utilities. Topographic survey shall be a minimum of 100 feet in width along the transmission line corridor including

3 any pertinent features beyond this width that may be relevant to the design of the project. Conventional, LiDAR & Aerial surveys will be considered for this project provided the following accuracy is maintained: 0.05 feet horizontal, 0.05 feet vertical for paved areas and 0.20 feet vertical for vegetated areas. Consultant surveyor shall be Ohio Licensed and must provide certification of accuracy to the City. Survey data must conform to the following criteria: Vertical Orthometric Height Datum: NAVD88 Geoid Model: GEOID12A Horizontal Coordinate System: Ohio State Plane, North Zone Map Projection: Lambert Conformal Conic Reference Frame: NAD83 (2011) Ellipsoid: GRS80 Combined Scale Factor: Units All units are US Survey Feet. Final construction plans to include title sheet, schematic plan, general notes, details, quantities, and plan and profile sheets (1 = 20 horizontal, 1 =5 vertical) Final delivered product shall include at a minimum the following: Complete construction plans & specifications in hard copy, Adobe PDF, and AutoCAD DWG format. AutoCAD format shall include the overall base drawing file, along with a complete set of individual plan sheets. Soil boring logs & reports Mapping & Survey quality reports for Aerial or LiDAR surveys (if used). Final construction plans to include title sheet, schematic plan, general notes, details, quantities, and plan and profile sheets (1 = 20 horizontal, 1 =5 vertical) City will acquire any required easements and provide any benchmark and property information data available and will provide copies of any and all easements acquired or under acquisition for the project. This project is 100% locally funded. The Schedule for design of this project is as follows: September 25, 2014 Consultant qualifications due by 3pm. October 1-3, 2014 Consultant interviews (If required). October 6, 2014 Consultant selection and notification to proceed with contract negotiations. October 15, 2014 Project kick off meeting. December 17, 2014 First review plans & estimate due. March 11, 2015 Second review plans & estimate due. Permit applications must be submitted. April 15, 2015 Final plans, supplemental specifications & engineer s estimate due. April 30, 2015 Project advertises for competitive bidding.

4 It is the intent of the City to select a Consultant to perform all services required for completion of the work included in this request. All submissions must be no more than ten pages (stapled, no binding please). Do not include qualifications of personnel that will not be involved with the design of this project. Provide a separate sealed envelope which shall contain a price for design and survey and a separate price for bidding assistance and construction administration. Submissions will be evaluated internally by a selection committee based on the following criteria: 1. Professional Qualifications pts. 2. Qualifications of key individual s pts. 3. Ability to complete entire project on schedule pts. 4. Familiarity with project area and requirements pts. 5. Past performance on similar projects (References) pts. 6. Recent Similar Experience pts. 75 pts. Consultants must submit five (5) copies of their response to the City of Sidney, 201 W. Poplar St., Sidney, Ohio 45365, Attention: Mr. Gary R.
