5 8.1. Maintenance All Total Oilfield NSC commercial vehicles shall be maintained in accordance with the National Safety Code and applicable provincial legislation and company policies. This program will address the following areas: Daily trip inspections Repairs and maintenance Scheduled inspections and maintenance CVIP inspections (annual and semi-annual) Record keeping as required by legislation The preventative maintenance and inspection program and policies will apply to all NSC commercial vehicles registered to Total Oilfield, solely or in combination, for a weight of more than 4500 kilograms. Lease operators are responsible for implementing and following their own maintenance programs. A person shall not operate or permit another person to operate a commercial vehicle if that vehicle or equipment related to the vehicle is in a condition likely to cause danger to persons or property. It is EACH driver s responsibility to ensure that the vehicle they are operating is properly maintained and in a safe condition. All vehicles are to be kept as clean as possible, inside and out, in order to provide a safe and enjoyable working environment as well as maintaining the company s image to customers and the public. REMEMBER, EACH TRUCK IS A MOVING ADVERTISEMENT. The company s written maintenance program will be kept at the principle place of business in both Alberta and Saskatchewan. Copies of the maintenance program will be available at each location of Total Oilfield where maintenance and inspection activities are carried out, and shall be readily accessible to those employees who carry out the maintenance program. Schedule 2 of the Commercial Vehicle Safety Regulation (AR 121/2009) will be made available at all locations where this maintenance program is applicable Equipment Inventory Total Oilfield will establish and maintain an inventory of their machinery and equipment Daily Trip Inspections When driven, a pre and post trip inspection shall be conducted on all commercial vehicles or combination of commercial vehicles registered for more than 4500 kilograms Vehicle/Trailer Maintenance Page 5
6 8.4. NSC Standard 13 Part 2 Schedule 1 The trip inspection must include all applicable components listed in NSC Standard 13, Part 2 Schedule 1. Any of the components that are routinely inspected may be added to the trip inspection form, and any components not applicable may be deleted from the trip inspection form. Total Oilfield shall ensure that a copy of Schedule 1, of NSC Standard 13 Part 2 is located within the vehicle (in truck binder and printed on driver s daily log book). Drivers MUST be able to produce Schedule 1 upon request of a peace officer Daily Trip Inspection Report (Pre/Post Trip) The person (driver) conducting the pre/post trip inspection must fully complete a written trip inspection report at the start of the trip. That report MUST CONTAIN THE FOLLOWING; The name of the registered owner of the commercial vehicle The power unit [& trailer(s) if applicable] licence plate, identification number OR unit number The odometer or hub meter reading at the time of inspection The name of the municipality or location on the highway where the inspection was conducted, including province (i.e. Whitecourt, Alberta) The year, month, day and time that the inspection report was completed Any defects found at the start, end and/or throughout the trip in relation to the items required to be inspected, or that No Defects Were Found The name and signature of the person inspecting the vehicle(s) declaring that the vehicle(s) were inspected in accordance with NSC Standard 13 and applicable legislation The name and signature of the person making the trip inspection report If the unit has a crane, the driver will also complete the Picker/Boom Truck Daily Inspection Log. The driver shall produce this trip inspection report to a peace officer upon request Vehicle/Trailer Maintenance Page 6
7 8.6. Trip Inspection Reporting and Repair Requirements No person shall allow a driver to drive and no driver shall drive a commercial vehicle with any uncorrected or unrepaired MAJOR DEFECT as identified in Schedule 1. A person authorized by the carrier to conduct a daily trip inspection shall certify on the trip inspection or report that any major defect has been repaired/corrected or certifies on the report that the repair/correction is unnecessary. A driver or person authorized by the carrier to conduct a daily trip inspection shall document any defect on the written trip inspection report. Defects shall be reported to the carrier as per the following; Immediately if the defect is a MAJOR DEFECT, or In a timely manner and not later than the next required daily trip inspection, in all other cases If a driver observes any defects specified in Schedule 1 while driving the vehicle, the driver shall record the defects on the trip inspection form and report the defects to the carrier as required. The vehicle CANNOT be driven if a MAJOR DEFECT is found. Trip Inspection reports are to be turned in daily at the driver s Branch Office. They must then be forwarded to the principal place of business within 30 days of receipt and filed for each vehicle in chronological order for at least 6 months from the date received. The mechanic reviews the trip inspection reports with the dispatcher/manager at the start of the day. It is then decided if it is a major or non-major defect & repair. The mechanic opens a work order listing all repairs. At this time the mechanic will stamp the trip inspection report and write the work order number and date on the trip inspection report. The trip inspection report will then be sent to the principal place of business for filing (unless otherwise authorized by the Registrar). All major defects are repaired immediately before the vehicle can be dispatched. If the vehicle is deemed safe to go with non-major items as listed in Schedule 1 of NSC Standard 13, those non-major items will be repaired prior to the next required trip inspection. Work order items are dated as per the repair. All completed work orders, and 3 rd party invoices if applicable, are sent to the principal place of business for filing (unless otherwise authorized by the Registrar) Vehicle/Trailer Maintenance Page 7
8 8.7. Inspections and Maintenance Schedule The scheduled inspections and maintenance as described below applies to all NSC commercial vehicles registered to Total Oilfield. It will be carried out in accordance with applicable legislation, schedule 2 of AR 121/2009 and manufacturer requirements. The applicable SERVICE CHECKLIST must be fully completed & attached to a work order. ALL information must also be complete on the work order. 9 8.8. PMVI and CVIP Inspections CVIP inspections will be completed as per the legislative requirements of Alberta, and PMVI inspections will be completed as per the legislative requirements of Saskatchewan as identified in the company s Inspections and Maintenance Schedule. In order to ensure these inspections are completed on time, a record of these inspections is kept on a data base and monitored by the Safety Department in Whitecourt, Alberta. A notice is sent out monthly advising all Branch Offices of PMVI/CVIP inspections coming due in the next month. For a yearly CVIP inspection (AB), complete both the CVIP form and copy of the Commercial Vehicle Record of Inspection. It is extremely important that the CVIP Certificate is filled out correctly. If the CVIP is not filled out correctly, it will be considered invalid and the vehicle will have to be re-inspected and recertified. All fields must be completed on the CVIP Certificate. Ensure the Owner s Name is exactly the same as what appears on the vehicle registration. Upon completion of a PMVI/CVIP inspection, the ORIGINAL copy MUST be placed in the truck binder of the power unit or in the document holder of the trailer Maintenance Records Total Oilfield shall maintain in a unit file the following records pertaining to each NSC commercial vehicle registered to them; Unit number, serial number (VIN), make, model, year, registered certificate weight, license plate number (COPY OF VEHICLE REGISTRATION/CAB CARD) Copy of the bill of sale to identify when the vehicle was put into service, and to verify the current year plus the required number of years of inspections and maintenance records Copy of the current and previous years PMVI/CVIP Inspection forms Copy of crane certifications and inspections when applicable Record of axle or suspension modification affecting the gross vehicle or axle weight rating A copy of warranty work or manufacturer recalls and repairs A copy of repair/parts invoices for major repairs All pre/post trip inspection reports for power units and trailers A record of ALL Service Inspections & Checklists, CVSA inspections, Work Orders and other related records of repair, lubrication and maintenance for the vehicle. These records must include; Nature of the inspection or work performed Day/month/year the inspection/work took place Odometer & hour-meter at time of work (power units) Hub-meter reading at time of work (trailers) Vehicle licence plate number/unit number 8.0. 10 Unless otherwise authorized by the Registrar (Divided Records Authority), these records will be maintained at the principal place of business. All maintenance, repair, inspection and lubrication records will be filed by vehicle in chronological (date) order. These records will be kept for a minimum of 5 years or for at least 6 months after a vehicle has been removed from the fleet Tire and Wheel Maintenance Tire Servicing Tires on all company NSC vehicles must be inspected during every pre & post trip inspection. Only competent personnel from a licensed tire servicing company may perform maintenance on tire and wheel assemblies to ensure the manufacturer s specifications are met. Total Oilfield will not permit the use of re-tread/recap tires on any NSC vehicles registered to them Wheel Torque Guidelines All wheel fasteners will be checked using a torque wrench as identified on the scheduled Truck and Trailer Inspection Reports. When any repairs are made requiring the wheel fasteners to be loosened or removed, a re-torque tag will be attached to the vehicle identifying which wheel fasteners need to be re-checked. This tag will be attached to the keys of a power unit and to the air lines or power cord of a trailer. Wheels are to have a re-torque check done as close as practical too, or within the first 100 kilometres, at any Total Oilfield shop or tire service facility. The re-torque information will be documented on the re-torque tag, which will then be sent to the principal place of business for filing in the vehicle maintenance records. It is important to clearly document each time wheel fasteners are checked using a torque wrench Cranes/Pickers and Rigging Drivers are required to inspect cranes/pickers prior to daily use. Rigging is also to be inspected for wear, damage and missing inspection tags. The Picker / Boom Truck Log Book Daily Inspection form must be completed when conducting this inspection. All pickers/cranes will be submitted to a yearly inspection for re-certification. Current certificates will be kept in the truck information binder. A copy of all daily and annual inspections will be kept in the vehicle maintenance records Vehicle/Trailer Maintenance Page 10
11 8.12. Non-NSC/Miscellaneous Maintenance Rental Equipment Maintenance & Inspection All Total Oilfield rental equipment must be in good working order before it is rented out. Upon return, equipment must be thoroughly cleaned, and any repairs or maintenance must be completed before it is rented again. Every effort will be made to ensure the customer receives equipment that is clean, in good repair, painted and of the highest standards. When rental equipment is returned containing drilling and work over fluids, arrangements will be made for product removal and disposal at an AEUB or other approved location Yard Equipment Maintenance & Inspection A detailed repair and maintenance file will be kept for all company forklifts, loaders, etc. These vehicles will be maintained in accordance with manufacturer and regulatory requirements. Each branch office will keep a file on each unit and a copy will be sent to the Whitecourt Safety Office Light Duty Vehicle Maintenance All non-nsc company vehicles (pick-ups and other related vehicles) will be maintained in accordance with the manufacturer and regulatory requirements Vehicle Safety Equipment All of the following should be found in every First Aid Kit Fire Extinguisher Spill Response Kit Tow Rope Booster Cables Flash Light company vehicle; Medium and Heavy Duty vehicles must also be equipped with Warning Triangles Vehicle/Trailer Maintenance Page 11
