PRECEDENCE Addenda shall form an integral part of the Specification and to be read in conjunction therewith. Addenda shall take precedence over all forms of the aforementioned Specifications and Drawings with which it may prove to be at variance or may otherwise be qualified in writing by authorized personnel. 2. GENERAL The General Conditions and all documents issued with this Specification shall apply and govern all phases of the Work covered by Addenda Contractors are advised to acknowledge receipt of all addenda on Page 3, Item 9 of the Tender Form when submitting a bid. 3. PURPOSE The Purpose of this Addendum is to inform bidders of clarifications, changes and additions to the Contract Documents and Drawings. 1.0 Please see revised specifications as per the attached. 3 1.0 INTRODUCTION Central Health (Lessee) is looking to lease two (2) residential homes in the area of Fogo Island, NL. These homes must be located within one (1) kilometer of the Fogo Island Health Center. Bidders are to submit a monthly lease rate for each home based on the requirements outlined in this tender document. Lease for each home to be based on a 5 year term with the option to renew for additional 3 to 5 years. 1.1 House Requirements Size: House # 1: House # 2: 1,800 sq. ft. with attached 600 sq. ft. self-contained apartment. 1,800 sq. ft.. Style: Bungalow. One level with crawl space Layout: Master Bedroom 14-0 x 14-0 (3) Bedrooms 10-0 x 12-0 (2) Washrooms 8-0 x 8-0 Dining Room 10-0 x 12-0 Living Room 12-0 x 16-0 Kitchen 10-0 x 14-0 Doctors Office 8-0 x 8-0 Laundry Room 8-0 x 8-0 Apartment Layout: (1) Bedroom 12-0 x 12-0 (1) Bedroom 10-0 x 12-0 Washroom 6-0 x 8-0 Kitchen 8-0 x 8-0 Living Room 10-0 x 16-0 Heating: Land: Baseboard Heaters Provided by lessor located within one kilometer range of Fogo Island Health Center. Bid to include interior wooden blinds for all windows, both units. All doors to be 3-0 wide minimum. Maintenance: Lessor to be responsible for all maintenance on house and the land including maintaining the lawn. All exterior stud walls to consist of 2"x6" studs, bracing between studs, and have double top plates. 3. All bedroom windows to meet egress code. Windows to have minimum unobstructed opening of 3.77 sq. ft., with no dimension less than 1' -3". Change types of window if necessary. 4. Built up beams to have minimum of 3.5" of bearing for columns within walls. 5. Lintels to have minimum of 1.5" bearing for lintel spans up to 10', or minimum of 3" of bearing for lintel spans greater than 1 0'. Unless otherwise specified. 6. All concrete foundation walls and floor slabs to have a compressive strength of 20mpo or 2175 psi after 28 days. 7. Footing shall rest on undisturbed soil, rock or compacted granular soil. 8. Building design base on specified snow load of 2.50kpo or psi. 9. All interior load bearing walls (if used) to consist of 2"x4" studs 16" o.c. As shown. Bracing between studs and have double top plate all load bearing walls to have ½ gypsum board on each side. Unless otherwise noted. 10. Where bottom of footings are less than 4 ' -0" below finished grade, footing to be frost protected with 1-½ rigid insulation extending 4' out from foundation walls. 11. Soffit to be non-perforated on gable ends. 12. All bottom plates on concrete wall to have sill gasket. 13. Crawl space to be vented by mechanical ventilation from hrv unit. 14. Attic hatch to be insulated and be a minimum size of 21.5" x 35.5". 15. Floor to be glued and screwed to floor joists. 16. All lumber to be SPF 2 or better. 4

All lumber to be SPF 2 or better. 4.0 Submittal / Approval Requirements Prior to signing of Lease by Central Health; - Central Health to approve on layout of homes. - Central Health to carry out on site inspections to ensure homes meet their needs and requirements. 4.1 Inquiries during the Tender Period To ensure the integrity of the competitive bid process, inquiries and other communications regarding this procurement, from the issue date of the solicitation up to the award date of the contract, are to be directed only to the Lessee s Project Manager for the Project, as noted below: Mr. David Perry, P.Eng. Failure to comply may result in bid disqualification. Inquiries must be submitted in writing by the prime contact of a Lessor. Inquiries must be received no less than 7 calendar days prior to the bid closing date to allow sufficient time to provide a response. Inquiries received after that time might not be answered prior to the bid closing date. 5

11 To ensure consistency and quality of information provided to Lessor with respect to significant inquiries received, and the replies to such inquiries, any information will be provided simultaneously to Lessor to which this solicitation has been sent, without revealing the sources of the inquiries. Commercially confidential inquiries will be entertained and treated as such but decision to respond will be solely at the Project Manager s discretion. 4.2 Response costs for Bids The Lessee will not be liable nor reimburse any Lessor for costs incurred in the preparation of bids, proposals or any other services that may be requested as part of the evaluation process. The Lessor agrees that the Lessee s sole obligation, in return for the Lessor s preparation and submission of its Proposal, is to give consideration to the Proposal in accordance with the Tender documents. The Lessee and any of its employees, assigns, agents or representatives shall not be liable to the Lessor or any of its officers, employees, assigns, independent Lessors, sub- Lessors, agents or representatives for any losses, expenses, costs, claims, damages, including incidental, indirect, special or consequential damages or liabilities arising out or by reason of or attributable to this Tender including, without limitation, the cost of preparing or submitting a Bid and any anticipated profits and contributions to overhead. 4.3 Treatment of information The information collected will be used solely for the purposes stated in this Tender. Any information submitted by a Lessor that is to be considered confidential must be clearly marked as such. The Lessee maintains the right to make copies of all bids for its internal evaluation process and provide copies to the Evaluation Committee, and staff advisors and representatives of the Lessee and other project stakeholders, which may support the Lessee and/or Evaluation Committee. The Lessor does, by the submission of a Bid, accept that the information contained in it will be governed in accordance with the process set out in the Tender. 4.4 Identity and legal capacity of Lessor The Lessor, regardless of the business name under which it may be operating, is expected to have the legal capacity to enter into binding contracts with the Lessee or other relevant agencies. 6

12 In this regard, the Lessor shall be expected to produce proof of legal capacity (such as Articles of Incorporation, or a copy of the registration of the business name of a sole proprietor, of a trade name, of a partnership, etc.) upon request. 4.5 Licensing Requirements Bidder shall be, or be eligible to be licensed, certified or otherwise authorized to provide the necessary professional services to the full extent that may be required by provincial law in the Province of Newfoundland & Labrador. By virtue of submission of a bid, the Lessor certifies they are in compliance with these requirements. The Lessor acknowledges that the Lessee reserves the right to verify any information in this regard and that false or erroneous certification may result in the Bid being declared non-responsive. 4.6 Insurance requirements Lessor to carry current Rental Property Insurance on both properties. Copy of valid certificate/policy to be provided to Lessee, and updated copy on yearly basis. 4.7 Landlord Responsibilities Lessor to comply with NL Landlord Tenant Act as set out by Government of Newfoundland, Service NL Conflict of interest The Lessee reserves the right to disqualify any Lessor that, in the sole opinion and unfettered opinion of the Lessee, has an actual, perceived or potential conflict of interest or an unfair advantage, whether existing now or likely to arise in the future. 4.9 Addenda Addenda may be issued during the bidding period and all addenda become part of the contract. If any document included in this offering or subsequent document issued contains any discrepancy, omission, inconsistency or ambiguity, the Lessor must notify the Project Manager immediately. Upon receipt of such notification, the Project Manager will undertake to provide all Lessors with direction via addenda. If a Lessor fails to provide such notification, the Lessor agrees that the interpretation subsequently rendered by the Project Manager will govern. 7

13 Lessor may submit questions for clarification regarding the contents of the Tender and/or subsequent addenda in writing only via to fax ( ) to the Project Manager up to 4 pm on the dates noted in Section 4.1.
