2 INVITATION FOR BIDS ( IFB ) NOTICE TO BIDDERS ISSUANCE DATE: March 1, 2016 IFB NO. PROJECT NAME: CLOSING DATE/TIME: ASPA WTR UTULEI & VATIA PRV CONCRETE VAULT CONSTRUCTION March 15, 2016, 10:00 a.m., American Samoa Time The American Samoa Power Authority (ASPA) hereby issues this Invitation for Bids ( IFB ) to evaluate and award a one time contract for the Utulei & Vatia PRV Concrete Vault Construction as per attached plans and drawings. Bidders shall provide sufficient written and verifiable information that responds to the requirements set forth herein and in the Scope of Work ( SOW ). The deadline for any questions/clarifications is March 7, 4:00 pm. Qualified Bidders must submit bids and forms in a sealed envelop, box, or other enclosure addressed to the ASPA Procurement Manager. Bids must be submitted to the ASPA Procurement Office no later than 10:00 a.m. on or before March 15, The sealed envelope or box must be labeled IFB No. ASPA WTR Utulei & Vatia PRV Concrete Vault Construction. Late submittals will not be opened or considered. For more information about this IFB, you may contact: Ioana S. Request additional information from any Bidder submitting a bid; 3. Select a firm for award based on other than least cost (e.g. capability to complete work in a timely fashion or based on proven technical capabilities); 4. 3 AMERICAN SAMOA POWER AUTHORITY GENERAL TERMS AND CONDITION I. SOLICITATION The American Samoa Power Authority ( ASPA ) issues this IFB in order to solicit bids for the as detailed in Attachment A Specifications & Drawings. II. SCOPE OF WORK The selected bidder is responsible for, but not limited to specification and drawing on Attachment B. III. PREPARATION INSTRUCTIONS Bid Price Bidder must set forth unit and total cost for Scope of work as stated on Attachment C - Bid Transmittal Form. ASPA will award this IFB in whole or in part to selected bidder(s). IV. CONTRACT TERM A Purchase Order will be issued to the awarded Bidder. V. BASIS FOR AWARD The awards will be made by ASPA in accordance with the evaluation criteria set forth herein and with Section of ASPA Procurement Rules. The award evaluation shall be documented and a determination shall be made towards Bidders whose bids are susceptible of being selected for award. ASPA reserve the right to make the award to the bidder who submits the bid, which meets the requirements, set forth herein and best meets the needs of ASPA after taking into consideration the aforementioned factors. ASPA also reserves the right to select portions of a bid, or to reject any and all Bids. The Procurement Manager shall notify the winning bidders accordingly. VI. WITHDRAWAL OF BID Any Bid may be withdrawn prior to the scheduled time for the opening of the bids by notifying ASPA in a written request. No bid may be withdrawn after the time scheduled for opening of bids. VII. OPENING AND COMPARISON OF BID Page 3 of 14
4 In accordance with procurement Rule 3-110, bids will be opened and recorded as part of the record on the date and at the time indicated above at the Procurement Office in Taunt or in another place designated by ASPA Procurement Manager in writing. VIII. IFB CONDITIONS ASPA reserves the right to issue any addendum to this IFB. Bidders shall send ASPA a signed form confirming receipt of any addendum, and shall submit supporting/additional information as required by any addenda. In the event that any bidder fails to acknowledge receipt of any such addendum in the space provided, his or her bid shall be considered irregular and will be accepted by ASPA only if it is in ASPA s best interest. In the event that addenda are not received until after the Bidder has submitted his or her bid, a supplementary bid may be submitted revising the original bid. Such supplementary bid must be received by ASPA prior to the scheduled time for opening of bids. IX. EVALUATION CRITERIA Bids will be evaluated by the Procurement Division, Requesting Division and approved by the Procurement Manager. Submission of a bid shall constitute a waiver of any challenge or dispute of the review process as well as the choice of methodology for selection of award. X. INDEMNITY PROVISION The successful bidder shall indemnify and hold harmless the American Samoa Power Authority from against any and all claims, damages, losses, and expenses, including attorney s fees, arising out of or resulting from the performance of the work, providing that such claim, damage, loss or expense is attributable to bodily injury, sickness, disease, death or damage to property of whatsoever nature, including the loss of use resulting there from, and whether or not is caused in whole or in part by any negligent act or omission of the successful bidder, its subcontractors, or anyone employed by any of them or anyone for whose acts any of the maybe liable, regardless of whether or not it is caused in part by the negligence of ASPA and/or the strict liability of ASPA, its agents and employees. XI. AWARD, CANCELLATION & REJECTION Page 4 of 14 Award shall be made to the lowest responsible and responsive bidder, whose bid is determined to be the most advantageous to the American Samoa Power Authority, taking into consideration the evaluation factors set forth in this solicitation. No other factors or criteria shall be used in the evaluation. The right is reserved as the interest of ASPA may require waiving any minor irregularity in bids received. The Procurement Manager shall have the authority to award, cancel, or reject bids, in whole or in part for anyone or more items if determines it is in the public interest. Award issued to the lowest responsible bidder within the specified time for acceptance as indicated in the solicitation, results in a binding contract without action by either party. In case of an error in the extension of prices, unit price will govern. It is the policy of ASPA to award contracts to qualified bidders. ASPA reserve the right to increase or decrease the quantity of the items for award and make additional awards for the same type items and
5 the vendor agrees to such modifications and additional awards based on the bid prices for a period of thirty (30) days after the original award. No award shall be made under this solicitation which shall require advance payment. Page 5 of 14
6 ATTACHMENT A PRESSURE REDUCING VALVE VAULT SPECIFICATIONS AND DRAWINGS 1. CMU WALL. 1.1 CONCRETE MASONRY UNITS All Workmanship and materials shall be in accordance with the latest edition of Uniform Building code and ACI CMUs shall have a min compressive strength at 28 days of 1500 psi. Mortar shall have a min compressive strength of 1500 psi. All CMU shall be 6" to match the existing materials and shall be filled with 2000 psi grout. All units shall be sound and free of cracks or other defects that interfere with the proper placement of the unit or significantly impair the strength or permanence of the construction CMU shall be constructed in running patterns throughout and shall be wetted during placement The initial bed joint shall not be less than 1/4" in thickness or more than 1" thick, subsequent joint shall not be less than 1/4" or more than 1/2" in thickness. 2. 8 aggregate, cement, water reducing and plasticizing agents; capable of developing minimum compressive strength of 2,400 psi in 48 hours and 7,000 psi in 28 days JOINT DEVICES AND FILLER MATERIALS Joint Filler: ASTM D1751, ASTM D994, asphalt impregnated fiberboard, closed cell polyvinyl chloride, molded vinyl foam or pre-molded sponge rubber Construction Joint Devices: Integral galvanized steel or extruded plastic Expansion and Contraction Joint Devices: ASTM B221 alloy, extruded aluminum; resilient neoprene filler strip with a Shore A hardness of 35 to permit plus or minus 25 percent joint movement with full recovery CONCRETE MIX PART 3 - EXECUTION Mix and deliver concrete in accordance with ASTM C94 and ACI Use accelerating admixtures only when approved by Engineer Use set retarding admixtures during hot weather only when approved by Engineer Add air entraining agent to normal weight concrete mix for work exposed to exterior Provide Engineer with tickets indicating mix times, delivery and additives to the mix. 3.1 EXAMINATION Verify site conditions and verify requirements for concrete cover over reinforcement Verify that anchors, seats, plates, reinforcement and other items to be cast into concrete are accurately placed, positioned securely, and will not cause hardship in placing concrete. 3.2 PREPARATION Prepare previously placed concrete by cleaning with steel brush and applying bonding agent in accordance with manufacturer's instructions In locations where new concrete is dowelled to existing work, drill holes in existing concrete, insert steel dowels and pack solid with non-shrink grout. 9 3.3 PLACING CONCRETE Place concrete in accordance with ACI 304, ACI 301 and/or ACI Notify Engineer minimum 24 hours prior to commencement of operations Ensure reinforcement, inserts, embedded parts, formed expansion and contraction joints are not disturbed during concrete placement Install vapor barrier under interior slabs on grade. Lap joints minimum 6 inches and seal watertight by sealant applied between overlapping edges and ends or taping edges and ends Install joint devices in accordance with manufacturer's instructions Maintain records of concrete placement. Record date, location, quantity, air temperature, and test samples taken Place concrete continuously between predetermined expansion, control, and construction joints Do not interrupt successive placement; do not permit cold joints to occur Screed floors and slabs on grade level, maintaining surface flatness of maximum 1/4 inch in 10 ft. 3.4 SEPARATE FLOOR TOPPINGS Prior to placing floor topping, roughen substrate concrete surface and remove deleterious material. Broom and vacuum clean Place required dividers, edge strips, reinforcing and other items to be cast in Apply bonding agent to substrate in accordance with manufacturer's instructions Place concrete floor toppings to required lines and levels. 3.5 CONCRETE FINISHING Finish concrete floor surfaces in accordance with ACI In areas with floor drains, maintain floor elevation at walls; pitch surfaces uniformly to drains at 1/8 inch per foot nominal or as indicated on drawings. 3.6 CURING AND PROTECTION Immediately after placement, protect concrete from premature drying, excessively hot or cold temperatures, and mechanical injury Maintain concrete with minimal moisture loss at relatively constant temperature for period necessary for hydration of cement and hardening of concrete. 10 3.6.3 Cure floor surfaces in accordance with ACI FIELD QUALITY CONTROL Field inspection and testing will be performed. Provide free access to Work and cooperate with appointed firm Submit proposed mix design of each class of concrete to Owner for review prior to commencement of Work Tests of cement and aggregates may be performed to ensure conformance with specified requirements. 3.8 PATCHING Allow Engineer to inspect concrete surfaces immediately upon removal of forms Excessive honeycomb or embedded debris in concrete is not acceptable. Notify Engineer upon discovery Patch imperfections as directed and in accordance with ACI DEFECTIVE CONCRETE Defective Concrete: Concrete not conforming to required lines, details, dimensions, tolerances or specified requirements Repair or replacement of defective concrete will be determined by the Engineer Do not patch, fill, touch-up, repair, or replace exposed concrete except upon express direction of Engineer for each individual area SCHEDULE - CONCRETE TYPES AND FINISHES Foundation Walls: 4,000 psi 28 day concrete, form finish with honeycomb filled surface Floors: 4,000 psi at 28 days, form finish with honeycomb filled surface SCHEDULE - JOINT FILLERS Basement Floor Slab Perimeter: Joint filler Type A set 1/8 inch below floor slab elevation Exterior Retaining Wall at Loading Dock: Joint filler Type F recessed 3/8 inch with sealant cover. 3. 12 ATTACHMENT B SCOPE OF WORK The American Samoa Power Authority is requesting the services of a qualified, professional contractor to provide labor, materials, equipment, tools, transportation, supplies and incidentals required for; The excavation and construction of two (2) 6' x 5' x 4.5' "open bottom" concrete vault in Utulei & Vatia, as specified and shown on drawing sheet no. "CO2". The exact location of the above vault will be marked out by ASPA engineers prior to excavation. The work shall comply with all ASPA requirements as specified and as shown in the drawings and specifications provided by ASPA. The contractor will provide all materials and services necessary for the successful completion of the project. ASPA is responsible for all the piping/plumbing work. Project Time Indemnity The contractor shall complete the work within twenty (20) calendar days. The Contractor shall indemnify, defend, and save harmless ASPA, its officers, employees, and agents from and against any claims and demands whatsoever, including costs and attorney s fees, resulting from the Contractor s negligent acts or omission in connection with the performance of the scope of work Clean up As the work progresses and immediately after completion of the work, the contractor shall clean up and remove all refused materials of any kind resulting from the work. The contractor shall not be allowed to deposit any kind of debris into the stream. Page 12 of 14
13 Environmental Pollution Control: The contractor shall conduct the work in accordance with local and federal laws of the EPA. Maintenance Period: The contractor shall agree to maintain the vault for a one (1) year period without additional costs to ASPA. Page 13 of 14
