King, District 3 MEMO FOR RECORD SUBJECT: Quimper Water System Kala Point Consolidation - Documents Addendum #2 The purpose of this memo is to forward Addendum #2 to the Quimper Water System Kala Point Consolidation Bid Package. This addendum provides updated wage rate information, provides better details on the Altitude Valve required, and makes some minor corrections to the bid documents. 1. Davis-Bacon Wages. The 10-day wage check is still WA issued 01/03/14. Enclosed a copy of the most current federal wage information. 2. Dates on pages 2-8 and 2-6 it refers to 2013 and should be Stormwater plan. Because this is a series of small project as defined by the county; and because we already have the county road permit the only storm water requirements are minimum requirement #2, see enclosure 2 this memo. 4. Bid item 6. Foundation Material. Use WDOT spec (1) Gravel Backfill for Foundations. Use Class A. 5. Altitude Valve. There were two different models listed. The required model is: 1-4" 210G-01BYKC altitude control valve. Ductile iron body, bronze trim, epoxy lined and coated, 150# flanged. 6. PRVs. The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is union or non-union. Union Identifiers An identifier enclosed in dotted lines beginning with characters other than "SU" denotes that the union classification and rate have found to be prevailing for that classification. Example: PLUM /01/2011. The first four letters, PLUM, indicate the international union and the four-digit number, 0198, that follows indicates the local union number or district council number where applicable, i.e., Plumbers Local The next number, 005 in the example, is an internal number used in processing the wage determination. The date, 07/01/2011, following these characters is the effective date of the most current negotiated rate/collective bargaining agreement which would be July 1, 2011 in the above example. Union prevailing wage rates will be updated to reflect any changes in the collective bargaining agreements governing the rates. 0000/9999: weighted union wage rates will be published annually each January. 8 Page 6 of 7 Classifications listed under an "SU" identifier were derived from survey data by computing average rates and are not union rates; however, the data used in computing these rates may include both union and non-union data. Example: SULA /13/2010. SU indicates the rates are not union majority rates, LA indicates the State of Louisiana; 2004 is the year of the survey; and 007 is an internal number used in producing the wage determination. A 1993 or later date, 5/13/2010, indicates the classifications and rates under that identifier were issued as a General Wage Determination on that date. Additional guidance and application materials include: Stormwater Calculation Worksheet required application form for all new development and redevelopment; includes flow charts for determining stormwater minimum requirements Construction Stormwater Pollution Prevention (SWPP) Best Management Practices (BMPs) Packet typical sediment and erosion control measures for the construction phase of rural residential development "Low Impact Development" Information Sheet describes site design techniques and on-site stormwater management BMPs that will result in projects that have less environmental impact and that meet stormwater minimum requirements in a way that is significantly simpler for the applicant Stormwater Site Plan Instructions and Submittal Template supplemental application form for projects requiring a Stormwater Site Plan (i.e., "medium" and "large" projects); includes glossary of stormwater management definitions and narrative template for Construction Stormwater Pollution Prevention Plan (SWPPP) component Jefferson County Stormwater Management Standards In an effort to protect water resources in Jefferson County, all new development and redevelopment must conform to the standards and minimum requirements set by the 2001 edition, as amended, of the Washington Department of Ecology Stormwater Management Manual for Western Washington (Manual). Section 6.7 of the Unified Development Code (UDC) establishes Jefferson County's stormwater management standards, with reference to the adopted Manual for definitions, criteria and thresholds for determining stormwater management requirements for a given development proposal. The Manual is comprised of five volumes and can be reviewed at the Department of Community Development (DCD), acquired from the Department of Ecology, or accessed from the Ecology website: There are ten (10) Minimum Requirements in the Manual, as described in section 2.5. The applicability of the requirements to a given project is discussed below. 1. Preparation of Stormwater Site Plans (Chapter 3 of Manual) 6. Runoff Treatment 2. Construction Stormwater Pollution Prevention 7. Flow Control 3. Source Control of Pollution 8. Wetlands Protection 4. Preservation of Natural Drainage Systems and Outfalls 9. Basin/Watershed Planning 5. On-site Stormwater Management 10. Operation and Maintenance What are the Minimum Requirements for my proposal? The two-page Stormwater Calculation Worksheet is the first form the applicant completes and signs. The general category of the project "small," "medium," or "large" is determined through the Worksheet and attached flow charts: Figure 1: Flow Chart for Determining Requirements for New Development and Figure 2: Flow Chart for Determining Requirements for Redevelopment. If the site has 35% or more of existing impervious coverage, then you use the redevelopment flow chart. If the site has less than 35% or more of existing impervious coverage, then you use the new development flow chart. Upon completion of the Worksheet, the project will be classified as "small," medium," or "large," which will, in turn, determine which of the Minimum Requirements from the Manual must be satisfied. Following is a synopsis: "SMALL PROJECTS" must address only Minimum Requirement #2, Construction Stormwater Pollution Prevention. The only submittal required as part of the application is the Stormwater Calculation Worksheet. DCD provides the applicant with a Construction Stormwater Pollution Prevention (SWPP) Best Management Practices (BMPs) Packet. The applicant is expected to ensure that the applicable sediment and erosion control measures are implemented during construction. "MEDIUM PROJECTS" must address only the first five Minimum Requirements. For most rural residential projects, completion of the template in the Stormwater Site Plan Instructions and Submittal Template satisfies the stormwater management component of the application. "LARGE PROJECTS" must address all ten Minimum Requirements. Applicants must submit a Stormwater Site Plan and may need to involve an engineer in addressing runoff treatment and flow control. More information is found in the Stormwater Site Plan Instructions and Submittal Template. Stormwater management standards are applied to all development and redevelopment proposals (unless exempt per UDC). Stormwater review by DCD or by the Department of Public Works occurs either in conjunction with another land use or building permit application or with a stand-alone stormwater management permit. Department staff can help determine what Minimum Requirements apply to your proposal and whether a stand-alone stormwater management permit is required. 12 When is a Stormwater Management Permit required? A stormwater management permit is either a stand-alone permit or incorporated with an associated building permit or other land use approval. If you are not applying for a building permit or some other County land use approval for your proposal, a stand-alone stormwater management permit may be required. If you are applying for another permit, stormwater review is incorporated into consistency review of that permit application and the building permit or other land use approval, if and when granted, also functions as an integrated stormwater management permit, complete with any and all conditions of approval related to stormwater management. Thresholds for determining whether a Jefferson County stormwater management permit is required for a proposed activity are contained in UDC Section A stand-alone or integrated stormwater management permit is required for the following activities: All grading of 500 cubic yards or more; except for those activities exempted under UDC Section b; All clearing and grading activities within or that would adversely affect environmentally sensitive areas such as Critical Aquifer Recharge Areas, Frequently Flooded Areas, Geologically Hazardous Areas, Fish and Wildlife Habitat Areas, and Regulated Wetlands as defined in Section et al of the UDC; Land-disturbing activities of 7,000 square feet or more; or Creation of 2,000 square feet or more of impervious surface. Class IV-G Forest Practices and mineral extraction proposals: A Jefferson County stormwater management permit is required in conjunction with a Class IV-General (or "conversion") Forest Practices Application (FPA) reviewed by the Washington State Department of Natural Resources and for a mineral extraction proposal that exceeds the thresholds above. Refer to UDC Sections 4.16 and 4.24, respectively, for regulations specific to forest practices and mineral extraction. Process If a stand-alone stormwater management permit is required or an integrated stormwater management permit is required in conjunction with a building permit or other land use approval, the applicant will submit a Master Land Use Application (MLA) form in conjunction with the Stormwater Calculation Worksheet. For "medium" and "large" projects, a Stormwater Site Plan is a required submittal (Minimum Requirement #1). The UDC Administrator will issue the stormwater management permit consistent only upon a finding that the proposed use or activity meets all applicable requirements of UDC Sections 6.6 (Grading and Excavation Standards) and 6.7 (Stormwater Management Standards) and any and all other applicable requirements of the UDC. The Administrator may require additional measures, such as the Optional Guidance conditions of Section 2.6 of the Manual, pursuant to environmental review of the proposal. Adjustments, Exceptions or Variances to the Minimum Requirements shall be considered pursuant to the UDC and Sections 2.7 and 2.8 of the Manual. A stand-alone stormwater management permit application is reviewed consistent with a Type I (administrative) permit process. Exemptions Grading Activities Exempted from Stormwater Management Permit UDC Section 6.6 contains Jefferson County Grading and Excavation Standards. Per UDC Section b, the following activities are exempt from the requirement for obtaining a stormwater management permit (if located outside of environmentally sensitive areas and their buffers), but are not exempt from Minimum Requirement #2 of the Manual, the 12 Elements of Construction Stormwater Pollution Prevention (SWPP), unless exempted per UDC Section 6.7.2: Maintenance of gravel roads; Construction of a residential driveway categorically exempt from review under the State Environmental Policy Act (SEPA); Construction of a Class I-Ill logging road (per RCW and WAC 222); Construction of drainage improvements in accordance with UDC Sections and 6.7; or Construction of a pond of one-half acre or less which is not in a regulated wetland. Activities Exempted from Stormwater Management Standards Pursuant to UDC Section 6.7.2, the following activities are exempt from meeting the minimum requirements: (1) Commercial agriculture or, (2) Forest practices regulated under WAC 222, except Class IV-General forest practices and Conversion Option Harvest Plans. Stormwater Disposal Policy Along Marine Shorelines Pursuant to UDC section d(2)ii all new development activity, including single-family residential construction, located on Marrowstone Island, Indian Island, and within % mile of any marine shoreline shall be required to infiltrate all stormwater runoff onsite. The Administrator will consider requests for exceptions to this policy on a case-by-case basis and may require a geotechnical report. This provision is intended to prevent seawater from intruding into underground aquifers, reduce non-point source pollution to marine waters, and reduce the potential for shoreline erosion due to tightline failure. This provision is interpreted as establishing a hierarchy in which the first and best alternative is on-site infiltration using drywells or other methods, the second best alternative is upland off-site disposal, and the least preferred alternative is direct discharge into marine waters through a stormwater tightline. In order to utilize the least preferred alternative, which is considered an exception to the policy, applicants must demonstrate through a geotechnical or similar report prepared by a licensed professional that both on-site infiltration and upland off-site disposal are not practicable or feasible. The report must include cost figures for comparison. If based on geotechnical evidence the Administrator grants an exception to this policy, a proposed stormwater runoff tightline that terminates at or near the marine shoreline shall be reviewed under the Jefferson County Shoreline Master Program and SEPA. Other agency approvals, such as Hydraulic Project Approval from the Department of Fish and Wildlife, may be required as well. A Joint Aquatic Resources Permit Application (JARPA) form would need to accompany any permit application that includes a stormwater tightline at or below the ordinary high water mark. stormwater mgmt info sheet.doc REV. 10/12/2003 2

13 JEFFERSON COUNTY DEPARTMENT OF COMMUNITY DEVELOPMENT 621 Sheridan Street Port Townsend Washington / / Fax Construction Stormwater Pollution Prevention Best Management Practices (BMPs) Packet For "small" projects (as determined through the Stormwater Calculation Worksheet), the applicant shall consider the twelve Construction Stormwater Pollution Prevention elements and implement applicable BMPs. A set of useful BMPs for typical rural residential construction is attached. There is no additional submittal required as part of the permit application. For "medium" and "large" projects, the Construction Stormwater Pollution Prevention Plan (SWPPP) is part of the overall Stormwater Site Plan required as part of the permit application. Refer to the Stormwater Management Information Sheet for guidance. The following twelve elements must be considered for Construction Stormwater Pollution Prevention before and during the construction phase of the project: 1. Mark Clearing Limits 7. Protect Drain Inlets 2. Establish Construction Access 8. Stabilize Channels and Outlets 3. Control Flow Rates 9. Control Pollutants 4. Install Sediment Controls 10. Control De-Watering 5. Stabilize Soils 11. Maintain Best Management Practices 6. Protect Slopes 12. Manage The Project Each of the twelve elements is described in more detail below: 1. Mark Clearing Limits By minimizing the limits of clearing on the site, a builder can minimize stormwater runoff and provide effective control of pollution. 2. Establish Construction Access Much of the sediment that leaves a construction site does so on the wheels of delivery and construction vehicles that drive off a project site. Construction access must be limited to a single location and a properly constructed Stabilized Construction Entrance (BMP C105) should be included on the site. 3. Control Flow Rates Stormwater that leaves a project site unimpeded may exceed the capacity of the existing stormwater control facilities downstream and may contain sediment that may be deposited as the velocity of the runoff decreases. Stormwater protection on a construction site should include measures to control the flow rate of runoff from the site. This can be done by installing a Sediment Trap (BMP C240) or other measure that will impede the flow of water off a construction site. 4. Install Sediment Controls In addition to limiting the rate of stormwater flow off a construction site, measures should be put in place to treat the runoff and remove sediment. Limiting of the cleared area (Element 1) will assist in this effort, but there will be exposed soils that may move with the runoff. Suggested BMPs for controlling sediment include Straw Bale Barriers (BMPC230), Brush Barrier (BMP C231), Gravel Filter Berm (BMP C232), and Silt Fence (BMP C233). 14 5. Stabilize Soils An additional measure that can minimize sediment transport in runoff is to stabilize soils on the site with mulch or some other covering. This will limit the amount of soil that is exposed to rainfall, thus limiting the sediment that could potentially leave the site. BMPs that could be used for this include Mulching (BMP C121), Nets and Blankets (BMP C122), and Plastic Covering (BMP C123). During periods of dry weather dust can become a problem and sediment could be transported from the site in high winds. BMP C140 Dust Control should be followed to limit loss of soils in windy conditions. 6. Protect Slopes If the cleared area includes slopes of 3:1 (Horizontal: Vertical) or steeper, the slopes should be protected to limit runoff. If the slopes are not protected, rills and gullies may form, transporting sediment to the lower elevations and potentially off the construction site. The slopes should be graded to minimize erosion and runoff at the downstream end of the slopes, and runoff should be collected and treated. The following BMPs could be used Surface Roughening (BMP C130), Interceptor Dike and Swale (BMP C200), and Pipe Slope Drains (BMP C204). 7. Protect Drain Inlets Runoff from urban construction sites often discharges into existing stormwater collection systems. Water enters the collection system through drain inlets. If there are drain inlets downstream of a construction site, they should be protected using BMP C220 Storm Drain Inlet Protection. 8. Stabilize Channels and Outlets Any temporary on-site channels or ditches that are used to control runoff should be stabilized to prevent erosion in the channel. BMP C202 Channel Lining and BMP C209 Outlet Protection should be used. 9. Control Pollutants The best way to control pollution is to limit the source of pollution. Construction debris should be maintained in a safe location. Vehicle maintenance on the construction site should be minimized and any spill should be promptly cleaned up. Concrete spillage should be kept to a minimum and cleaning of the concrete trucks after they have unloaded should be done in an area that will not drain off site (see BMP C151 Concrete Handling). 10. Control Dewatering In some cases, excavation for the foundation or below ground structures will encounter ground water. This water must be removed (dewatered) from the excavation. Discharge of this ground water must be treated in a manner that will not cause damage downstream due to flow rates or added pollution. There are no specific BMP identified for this activity, but the water should be handled with care to assure that soils or other pollutants are not added to this flow. 11. Maintain BMPs Installation of the appropriate BMPs is not adequate to completely control stormwater runoff. The BMPs that have been installed on the project must be inspected and maintained during the duration of the construction project. In addition, the temporary controls that were installed for construction should be removed within 30 days of completion of the work. Typically, once construction has been completed, the temporary facilities are not maintained, and by removing the facilities, it will ensure that these won't fail and discharge water or sediment that had been previously trapped or contained. 12. Manage the Project Management of a project has four aspects: 1. Phasing construction to prevent transportation of runoff and sediment, 2. Limiting the work during seasons where large amounts of rainfall could be anticipated, 3. Coordination with Utilities and other Contractors, and 4. Inspection and Monitoring. All of these for aspects are important and must be followed to ensure a project that will have minimal impact on the environment. Volume II of the Manual contains additional BMPs that could be used on-site.
