10 Member of AHCA/NCAL No substandard quality of care or immediate jeopardy on any surveys within the last three years ( ) or leading up to award notification in 2015 Applicants with multi-levels of care apply for the entire organization or a distinct part of the organization, but make sure you are consistent throughout the application Applicants must participate in an educational program
11 Element Requirement Notes Format Microsoft Word Applicants at the Bronze award level are required to submit their applications in Microsoft Word format. The file name extension should read.doc or.docx. Page Limit Page Size Page Orientation Text Pages Pages with graphs, figures and tables Bronze: 5 pages maximum Standard, 8 ½ -by-11 inch, white Portrait Portrait or landscape The acronym list does not count against this requirement. However, any responses to the criteria or additional information that exceeds 5 pages will result in disqualification. Margins Page Numbering 1-inch minimum all around Number pages 1 5 in sequence. Larger margins are acceptable. The acronym list (see below) can be numbered.
12 Element Requirement Notes Responses to Criteria Criteria Version Applicants must respond to the 2015 Bronze Quality Award criteria. Font and Type Size: Running text Text within tables Text within graphs and charts Criteria Labeling Prose Style Writing Times New Roman, 12pt min. Times New Roman, 10pt min. Any font, legible text size Applicants must complete all sections of the application and must label responses sequentially to correspond to all section and subsection numbers and letters of the criteria. Each criterion must be responded to separately. The required criteria labeling format is available in application packet. Applications should be written in prose style using complete sentences. Applicants are encouraged to use properly labeled charts, tables and graphs to present evidence and results related to the criteria. Applicants should avoid using tables in lieu of prose responses unless required or clearly appropriate. Prose text in a box does not constitute a table. Applicants must use a minimum of 10 point Times New Roman font in all tables. Use of a smaller font in tables will result in disqualification.
13 The Bronze Application is most importantly a statement of priorities Key = Most Important You may have a clear picture of what s most important (key), but others in the center may not This comprehensive understanding creates an unwavering focus on key performance requirements and results
14 5-page application that addresses 20 questions (criteria) Criteria are straight-forward questions Many can be addressed in 2-3 sentences There are no trick questions
15 Responses to the criteria are evaluated on a simple met/not met basis; Examiners do not use a list of right answers All applicants receive a feedback report identifying which criteria were met and not met (and the reasons why)

17 Glossary terms are noted in SMALL CAPS Read the definition of each term in the glossary Some terms might have a different meaning than you think Definitions provide important information about the criteria Example APPROACH: The methods your organization uses to address the requirements of the Baldrige Criteria items Besides the methods themselves, approach refers to the appropriateness of the methods to the item requirements and your organization s operating environment, as well as how effectively your organization uses those methods.

22 P.2.b.1 Strategic Planning: What are at least two major STRATEGIC CHALLENGES or ADVANTAGES for your organization (e.g., entry into new markets or SEGMENTS, human resource recruitment and retention, new alliances with suppliers, physicians, or other partners, introduction of new technologies, changes in the health care environment that impact your organization s delivery of services, changes in strategy, or other challenges or advantages)?
23 P.2.c.1 What are the KEY elements of your PERFORMANCE improvement SYSTEM? In your response, describe the KEY steps and/or tools that you typically use for PROCESS improvement or INNOVATION. To qualify for the Bronze award level, you must be able to articulate the APPROACH you generally use to improve a PERFORMANCE outcome. This may be a methodology such as FOCUS-PDSA, Six Sigma's DMAIC, or another APPROACH that has been developed or adopted by your organization from other resources such as Advancing Excellence.

The applicant describes the organization's major health care service offerings and the relevant importance of each to the organizational success. The applicant provides a mission statement, specific methods by which it is communicated across the organization, core competencies and their relation to the mission statement. The applicant provides a complete profile of the organization's workforce. The applicant describes the major equipment and technologies that the organization uses. The applicant describes the regulatory environment in which the organization operates and the key bodies of regulation related to health care delivery, occupational health and safety, physical plant, payment and reimbursement regulation.
25 Be thorough, but concise, in response to the criteria e.g., P.1.a(3): Mission and how it is communicated. Be sure to say that your mission statement is posted in the lobby, EVEN if you have stated that it is very important to you. e.g., P.1.a(4): Organization s workforce profile... education levels Be sure to address every section, so say something about the education level of the staff, not just their licensure. Plan ahead, outline before writing Write in complete narrative sentences Have someone proof read the application 25
26 There are no trick questions The Bronze Award is judged based on consistency. The applicant who addresses all areas according to the specific demands of the criteria is likely to be successful. Label sections as directed Answer all questions in the Bronze criteria, unless explicitly noted (i.e. P.2.c.3 is for ID providers only) Adhere to the technical requirements 26
27 All suppliers must maintain a high standard in accordance with the guidelines set forth by the corporation's product Standard Review Committee. Likewise, all business relationships must meet corporate compliance codes that are disseminated to each XXX business unit and monitored for strict adherence by the organization's internal audit system 3c. 28 If you mandate that centers apply, make sure they have the resources to do it well. Strongly encourage centers to use the Bronze Criteria Series. You can not supply any text to be used verbatim in a centers application. Warn centers against copying text verbatim from corporate materials. Coordinators (including consultants) can function as application reviewers, but not as application writers. 28
