1 Scottish Paediatric Intensive Care Transport Service (Edinburgh) Standard Operating Procedure PICU Retrieval Team Response Vehicle Date written: November 2011 Review date: November 2012 Related documents: Activation SOP Author: Jon McCormack Approved by: David Rowney; E Doyle ( on behalf of CMT) Aims To ensure all team members are aware of the use of terms and conditions for business and clinical uses of the PICU Retrieval Team Response Vehicle. Background The Retrieval Team in conjunction with the AF Noble Subaru and the Sick Kids Friends Foundation has acquired a Subaru Forester to fulfil several roles within of the service. It is fitted with a green light system and is also detailed with Battenberg livery and PICU Retrieval Team identifiers. Application All Retrieval Team members, NHS Lothian Fleet Drivers. Vehicle uses The vehicle may be used to fulfil the following roles: Clinical Transport: Travelling to locations for aircraft pick-up (e.g. Airport, NRIE helipad) Outbound travel to retrieval location only when SAS assets are not available.* Subsequent inbound transport of parents in this scenario. Inbound travel of patients is prohibited. Repatriation of patients who would otherwise be travelling by taxi or NHS Lothian fleet car with a Fleet driver if SAS assets unavailable and this would free up a critical care bed space. Non-clinical uses: Transport for the Retrieval team & equipment for teaching purposes Transport of team staff for other non-clinical events (e.g. feedback sessions) Use at demonstration events, fundraising events or similar for the Sick Kids Friends Foundation. Any uses outwith the above should be discussed in advance with the PICU CNM in the first instance. The priority for the vehicle should always be given to the Retrieval team on clinical duty. For non-clinical use the vehicle should be booked in advance via the PICU A&C support staff. *Where the vehicle is used as outbound transport for clinical purposes due consideration must be given to the mechanism of returning the vehicle to RHSC. If there are adequate retrieval medical staff (e.g. Consultant and Fellow/ANP on duty) a three person team may be dispatched with the understanding that one person will return in the car. An NHS Lothian driver may be available SOP to be updated when details available.
2 Entitled drivers Only personnel who have registered their driving license with the NHS Lothian Transport Fleet via the retrieval co-ordinator are entitled to drive the vehicle. Copies of both parts of entitled drivers licences will be held by the retrieval co-ordinator and a log of entitled drivers will be kept in the vehicle for verification purposes with the vehicle log sheet. It is important for insurance purposes that non-registered drivers do not drive the vehicle. If you have not registered your license YOU ARE NOT INSURED. NHS Lothian Fleet regulations do not permit staff with 6 or more points on their driving license (or drink/drugs related motoring convictions) to drive the vehicle. All Scottish Ambulance Service personnel are insured to drive the vehicle whilst on NHS Lothian business only, provided they satisfy the above licence criteria for endorsements/convictions. This is a valid option for return of the vehicle when the team have used it for outbound clinical transport. Similarly, all NHS Lothian fleet drivers are entitled to drive the vehicle. Green lights usage The vehicle is fitted with a Green LED light bar and PICU decals. This is purely for team identification and unit profiling purposes and does not permit driving with exemptions, i.e. breeching traffic regulations and speed limits. The vehicle must be driven in full compliance with the Road Traffic Act at all times. Any breach of this risks accident and police prosecution of the driver concerned, and is likely to be harmful to the reputation of the PICU Retrieval Team and the SKFF. Where and emergency blue light response is required, the team should request an SAS emergency response as per standard practice. Any fines and fixed penalty charges (e.g. parking, speeding fines) incurred are the responsibility of the driver, whether or not they are incurred during business use. Under no circumstances will NHS Lothian reimburse such fines. Do not use a mobile phone whilst driving. Using the vehicle Everyone who drives the vehicle must undergo a daylight familiarisation of the vehicle with the retrieval co-ordinator before use. All staff should read and agree by signing a copy of this SOP which will be kept on file with license copies. The key and fuel card are in the controlled drugs cupboard in PICU. The logsheet and entitled drivers list is kept in a folder in the drivers door pocket. Please check the vehicle for damage prior to departure and note on log sheet. The log-sheet must be completed for all journeys. After any journey where the fuel gauge falls below ¾ full, the vehicle must be refuelled with DIESEL to ensure vehicle readiness prior to parking back at hospital. The fuel card can be used at any ALLSTAR fuel stations (includes most supermarkets) but check with cashier first. An accurate mileage must be given to authorise payment by fuel card. The receipt should be kept in the log folder. Any defects or damage found on the vehicle must be reported immediately to the Retrieval Coordinator. If new damage relates to a Road Traffic Accident the Vehicle Damage Report Form must also be completed and passed to the retrieval co-ordinator. Remove all rubbish/waste after each journey.
3 Weekly checks The vehicle will be maintained in a road-worthy state by the Retrieval Co-ordinator. The weekly check sheets will be logged in the PICU vehicle folder. Weekly checks include: External, internal and tyre inspection for damage (e.g. dents/scrapes etc ) Open the bonnet and check the following:- Windscreen washer bottle is full Oil level Check all lights are working (including green light system) Tyre pressures including spare: Front 32PSI, Rear 32PSI Wheel changing kit is correct and in vehicle Check road atlas for Scotland and GPS are present Wash the vehicle Breakdown In the event of breakdown/accident the vehicle has a comprehensive 24/7 Recovery & Assistance package provided by Subaru UK (includes running out of fuel and changing flat tyres). The phone number for this is located on the card stuck onto the bottom left of the windscreen. Try to park the vehicle off the road if possible. If this is not possible leave lights and hazard warning indicators on, vacate the vehicle and stay well off the road. High visibility jackets should be worn at all times when out of the vehicle. If the vehicle is in clinical use ensure that all medical equipment is secure (keep drug bag with person at all times) whilst awaiting repair or recovery. Contact PICU and EMDC to request alternative transport arrangements. Parking The vehicle must be parked at RHSC and the parking permit displayed. At the moment the allocated space is on the bark surface next to the exit gate (dedicated space to follow). Please use extreme caution when parking to avoid pedestrians and obstacles. The vehicle has a reversing camera with a dashboard display which operates when reverse gear is engaged. The vehicle must not be parked on single/double yellow lines at this or any other hospital as the driver will be liable for parking fines received. All medical equipment must be removed from the vehicle when unattended. The vehicle should not be left in an on-street area overnight. Please note, the insurance will be invalidated if the vehicle is left unlocked or the keys are left in the vehicle. Maintenance The vehicle will be maintained by AF Noble Subaru. It requires servicing at 12,000 miles or 1 year.
4 Accident In event of an accident the following must be done:- If there is vehicle damage or personal injury dial 999 and ask for police and/or ambulance. Do not admit liability Ensure you have details of other vehicles and witnesses. Fill in the road traffic accident and vehicle damage report form (copy in vehicle). Complete a Vehicle Defect Report Form and return to retrieval co-ordinator. Equipment stored in the vehicle Journey Log sheet Accident forms List of registered drivers Scotland road atlas First Aid Kit, fire extinguisher and high-visibility bibs Garmin GPS Documents stored in vehicle folder in PICU Non-clinical use booking diary Blank and completed log sheets Blank and completed vehicle defect forms Blank and completed accident forms Completed vehicle weekly check-sheets Fuel receipts List of registered drivers and date of car familiarisation session. Code of Good Practice This vehicle has been very generously donated to the PICU Retrieval Team through collaboration between AF Noble, Subaru UK, the Sick Kids Friends Foundation and MacraeVision. This is a valuable asset to the team and will greatly enhance our capabilities. It is a high profile vehicle in the public eye. Please drive and park courteously and with due consideration to other road users and pedestrians at all times and within legal guidelines. Any misuse or breach of this code of good practice is likely to be harmful to our reputation, to that of our supporting charities and may jeopardise the future support of this vehicle.
