Providing Shelter, Enabling ndependence Role Profile Post Title: Support Worker Direct reporting to: Hostel Manager Salary: 24k pa Contract: Permanent, full-time Hours: 35 hours pw, excluding lunch breaks Additional: This post requires shift working and occasional Saturday working (one Saturday a month) Job Description Role Purpose Provide personalised advice and support for clients to facilitate changes, which will enable individuals to move-on to suitable accommodation and maintain independent living by: Encouraging and facilitating access to training, education, and employment, and volunteering opportunities for individuals. Designing and delivering workshops and group sessions for the client group. Providing welfare and advocacy on behalf of the client group. Proactively engaging with external clinical support, and specialist agencies for the benefit of the client group. Key Duties and Main Responsibilities 1.0 Residents Support Service 1.1 Provide a comprehensive support, advice and advocacy service to the client group in accordance with Branches Hostel s policies and procedures. 1.2 Use SMART planning to provide tailored support to service users: Develop and keep under review support plans with individuals to maximise their opportunities towards achieving and maintaining independent living. Maintain accurate and up-to-date records. Develop action plans and keep under review to facilitate service users personal development and the skills to live independently. Plan, keep under review and complete recovery and rehabilitation plans for each service user. 1.3 Conduct initial assessments to and from referred services. 1.4 Carry out risk assessments and keep these under review to identify and prioritise individuals changing support needs; and work with relevant external agencies to assess and manage associated risks. 1.5 Make and review referrals to appropriate specialist providers and services to ensure access to relevant external services is achieved and maintained. 1.6 Signpost individuals to appropriate resources including training, education, employment and volunteering opportunities. 1.7 Develop and maintain constructive, professional working relationships with the client group. 2.0 General Support Service Provision 1 P a g e

2.1 nteraction with service users, as a visible and supportive presence in the hostel. 2.2 Ensure individuals are kept informed of their rights and responsibilities regarding the service, are enabled and empowered to become involved in the running of the service, through consultation and participation. 2.3 Plan for and deliver group-work and advice sessions for the client group. 2.4 Contribute to service initiatives and multi-agency projects including on-site training, case management, workshops, and rehabilitation work. 2.5 Through close working with the Resettlement Worker, contribute to move-on planning for individuals who are ready to move on to permanent accommodation. 2.6 Maintain up-to-date knowledge of issues relevant to the client group; and disseminate and advise the support team on changes to best practice and legislation, amendments to ethical frameworks, research findings and publications, and shared best practice. 3.0 Finances and Administration 3.1 Develop and review individual personal budgets with service users, including service charge and rent contribution obligations. 3.2 Adhere to Branches Hostel s financial procedures and policies at all times. 3.3 Complete all administrative duties in accordance with Branches Hostel s policies and procedures. 4.0 Service Development 4.1 Contribute to promoting, developing and maintaining effective joint working with external services to ensure the range of support options offered to the client group remains accessible and relevant to their needs. 4.2 Participate in organisational projects including training and case management reviews. 4.3 Contribute to reports for multi-agency reviews, commissioners, funders and other stakeholders, as required. 4.4 Contribute to maintaining effective systems of service user consultation: collating feedback; and using information gathered to develop the quality of service provision. 4.5 Ensure services are inclusive. 4.6 Represent Branches Hostel at local events as required, supporting the Hostel Manager. 5.0 Other 5.1 Ensure information available to the client group is of a high standard, relevant, in plain English and up-to-date. 5.2 Contribute towards the development of Branches Hostel s policies and procedures. 5.3 Assist with the development of existing and new support services. 5.4 Comply with Branches Hostel s policies and procedures, including but not limited to: Safeguarding, 2 P a g e

Health and Safety, Professional Boundaries, Confidentiality and Data Protection, ensuring that policies and procedures are adhered to in all aspects of your work. 5.5 Attend and fully participate in supervision meetings, team meetings and case management discussions. 5.6 Keep under review and evaluate own performance to identify strengths and areas for development. 5.7 Maintain knowledge of T including Word, Excel, Access and databases necessary for carrying out the role. 5.8 Work on a rolling shift rota: includes early and afternoon shifts, weekends and emergency on-call out-of-office hour s duty cover, as required to maintain effective delivery of services. 5.9 Carry out any other tasks commensurate with the post. Person Specification Support Worker 1.0 Knowledge and Skills (E) 1.1 Experience of providing support to individuals in a professional one-to-one context; including managing coaching and mentoring conversations and the appropriate follow up. 1.2 Experience of supporting vulnerable individuals who have complex needs. 1.3 Excellent self-management, taking responsibility for your own caseload; effectively prioritising and managing conflicting areas of work. 1.4 Excellent communication skills, including clear and succinct written communication. 1.5 Excellent interpersonal skills; communicating appropriately with the client group; and equally confident in communications with external agencies. 1.6 A commitment to working flexibly and creatively in response to changing organisational requirements. 1.7 Experience of handling complex and difficult situations, in relation to the provision of services to a vulnerable client group. 1.8 Understanding of risk assessment, planned support and SMART goal setting; and apply this in an outcomes-based delivery service. 1.9 Knowledge of housing and welfare benefits, as relevant to Branches Hostel s client group. 2.0 Aptitude and Abilities 2.1 A self-starter, with a can do pro-active attitude to day-to-day work and problem solving. 2.2 Ability and aptitude to be hands-on and involved in the day-to-day running of a small hostel; including managing conflict. 2.3 nitiative and ability to provide and deliver solutions to issues arising in support service delivery. 2.4 Ability to interact and communicate with the client group and be a visible, engaged and supportive presence for the client group in Branches Hostel. 2.6 f no direct experience in homelessness, the ability to gain knowledge and understanding of the support needs of homeless people, and convert this knowledge into best practice support work. 2.7 Ability to analyse and evaluate written and numerical information, and data. 2.8 Ability to liaise effectively, develop and maintain constructive partnerships with external agencies. 2.9 Ability to contribute to developing and delivering advice and information sessions for the client group, monitor and evaluate their effectiveness. 2.10 Honesty, reliability and flexibility.
