6 PHR Enabled - Benefits Patients may see their clinical data online. Allows patients access to their information with no staff intervention. Provide patients with the ability to share their health information with providers from a different practice. Helps providers meet MU2 measure!
7 Meaningful Use Stage 2 (2015 Extended) Measure 2 - More than 5% of patients One patient (or authorized representative) must actually access their Portal account to view, download or transmit their health information. Providers scheduled for Stage 1 in 2015 may exclude from Measure #2 for 2015 only.

9 PHR - My Chart Viewing (Consolidated)
10 Consolidated PHR View PHR data gets populated from generated CCD-A documents sent to the patient s Portal account. Portal uses the Rosetta CCD-A Document interface agent to generate these PHR documents.
11 Consolidated PHR View The consolidated view shows a longitudinal view of the patient s data. Each section will display the distinct data received in the software processing (machine readable) sections of the CCD-A, across all PHR documents.

15 PDF View These PDF documents provide a more user friendly view of the CCD-A information received. These PDF documents show what was sent in the human readable section of the CCD-A file sent to the patient s Portal. Each PDF document will be either a Longitudinal document or an Encounter Specific document.

17 PHR Processors PHR Requests (Longitudinal) PHR Backfill (Longitudinal) PHR Partial (Encounter) PHR
18 Patient Requested PHR PHR Requests Processor When patient requests his/her health record, this processor will generate the CCDA and push it to the patient s Portal account. A Longitudinal (Complete) CCD-A of all the patient s PHR data is sent. Patient will receive a notification when the document is available.
19 Backfill PHR PHR Backfill Load Processor A one-time process that will upload a longitudinal PHR document to EVERY enrolled patient s Portal account if one does not exist. A one-time process that will upload the patient s PHR document when a NEW enrollment is completed.
20 Backfill PHR PHR Backfill Load Processor Enabling this processor will place a large volume of records into the export queue. It takes time for the export queue to complete all of these records. Patients may not see these documents right away.
21 Partial PHR PHR Partial Load Processor Sends encounter specific PHR information to the Patient Portal when the encounter locks. Patients will NOT receive notification when these PHR documents are received.
