4 HSP Supplemental Information Initiative Background Toronto Central LHIN is looking to address health challenges locally by engaging people and providers at the sub-region level to co-design solutions Through provider engagements, there was a recognized need for high quality service-level data to be collated at the community level across the LHIN HSP Supplemental Information Initiative was launched in early 2017 in order to provide planners and HSPs with a clear line of sight into service capacity Community providers: CMHA, CSS, Home & Community Care (former CCAC), CHC In 2017, the response rate was approximately 99% (140 of 143 HSPs) Contact Ting Lim at Ting.Lim@tc.lhins.on.ca for any questions More information can be found on the Toronto Central LHIN website: 4
5 Key Outputs of the HSP Supplemental Initiative Updated HSP contact list Open Data (Database/spreadsheets) of providers in each service category: Locations; Hours of operation; Catchment area; Priority conditions; Priority populations Client tools Interactive dashboard (HSP360) Data collection refresh (May 2018) Various analytics products Local Collaborative (showcased in July 2017) Sub-region asset mapping of community services for sub-region service profiles Currently shared with sub-region leads for development of Sub-Region Advisory Tables Which HSPs, What services, Clients served, Funding, etc. 5
6 Data Refresh for FY 2018/19 To continue our efforts in providing you with an up-to-date resource, we are asking all community providers to review their supplemental information for 2018/19 As of May 2018, HSP Supplemental data collection for 2018/19 will leverage HSP360 to provide a new streamlined process that will allow providers to easily input or update the information for their organization and LHIN-funded services Once submitted, data will be made available on the HSP360 dashboards HSPs may update their information as soon as additional changes come into effect 6

8 Important Timelines Please review and complete data forms by Friday June 1, 2018 Four orientation sessions have been planned to support form completion: Monday, May 7 th from 2:00-3:00PM Thursday, May 10 th from 10:30-11:30AM Tuesday, May 15 th from 2:00-3:00PM Wednesday, May 16 th from 10:00-11:00AM Personalized individual support is available for those requiring it Please contact Ting Lim (Ting.Lim@tc.lhins.on.ca) to schedule an appointment. 8

11 Guiding Principles Decision Support Capacity HSPs have access to a userfriendly reporting and analytic tool, building decision support capacity Transparency HSPs can view each other s performance data Accountability HSPs can better see how they contribute to the overall LHIN and system results Enable Discussion HSPs can compare against peers enabling discussions around planning, target setting and quality improvement Centralized/Easy Access HSPs can access performance data in one location reducing need for manual report creation Holistic View HSPs can access performance data across programs and sources in one shared view

13 Accessing HSP Open Internet Explorer or Google Chrome on your computer. 2. In the address bar, type in and press Enter. 3. On the Login page, click on the Login link to enter your username and password. 4. Navigate to the Community Tab and hover over HSP Supplemental to view the submission form and dashboards. 13

15 Considerations Identification Tab Review and ensure the information is up-to-date for the 2018/19 fiscal year. If you ve provided data last year, this data will be pre-populated on this form. If there have been changes to your HSP (e.g. merges, transfers), the data on the form has been modified to best reflect the new structure please review and make any updates. In the Submitter(s) Details section, include additional submitter details only if there are more than one person submitting data on this form. In the HSP Locations tab, please ensure details for the head office have been provided. In the HSP Funders tab, please ensure the percentages add up to 100%. RQT Survey NEW! Please complete this survey so that the LHIN can better understand the methods used by community health service providers in capturing patient/service user experience information and how it is being used. This will help shape the plan for enhancing measurement across the system and improve service user experience and outcomes. 15
16 Considerations Access Tab In the Access tab, - If there are any services without any hours entered, the cell within the Access column will be shaded in yellow - Period 1, 2, and 3 are used if the service has multiple opening and closing hours per day (e.g., social & congregate dining). If there are no breaks, input your hours within Period 1 - Use the Comments section to explain any service irregularities (e.g., every 3 rd week) - Please enter values based on 24 hour clock - There is now the ability to check off 24 hours if your service provides 24 hour service - If there is a functional centre based at a location, but no applicable hours, input 00: :01
17 Considerations Service Area Tab In the Service Area tab, - Select the neighbourhoods that your service intends on serving; this is not necessarily actual service area - If there are any services without any service areas entered, the cell within the Sub- Regions column will be shaded in yellow - Within this form, you can: - Sort the table columns - Select the whole LHIN or sub-region by double-clicking on the name - Check off province wide to select all LHINs and neighbourhoods - If you provide services to other LHINs (but not all LHINs), you can select each one - View the neighbourhood mapping 17
18 Considerations Priority Groups Tab In the Priority Groups tab, - You can: - Check off both the priority populations AND conditions for a particular service in one place - View definitions of the priority populations 18
19 Considerations Overall Submission There are validation checks on each page. As there are no limitations to the number of locations, neighbourhoods, etc. that you can select, please ensure all applicable fields are filled to the best of your knowledge. If a validation check is not met, you will not be able to submit your data; however, if there is a validation check that you feel cannot be corrected as exceptions may apply, please contact Ting Lim (Ting.Lim@tc.lhins.on.ca) for further guidance. There is the ability to add comments on almost all tabs. Ensure you click on Save and Next to save any changes and/or comments you have made on that tab. 19
