These are questions that were received prior to the published deadline for questions of April 03, IMPORTANT NOTE the following is added to ITB # It is the Proposer s responsibility to use the revised information when preparing the response. 1. ALL BIDDERS: must include in the Bid Schedule under OTHER pricing for providing underground power. 2. Q: What is the City budget for this project? A: $ 80, Q: What is the duration or the project? A: Ninety (90) days 4. Q: Will Builders Risk be needed? A: No, For Insurance Requirements See Attachment F. 5. Q: Will the contractor be responsible for permit fees? A: No, permit fees will be waived; however the contractor is responsible for pulling all permits. 6. Q: What are the construction hours? A: Monday Saturday 7:00 AM 5:00 PM 7. Q: Please clarify the information provided on carpet tiles? A: The Carpet Tile is the dash area and the squares are the ceramic tiles 12. Q: Wilton Manors is surrounded by salt water, a 10 year warranty is not available unless the Englert 2000 series is in aluminum..032 in a standard color would be suggested..040 is available, it just cost additional. 3 City of Wilton Manors Finance Department 2020 Wilton Drive Wilton Manors, FL Phone (954) Fax (954) GC-27 INSURANCE REQUIREMENTS CONTINUED ATTACHMENT F Proposers must submit with their proposal, Proof of Insurance, meeting or exceeding the following requirements The successful Proposer shall secure and maintain, at its own expenses, and keep in effect during the full period of the contract a policy or policies of insurance, which must include the following coverage and minimum limits of liability: (a) (b) Worker s Compensation and Employer s Liability Insurance for all employees of the successful Proposer engaged in work under the Contract in accordance with the laws of the State of Florida. The Successful Proposer shall agree to be responsible for the employment, control and conduct of its employees and for any injury sustained by such employees in the course of their employment. Comprehensive General Liability Insurance with the following minimum limits of liability: $ 500, Combined Single Limit, Bodily Injury and Property Damage Liability per occurrence to include: (1) Premises and Operations; (2) Independent Proposers; (3) Products and Completed Operations: (4) Broad Form Property Damage; (5) Broad Form Contractual Coverage applicable to the Contract and specifically confirming the indemnification and hold harmless agreement in the Contract; (6) Personal Injury Coverage with employment and contractual exclusions removed and deleted. (c) Comprehensive Automobile Liability Insurance for all owned, non-owned and hired automobiles and other vehicles used by the Proposer in the performance of the work with the following minimum limits of liability: $ 500, Combined Single Limit, Bodily Injury and Property Damage Liability per occurrence (d) Professional/Errors & Omissions - $250, (e) Employee dishonesty/theft - $250, ALL LIABILITY INSURANCE POLICIES SHALL SPECIFICALLY PROVIDE THAT THE CITY OF WILTON MANORS SHALL BE NAME AND LISTED AS AN ADDITIONAL INSURED. All of the policies of insurance so required to be purchased and maintained shall contain a provision or endorsement that the coverage afforded shall not be canceled, materially changed or renewal refused until at least thirty (30) calendar days written notice or such other notice as is required by the policy has been given to City by certified mail. Additionally, in the event the successful Proposer receives notice of cancellation from its insurance company, the successful Proposer shall deliver a copy of such notice to the City within five (5) business days of the receipt of such notice of cancellation The successful Proposer shall ensure that any company issuing insurance to cover the requirements contained in this Contract agrees that they shall have no recourse against City for payment or assessments in any form on any policy of insurance The successful Proposer shall not commence work under the Contract until obtaining all of the minimum insurance herein described The successful Proposer agrees to perform the work under the Contract as an independent proposer, and not as a subcontractor, agent or employee of the City The required insurance coverage shall be issued by an insurance company authorized and licensed to do business in the State of Florida, with the minimum rating of B+ or better, in accordance with the latest edition of A.M. Best s Insurance Guide The successful Proposer must submit, no later than ten (10) days after award and prior to commencement of any work, a Certificate of Insurance naming the City of Wilton Manors as additional insured. The documentation submitted has been reviewed and accepted by Miami-Dade County RER - Product Control Section to be used in Miami-Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ). This NOA shall not be valid after the expiration date stated below. The Miami-Dade County Product Control Section (in Miami-Dade County) and/or the AHJ (in areas other than Miami-Dade County) reserve the right to have this product or material tested for quality assurance purposes. If this product or material fails to perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. RER reserves the right to revoke this acceptance, if it is determined by Miami-Dade County Product Control Section that this product or material fails to meet the requirements of the applicable building code. This product is approved as described herein, and has been designed to comply with the Florida Building Code including the High Velocity Hurricane Zone of the Florida Building Code. DESCRIPTION: Englert Series " Aluminum Roof Panel over Wood Deck LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following statement: "Miami-Dade County Product Control Approved", unless otherwise noted herein. RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change in the applicable building code negatively affecting the performance of this product. TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section of this NOA shall be cause for termination and removal of NOA. ADVERTISEMENT: The NOA number preceded by the words Miami-Dade County, Florida, and followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be done in its entirety. INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors and shall be available for inspection at the job site at the request of the Building Official. Flashings, penetrations, valley construction and other details shall be constructed in compliance with Roofing Application Standard RAS 133. Panels shall be installed with galvanized steel clips (3½" long x 2 ½" wide x 1¾" tall), attached to substrate with (2) two corrosion resistant #10-16 self-tapping pancake screws of sufficient length to penetrate through the sheathing a minimum of 3 / 16". Clip fastening shall start 3" from panel end and shall be at maximum spacing as described below in Table A. Male leg of snap seam shall be caulked with a minimum 1 / 8" bead of elastomeric construction sealant prior to installation of female leg of panel. 9 SYSTEM LIMITATIONS 1. Fire classification is not part of this acceptance; refer to a current Approved Roofing Materials Directory for fire ratings of this product. 2. The maximum designed pressure listed herein shall be applicable to all roof pressure zones (i.e. field, perimeters, and corners). Neither rational analysis, nor extrapolation shall be permitted for enhanced fastening at enhanced pressure zones (i.e. perimeters, extended corners and corners). 3. Panels may be rolls formed in continuous lengths from eave to ridge. Maximum lengths shall be as described in Roofing Application Standard RAS All panels shall be permanently labeled with the manufacturer s name and/or logo, and the following statement: Miami-Dade County Product Control Approved or with the Miami-Dade County Product Control Seal as seen below. All clips shall be permanently labeled with the manufacturer s name and/or logo, and/or model. 5. All products listed herein shall have a quality assurance audit in accordance with the Florida Building Code and Rule 61G20-3 of the Florida Administrative Code. 6.
