Subject: Limited Tender for Supply of Degradable Polythene Bags Sir This hospital intends to purchase Degradable Polythene Bags (as per size and colour given below) for one year for hospital use. You are therefore, requested to submit/deposit the Sealed Tender in the tender box kept in chamber of Dy. Medical Supdt. of this hospital on 12/03/2010 between 9.00 AM to 1.00 PM. The tender shall be opened on 12/03/2010 at 2.30 PM in the Committee Hall of this hospital alongwith signed Terms and Conditions of said tender which are enclosed herewith. The samples of Polythene Bags are required to be submitted along with tender. 2 TERMS & CONDITIONS ANNEXURE B 1. As per Ministry of Environment and Forest Notification tenderers must give the undertaking and certificate that the degradable plastic bags are in accordance with specification approved by Delhi Pollution Control Borad. In the case of false declaration to this effect entire responsibility would rest on the tenderers. 2. F.O.R. destination. 3. Degradability of plastic bags should be of one year. 4. Plastic bags should be made of degradable plastic quality only. 5. Date and name of manufacturers must be printed over the bags. 6. Name of the hospital in bold capital letters should be clearly printed on all bags with BIO HAZARD word and symbol must be printed on all bags except black bags. 7. The supplier manufacturer must submit a copy of mandatory certificate for degradable material plastic bags from any one of the following lab: i National Chemical Laboratory, Pune ii Sri Ram Institute of Industrial Research Delhi iii Central Institute of Plastic engineering technology, Chennai. 8. Goods accepted subject to inspection and approval and subject to submission of affidavit as per format enclosed in Annexure A 9. The bags should be leak proof. 10. The validity/period of tender will be for one year from the date of sanction of the Medical Superintendent. The tender may be extended for a period of three months at the discretion of the Medical Superintendent. 11. The affidavit (as enclosed) regarding biodegradibility of the Plastic Bags should be submitted with every supply on Stamp Paper. 12. Any conditional acceptance is liable to rejection of tender. 13. All pages of tender document submitted should be numbered & signed by authorized signatory of participating firm with seal. 14. Taxes, if applicable, shall be as notified by Govt.

3 : 2 : 15. The successful tenderer shall supply the articles within six weeks from the date of issue of supply of supply order. In case of delay/failure in supply, the indented items shall be purchased from the market at the risk and cost of contractor. The extra expenditure involved in procuring supplies from elsewhere will be recoverable from the supplier in full at discretion of the Medical Superintendent, IGESI Hospital, Jhilmil, Delhi. The recovery thus due shall be deducted from any sum then due to supplier from the ESI Corporation from this or any other contract placed with said contractor by the Employees State Insurance Corporation. 16. Tenders in sealed envelope superscribed as Limited Tender for supply of Degradable Polythene Bags can be dropped in the tender box kept in chamber of Dy. Medical Superintendent of this Hospital, on 12/03/10 between 9.00Am to 1.00 PM. No tender thereafter shall be accepted. The tender shall be opened on 12/03/10 at 2.30 PM in the Committee Hall of this hospital in presence of tenderers or their representatives who wish to be present. In the event of 12/03/10 being declared a holiday, the tender will be opened on next working day at same time and place. 17 If the tender is sent by post it must be received in office of Medical Superintendent by 1.00PM on 12/03/10. Proof of postage won t be considered as evidence for timely submission of tender. 18 All disputes subject to Jurisdiction of Delhi only. 19 M.S. reserves the right to accept or reject any or all tenders without assigning any reason thereof. 4 ANNEXURE A AFFIDAVIT I for M/s (Name of Govt. Agency)/Co-operative Store do hereby solemnly affirm and declare as under: 1. That the Degradable Plastic bags are made from LDPE & LLDPE. 2. The the degradable plastic bags are in accordance with the specifications approved by the DPCC. 3. That the Degradable plastic bags has been manufacture by M/s (Name of manufacturer) 4. That the plastic bags are tested by (Name of the lab) and certified to be degradable as per DPCC norms.
