Bhd. to the criteria within the Sabah Timber Legality Assurance System (TLAS) Principle 5 - Mill Operations and Principle 6 - Trade and Customs standards. Compliance is defined when the organization demonstrates that there is no non-compliance to any applicable criteria. Non-compliance to the Sabah Legality Standard occurs when the organization does not comply with 1 or more criteria. Non-compliance must be formally addressed prior to certification or within 2 months following a surveillance assessment. Limitations of Assessment The information presented in this report is based on documentation and other information presented by the client and observations made during the field verification under the assessment. GFS reporting of information presented by the client are taken as accurate, unless evaluated differently through the observations made during the assessment. All information presented in the report is accurate and complete to the best of knowledge of GFS. Background Information: Kilang Kayu Lohan Sdn. Bhd. (Kilang Kayu Lohan) registered in Kuala Lumpur, Malaysia with registration # M. The company produces and trades sawn timber for local market. Kilang Kayu Lohan has obtained FSC Chain of Custody certificate, certificate #: SGS-COC for their products (sawn timber, moulding, engineered wood products) since 27 th January 2016, with expiry on 26 th January Associated Organizations & Subcontractors: Kilang Kayu Lohan is the sister company to Lusera Sdn. Bhd., which is located in Kota Kinabalu. Sawn timbers produced are mainly sold to Lusera Sdn. Bhd.. Kilang Kayu Lohan does not engage other companies as subcontractors to provide or manufacture products. 4 003/2016 Minor / Submission of JKKP 8 Form (Register of Accident, Dangerous Occurrence, Occupational Poisoning, and Occupational Disease) for year 2015 was not submitted as stipulated in Guidelines on Safety and Health (Notification of Accident, Dangerous Occurrence, Occupational Poisoning and Occupational Disease) regulations 2004 [NADOPOD]. Based on the Safety Committee Meeting Minutes (27 th June 2015), one minor injury was mentioned but no report was documented by Kilang Kayu Lohan. Status of Outstanding GAPs: 2015 GAP # Type Checklist Status Description 001/2015 Minor Closed Based on the employment record, all 3 temporary workers without valid documents were terminated. Kilang Kayu Lohan had contributes SOCSO payment for Mauyan AK Embah who was previously not covered. Kilang Kayu Lohan has kept a record for PPEs distribution and general briefing was provided during the issuance. 002/2015 Minor 5.3.2, Kilang Kayu Lohan has documented the minutes of Closed Safety & Health Committee Meeting which held twice a year. The meeting includes general safety awareness for the workers. Fire extinguisher handling and fire training was provided in year Observations: 2016 OBS # Checklist # Description One sampled log (#1729; YS; KTS Plantation) was not listed in the delivery notes which attached to the removal passes for the consignment. However, the log was listed in the Timber Disposal Pass (#271759) and royalty paid Briefing on PPE equipments & safety measures was provided during PPE hand-over but was not mentioned in the PPE distribution record Kilang Kayu Lohan is still in process to rectify all issues raised by DOSH in the visitor book. Highlights of close-out visit (if applicable) NA Date Recommendations Kilang Kayu Lohan has demonstrated compliance to the Sabah Standard Principles 5-6 for Chain of Custody and is recommended to receive a GFS Audit Statement and Sabah STCC by SFD.

5 Minor gap identified in this assessment should be addressed before the next surveillance visit. Actions required to close the gaps include: To submit the annual incident report (JKKP08 form) as required by DOSH and keep a record of any accident occurrence within the mill area. To record any briefing and internal training which being provided to the workers. To take appropriate action to rectify the issues raised in the DOSH visitor book. The next surveillance visit is scheduled in April End of Summary Report
