Bhd. to the criteria within the Sabah Timber Legality Assurance System (TLAS) Principle 5 - Mill Operations and Principle 6 - Trade and Customs standards. Compliance is defined when the organization demonstrates that there is no non-compliance to any applicable criteria. Non-compliance to the Sabah Legality Standard occurs when the organization does not comply with 1 or more criteria. Non-compliance must be formally addressed prior to certification or within 2 months following a surveillance assessment. Limitations of Assessment The information presented in this report is based on documentation and other information presented by the client and observations made during the field verification under the assessment. GFS reporting of information presented by the client are taken as accurate, unless evaluated differently through the observations made during the assessment. The company produces and trades sawn timber, finger joint, and lumber mouldings for export sales. PPE distribution records are available during document review. However, most of the workers in the production line were not equipped with PPE during mill visit the assessing team observed workers at the band saw, finger joint and moulding section wore slippers while working. No fire training was provided to mill workers to date. The assessing team interviewed few workers during mill visit; most of the workers do not have safety awareness and know-how on the immediate response if an emergency occurs. 4 001/2015 Minor 5.1.2, Closed The assessing team observed: a. Inconsistency on volume recording using cubic meter (m 3 ), tons (Ts), and metric tons (MT) in different documents the removal passes received upon the arrival of raw materials, the raw material arrival book, Appendix 1 and KK Assan s actual measurement (monthly). Hence the total of volume monthly receiving did not correspond (as shown in the table). b. Inconsistency on recording the arrival of plantation logs into Log Arrival Book. c. Observed inconsistency of total volume written on removal pass issued, only total of logs recorded in the removal pass. d. The total volume of sawn timber received was not recorded in the arrival book. 002/2015 Minor Closed 16 th March 2016: Starting January 2016, KK Assan had converted Tons on the Removal Pass to cubic meter in order to standardize all the receiving volume into cubic meter. Hence the total received volume in arrival book and the total supply volume in Appendix 1 tallies. The arrival of plantation logs and sawn timber were consistently recorded into log arrival book. No submission of JKKP 8 Form (Register of Accident, Dangerous Occurrence, Occupational Poisoning, and Occupational Disease) for year th March 2016: KK Assan submitted the JKKP 8 of year 2015 to DOSH Kota Kinabalu through fax on 31 st December Observations: 2016 OBS # Checklist # Description The licensee shall use the latest export license number - JP(PT)090/00(M)R instead of the old number - JP(PT)113/93(M) as written in the export documents. 5 Recommendations Kilang Papan KK Assan Sdn. Bhd. has demonstrated compliance to the Sabah Standard Principles 5-6 for Chain of Custody and is recommended to receive a GFS Audit Statement and Sabah STCC by SFD. Minor gaps identified in this assessment should be addressed before the next surveillance visit. Actions required to close the gaps include: a. C6.1.1, C6.1.5 KK Asan shall maintain copies of FDS documents Borang EIS 1/2000, JHP 1, JHP 2 for administrative purpose. b. C5.3.2 KK Assan shall conduct safety and health training in order to equip workers with first aid knowledge as well as to increase safety awareness among workers during operation. Training conducted is required to be documented and maintained. c. C5.3.8 The purpose to conduct fire prevention training is to increase awareness among workers on handling emergency cases. Records of training shall be documented and maintained. The company shall also prepare standard operation procedure (SOP) on fire prevention and controlling.
