SUMMARY OF FINDINGS/COMMENTS The permit has been operating without a permit since September 28, Unpermitted Discharge in 50 drainage easement from a 2 line that serviced either recirculating sand filter #1 or #2. Recirculating sand filter 5 was collapsed at time of inspection. Weeds have been allowed to grow in the filter media at each individual sand filter. Unable to determine if complete records were being maintained and current of all activities related to the removal of waste materials accumulated in the septic tanks and other vessels or components of the waste disposal system. Grab samples are not being obtained at the peak loading period from each individual recirculating sand filter. According to the Department s records, Mr. Whitehead does not appear to have a wastewater license. Complete biannual operational reports covering May- October and November through April are not being submitted to the Department. I observed gallon tanks sitting on the ground near recirc filter 3 & 4. According to Mr. Whitehead, the two force main lines at the hair pin near the corner of lot 8 were intercepted And a manifold was installed to transport sewage to recirculating sand filter #3. According to Mr. Whitehead, the tank broke at Lot 56 due to settling. The tank was fiberglassed in approximately four months ago. According to Mr. Whitehead, the high level alarms are activated after a substantial rain event. At the time of both inspections, the low level alarm was activated. The recirculating sand filter was torn at the #3 recirc sand filter. Unable to determine if there is any surfacing at the leachfields due to the height of vegetation. Unable to determine if inspection ports were installed at the end of each lateral lines and in the middle as indicated in drawings. Lateral head should be monitored in a maintenance program to determine if each lateral line in each zone has the proper squirt height. 10 ARKANSAS DEPARTMENT OF ENVIRONMENTAL QUALITY MEMORANDUM TO: Cindy Garner, Manager Enforcement Branch, Water Division THRU: Dale Washam, Inspector Supervisor, Water Division FROM: Alison West, District Field Inspector DATE: September 3, 2008 SUBJECT: Subdivision AFIN: State Permit Tracking No.: 4543-W On August 8, 2008, I conducted an inspection at. Subdivision has a wastewater treatment facility that has been operating without a permit since September 28, On August 27, 2008, I conducted a follow-up inspection with Mike Whitehead, Operator, and Steve Hesse, Engineer. On, September 3, 2008, I conducted a follow-up inspection to check the unpermitted discharge that was discovered during the August 27, 2008 follow-up inspection. At that time, I observed that the discharge had stopped and a U had been installed to direct waste to the manifold system. The recent inspections revealed many of the similar deficiencies as observed during the August 2005 inspection, February 2006 inspection and October 2007 inspection. During the past inspection, we determined that there are ~twelve houses that are built in. Of the twelve houses, Lot 134 has a permit for an individual septic system through the Arkansas Department of Health. During the inspection on August 8 and August 27, 2008, the following deficiencies were found: 1. Subdivision has been operating without a permit since September 28, Unpermitted discharge in 50 drainage easement from a 2 line that serviced either recirculating sand filter #1 or #2. According to Mr. Whitehead, a simple U was tied into the other 2 line where the discharge was taking place. Anything coming down that line will be directed back to the manifold and follow the force main to recirculating sand filter #3. The discharge has stopped according to Mr. Whitehead on August 29, Recirculating Sand Filter #5 was collapsed at the time of inspection. 4. Weeds have been allowed to grow in the filter media at each individual sand filter. The recirculating sand filter at #1, #2, and part of #4 cannot be seen due to the amount of vegetation.

According to Mr. Whitehead, the force main lines at the hair pin near the corner of lot 8 were intercepted and a manifold was installed to transport sewage to recirculating sand filter # gallon tanks were sitting on top of the ground near recirculating sand filter #3 and According to the Department s records, it does not appear as though Mr. Whitehead is a licensed wastewater operator. 9. Unable to determine if complete records were being maintained and current of all activities related to the removal of waste materials accumulated in the septic tanks and other vessels or components of the waste disposal system. According to Mr. Whitehead, each home is responsible for problems with the septic tanks. 10. Grab samples are not being obtained at the peak loading period from each individual recirculating sand filter. 11. Complete biannual operational reports are not being submitted to the Department. 12. According to Mr. Whitehead, the tank broke at Lot 56 due to settling. The tank was fiberglassed in approximately four months ago. 13. According to Mr. Whitehead, the high level alarms are activated after a substantial rain event. At the time of the inspection and follow-up inspection, the low level alarm was activated at system 3 recirc panel. The pumps are set at 17 minutes off. They run for 2 ½ minutes. There is an override cycle for 10 minutes. Mr. Whitehead doesn t know when the alarms are going off unless someone calls him. The audible alarm has been disconnected. 14. The recirculating sand filter liner was torn at #3 sand filters. 15. Unable to determine if there is any surfacing in leach fields due to the height of grass. 16. Unable to determine if inspection ports were installed at each end of the lateral trench and in the middle as shown on the trench drawing detail. 17. The leach field consists of low pressure pipe system. Lateral head needs to be monitored at inspection ports of the lateral lines in each zone to ensure each zone has the proper squirt height. According to the system layout, this should be done as part of a regular maintenance program for the system. In addition, I request that a State Permits Engineer assist me to assess the waste water treatment plant. During the inspections on August 1 and 2 of 2005, I estimated that homes had been built that are utilizing the recirculating sand filter. It appeared as though no homes had been built in the area that utilizes the #4 and #5 recirculating sand filters. During the February 2006 inspection, the following copies were submitted to the State Permits Branch: a copy of the August 1 and 2, 2005 inspection, a copy of a letter dated October 6, 2003, from Henry Insua, State Permits Engineer; a copy of a letter dated January 23, 2003, from Craig Corder, Engineer Supervisor with the Arkansas Department of Health; a copy of the letter dated January 14, 2003, from Craig Corder, Engineer Supervisor with the Arkansas Department of Health; a copy of a letter dated August 25, 2000, from Craig Corder, Engineer Supervisor with the Arkansas Department of Health.
