BOARD MEMBER HOWARD MALL, MANAGER Crystal Metz - Secretary April 18, 2008 RE: PSWD-19A Response (Sewage) West Virginia Public Service Commission Sandra Squires, Executive Director 201 Brook Street Charleston, W On August 9, 2006, the Enlarged Hepzibah PSD filed an application requesting both a water and sewer rate increase. The district requested the Staff to determine the amount of increase to its' existing sewer rates and charges. The PSD received their rate increase June Since the approval of the increase, the Enlarged Hepzibah PSD has been able to bring their payables up to date and able to keep them current. The following activities have taken place affer July 1, 2007 in response to Case # PSWD-I9A Sewage Response. I. The District installed an E-One Grinder Station for Customer Robert Reed. The PSD advising the customer before building the house that he could gravity feed, The customer would not raise the elevation of his home. The Customer filed a formal complaint with the Public Service Commission. The District agreed to supply the station and the customer would pay the installation to avoid a hearing. Mr. Jim Weimer of Your staff called the District several times asking the District to come to some kind of an agreement with Mr. Reed. Rule 5.3K states "The utility shall make changes and bear the full costs of
3 changes in the customer s service pipe required due to changes in grade, relocation of mains and other causes not related to the customer. The customer shall bear the full costs of changes in service pipe or meter location desired by the customer for his or her convenience 2. Mr. Weimer of the Public Service Commission advised the District to apply for an additional tap fee for customers who need an -One Grinder Pump The PSD was asked by the Public Service Commission to remove this request as the Water and Waste Water Division of the Public Service Commission had some problems with this suggestion. I believe that when we need to install a station it should fall under the Main Line Extension ruling. 3. The PSD has installed the following taps. Leonard Cattaneo (sewage tap) The Customer did all the excavation and the PSD rented the drill to drill through the manhole Freeman Morgan (water tap) supplies included (1) 4 x4 Fernco, (1) 4 45 L, (1) 4,22 (2) 4 class 35 and ((I) 4 glue cap. Robert Pierce (relocated sewer tap) supplies included (I) 4 22, (2) joints of pipe, (1) running Y and (1) sewer cap. Tarzan Cain Car Lot (sewage tap). Danny Stout (sewage tap) supplies included (I) 4 saddle and (2) joints of SCH 35 pipe. Darrell Wiles (sewage tap) supplies included (1) 4 running V, (1) 4x4 Fernco, (8) joints SCH 35 sewage pipe. Contractor used for excavation was Amos Paving. Donald Friend (water tap) supplies used (2) 4 45L, (I) 4x4~4 wye, (I) 4 cap, 6 4 SCH 35 pipe. Donald Friend Located sewer tap. Donald Menendez (water and sewer tap) PSD open cut road and ran water to the property line of the customer.
4 4. The PSD is currently upgrading lit? stations. We have eliminated a lit? station in the Spelter area, which was serving one home. The Electrical panel and all parts will be used as spare parts. 5. George Harvey Lit? Station repair, supplies used (1)4x4 sq hanger, (4) float garments, (1) 2 Ib cable, (33 1 x I Angle iron, (1) 9x9~4 weather tight enclosed meter box. 6. Jackson Lit? Station, replaced floats and installed new contactor, supplies used (3) 1 % aluminum angel, (3) float holders and (2) N hangers. B & M Environmental did all electrical work. 7. Re-plumbed #2 Lift station at the Pete Dye Golf Course. The check valve location was leaking badly. Stuart McMunn Company of Clarksburg replaced the unit. 8. Re-plumbed both sewage treatment plants separation of ditches so that each sludge return line would service separate ditches. Supplies used would be duplicated because we re-plumbed both plants. (IO3 SCH 80 4 plastic pipe, (I) 4 elbow, (1) 4 Flange to Plastic and (3) Flange Kits. 9. Rebuilt pump at Pete Dye Club House, cost $ IO. Replaced a motor at the Spelter plant. The bearing was wearing out. The motor was repaired at a reasonable price and will be on hand and used when needed. 11- Recalibrated flow meters at both plants, cost $ The PSD is diligently working on inflow and infiltration problems in our area. This also includes notifying customers of rain gutters being tied into the system and other infiltration on the properties. The customers will be given an ample amount of time to repair their problems. After several months, an additional letter will be sent to the customers who have not complied and a surcharge will be added, which is part of our rates. 13. The PSD is currently working on a smoke testing program trying to eliminate inflow and infiltration in our system. We recently had a CEI inspection performed by the Department of Environmental Protection and we were able to meet all requirements in that procedure with the help of our consultant B & M Environmental, The Board Members are aware of
5 the extra cost of this service, but feel that it has been beneficial to our PSD. The extra manpower that his company provides saves the PSD a considerable amount of money throughout the year with the equipment and Class four operators he has on his staff The Company provides electricians, along with cranes, flow monitoring for manholes and other equipment that a small PSD cannot afford to have on hand. Tim Bosley of B & M Environmental has a staff of twelve employees, 2 Class IV operatiors, four Electricians. B & M Environmental maintains the plants seven days a week. The electricians are available to the PSD whenever needed. Mr. Bosley s staff does all process control, testing, reports and preventive maintenance. Mr. Bosley is working on several items to improve the system As always the PSD Board Members encourage there current employees to obtain their licenses, to advance their knowledge as well as their pay so that they can do many of the services that B & M Environmental provides. The Board has taken into consideration both the hiring of B & M Environmental and the advancement of one of the employees of the Enlarged Hepzibah PSD. They will evaluate this during the next several months as the Board feels this is a concern of the Public Service Commission. One of the major questions is if one of the employees does receive his license, the Board feels that he will try to use this License to apply for a position with another utility and the PSD will have to re-evaluate their concerns. Management and the Board Members have tried to encourage these employees to study and go for their Class I1 license. There is very little interest from the Employees. Sincerely Howard C. During March 2007, The Enlarged Hepzibah PSD applied for a 30-B to offset the Rate increase from the Clarksburg Water Board. The Clarksburg Water Board received their rate increase on November 3,2007 and our District was not able to put the increase into effect until January of This cost the District a considerable amount of dollars in higher water bills and a loss of a considerable amount of dollars from our customers. The District is continuing to replace current meters with the touch-tone meters at the rate of 6-12 per month. The Rockwell meters can be replaced at a discount rate when the old meters are turned in thus saving the district approximately $20.00 per meter. The Enlarged Hepzibah PSD is seeking a grant or a low interest loan to replace 1,000 ft. of 8 water line that is badly needed of replacement. With the help of Jim Wiemer of your Staff, the PSD was able to reduce the number of leaks on this section of pipe by decreasing the closing of the valve on our Hepzibah tank.
7 The PSD has had several water leaks since our last response to the Public Service Commission. The list of leaks are the following locations. 7. Dawson Mines (Rt 79) 2. Robert Martin (Meadowbrook Rd) 3. Guy Bumgardner (New Quarters) 4. Hepzibah Church (Rt. 79) 5. Mclntyre Camp (off Rt. 79) 6. Gore Straight (caused by Department of Highways) 7. Robert Davis (Rt 79) (caused by Gas Company) 8. Gore Straight (Rt. 79) 9. Katherine Minnite (Rt. 79) 70. Upper Erie 77. Rt 79 and Rt Spelter 73. John Owens (Meadowbrook Road) 74. Pete Dye Golf Course The PSD had a severe leak on Meadowbrook Road for several weeks. The District could not find the leak. The Bridgeport readings were very high. The District contacted Clarksburg water and WV Rural Water to help us look for the Leak. This leak was very expensive to the District. The District made several attempts to dig and try to locate the source of the leak. WV Water found the leak. After the repair of the leak, our Bridgeport readings were around 7/3 of the past readings. The Following Water taps have been installed since our last response. 7. Don Menendez 2. Danny Stout 3. Roy Beatty 4. Donald Friend. (Meter well repaired) The District Installed a Yard Hydrant for customer Mitch Blanton who lives on the end of the line and receives bad tasting water. We installed the Yard Hydrant to periodically flush out the line. The District installed an extra meter for customer Tom Mace to be used for a Sprinkler Sys fem. The District has started a Backflow Prevention Program. The District is contacting several of our Commercial and Industrial Customers regarding this matter. The PSD will supply a list of Backflow preventive companies for their consideration. Howard C. Mall
