According to the Engineer's Report for the Lakefield Water Treatment Plant, the raw water quality is generally low in turbidity with moderate alkalinity, colour and ph. The communities of Burleigh Falls and Young's Point are located upstream of the Lakefield Treatment Plant. These communities are on private septic systems. The closest municipal sewage outfall upstream of the Lakefield Water Treatment Plant is from the Village of Bobcaygeon, which is approximately 50 km away. The Otonabee River is part of the Trent Severn Waterway and, as a result, experiences extensive recreational activities (boating and cottages) during the summer months. lakefield DRINKING WATER SYSTEM Page 3 of 14

7 I"~ The raw water intake consists of dual 350 mm HDPE intake pipes (one duty and one standby) extending into the Otonabee River. The inlet pipes terminate in an intake crib that is located approximately 15 m from the shore. Both inlet pipes are equipped with zebra mussel control systems, air supply lines (for dislodging weeds), and raw water sample lines. Raw water from the intake is directed to a chamber located on the bank of the Otonabee River. A single 350 mm HDPE pipe directs water from the chamber into the Lakefield Water Treatment Plant. Site (Name): Comments: Treatment Plant Treated Water POE Sub Treatment Facility The Lakefield Water Treatment Plant is a surface water treatment plant which utilizes an Actiflo {floc/clarification) process followed by rapid sand/anthracite filtration and disinfection. Raw water from the Otonabee River is drawn into the Lakefield Water Treatment Plant through a bar screen via three vertical turbine low lift pumps (each rated at 20.8 Lis). A coagulant (aluminum sulphate) is added to the raw water at the low lift discharge header which is then directed to an in-line static mixer. The water is then directed to a flow splitting chamber which directs the water into two inparallel ballasted floc/clarification trains (Actiflo ). Each Actiflo train contains a sand and polymer injection chamber with a hydrocyclone for sand/sludge separation, a maturation tank and a settling tank equipped with 60 degree tube settlers. The water is then directed to one of three dual media sand/anthracite filters (each with a design filtration rate of 6.4 m/hr). Each filter is equipped with filterto-waste capability, air scour, flow measurement, head measurement, and effluent turbidimeter. A granular activated carbon (GAC) contactor is also provided for seasonal taste and odour control. When taste and odour control is not required, the GAC contactor is bypassed discharged directly into the clearwell. Sodium hypochlorite disinfectant is added to the filtered water (after filtration) at the head of the 1,000 m3, dual-celled clearwell. A solution of caustic soda is injected into the high lift pump chamber for ph adustment. Treated water is pumped into the Lakefield distribution system through a common 300 mm discharge header via three vertical turbine high lift pumps (each rated at 29 Lis at 63.4 m TDH). Site (Name): Comments: Clearwell Other Sub Pumphouse The Lakefield Water Treatment Plant is equipped with two independent and interconnected baffled clearwells with a total capacity of 1,000 m3 and a minimum level of 0.6 m. Under normal operations, treated water entering Clearwell 1 is directed into Clearwell 2, then into Pump Chamber 2 and finally into Pump Chamber 1. Each clearwell and pump chamber are interconnected and valved to facilitate isolation of each unit for cleaning and maintenance. Pump Chamber 1 is equipped with two vertical turbine high lift pumps (each rated at 29 Lis at 63.4 m TDH) and Pump Chamber 2 is equipped with a single vertical turbine high lift pump (rated at 29 L/s at 63.4 m TDH). All three high lift pumps direct treated water into the distribution system via a common 300 mm discharge conduit. 8 r''r: Comments: The wastewater disposal system at Lakefield WTP receives Actiflo clarifier sludge and filter backwash. The system consists of a surge tank and an expansion chamber, a wastewater clarifier, and a wastewater pumping chamber. The sludge generated from the settling tanks is re-circulated to the hydrocyclones where the sand is separated from the sludge. The recovered sand is reintroduced into the Actiflo process and the sludge is directed to the wastewater settling tank. Wastewater generated from filter backwashing is directed to the backwash tank and then into the wastewater clarifer where the supernatant (once subject to sodium bisulphite dechlorination) is discharged to the Otonabee River. The dewatered sludge is directed to the sanitary sewers for treatment at the Lakefield Sewage Lagoons. Site (Name): Comments: Distribution Other Sub The Lakefield Water Treatment Plant and Distribution System serves approximately 3,1 00 people through 1,100 service connections. The distribution system in the Village of Lakefield includes approximately 22,000 m of cast iron, ductile iron, PVC, and asbestos cement watermains and 115 hydrants. There are two pressure zones within the Village of Lakefield Distribution System. The primary pressure zone serves most of the Village and receives treated water from the treatment plant's high lift pumps or the standpipe. The primary pressure zone extends north of the Village, to the Lakefield College School (located in the Township of Douro-Dummer). A second, much smaller pressure zone serves Rolliston Street and Aldrich Court and receives treated water from a below ground pressure boosting station located at the intersection of Rolliston Street and Strickland Street. Site (Name): Standpipe Treated Water POE Sub Reservoir Comments: The Lakefield Water Treatment Plant has a standpipe located east of the intersection of Strickland and Rolliston Streets (in the northeast portion of the distribution system) to balance peak demand flows and provide firefighting flows. Treated water enters and exits the reservoir through a common 300 mm conduit. The standpipe is equipped with a pressure monitoring system that transmits a signal (indicating the reservoir storage volume) to the water treatment plant through radio waves, a recicualtion pump, rechlorination pump and mixing system. The 2001 Engineer's Report produced by RAL Engineering Ltd. (Newmarket, ON) states that the stand pipe has an effective volume of approximately 900 cubic metres. Site (Name): Comments: Elevated Tank Other Sub Reservoir The Lakefield Water Treatment Plant has an elevated tank located in the west part of the system that works in paralled with the standpipe to balance peak demand flows and provide firefighting flows. Treated water enters and exits the elevated tank through a common conduit. The elevated tank is equipped with recirculation pumps, rechlorination equipment and a mixing system. The elevated tank has an effective volume of approximately 2750 cubic metres and communicates with the plant SCADA system via radio. LAKEFIELD DRINKING WATER SYSTEM Page 5 of 14

9 ('~ INSPECTION SUMMARY INTRODUCTION * The primary focus of this inspection is to confirm compliance with Ministry of the Environment legislation and authorizing documents such as Orders and Certificates of Approval, as well as evaluating conformance with Ministry drinking water related policies and guidelines during the inspection period. The Ministry is implementing a rigorous and comprehensive approach in the inspection of drinking water systems that keys on the source, treatment and distribution components of the system as well as management practices. This drinking water system is subject to the legislative requirements of the Safe Drinking Water Act, 2002 (SDWA) and regulations made therein, including Ontario Regulation 170/03, "Drinking Water Systems" (O.Reg.170/03). This inspection has been conducted pursuant to Section 81 of the SDWA. This report is based on a "focused" inspection of your system. Although the inspection involved fewer activities than those normally undertaken by a detailed inspection, it contained most of the elements required to assess key compliance issues. Your system was chosen for a focused inspection during this inspection cycle because inspection findings over the past three years were such that the number of violations were minimal or non existent, there were few or no orders issued to you that were of significance in the maintenance of water potability and there were no deficiencies as defined in 0. Reg. 172/03. The undertaking of a focused inspection at your drinking water system during this year's inspection cycle does not ensure that a similar type of inspection will be conducted at any point in the future. An announced inspection of the Lakefield Water Treatment Plant (WTP) was started on January 19, 2015, for a review of the SCAD A system and all monitoring programs. An operator from the the PUS I, who is the main operator at the WTP was on site for the duration of the inspection. A follow up inspection of the physical plant, storage towers and documentation was performed on January 20, 2015 The Lakefield Drinking Water System was issued Drinking Water Works Permit Number , Issue Number 2 on July 12, The Municipal Drinking Water Licence Number , Issue Number 2 was issued on November 25, The Permit to Take Water, Number BHHG, was issued for the treatment system on June 5, 2013 and expires on October 26, CAPACITY ASSESSMENT * There was sufficient monitoring of flow as required by the Permit and Licence or Approval issued under Part V of the SDWA A review of SCADA information provided the required monitoring as indicated in the Permit and Licence. 10 CAPACITY ASSESSMENT * The owner was in compliance with the conditions associated with maximum flow rate or the rated capacity conditions in the Permit and Licence or Approval issued under Part V of the SDWA. The values which were reviewed during the inspection were within the required flow rates and the rated capacity as provided in the Permit and the Licence. TREATMENT PROCESSES * The owner had ensured that all equipment was installed in accordance with Schedule A and Schedule C of the Drinking Water Works Permit. * The owner/operating authority was in compliance with the requirement to prepare Form 1 documents as required by their Drinking Water Works Permit during the inspection period. A Form 1 is used by the owner of a drinking water system to document watermain additions, modifications, replacements and extensions that do not require a Schedule C application. These forms are required to be kept for a period of ten years and should be available for review by a Provincial Officer upon request. The criteria to be used by the owner/operating authority in determining if a Form 1 is required versus a Schedule C application is found in the Permit, Schedule B, conditions 3.1 and 3.2. The operating authority and owner should complete a Form 1 on any watermain additions, modifications, replacements and extensions that do not require a Schedule C application, moving forward. * The owner/operating authority was in compliance with the requirement to prepare Form 2 documents as required by their Drinking Water Works Permit during the inspection period. A Form 2 is used by the owner of a drinking water system to document minor modifications or replacements to the drinking water system that do not require a Schedule C application. These forms are required to be kept for a period of ten years and should be available for review by a Provincial Officer upon request. The criteria to be used by the owner/operating authority in determining if a Form 2 is required versus a Schedule C application is found in the Permit, Schedule B, conditions 4.1, 4.2 and 4.3. The operating authority and owner should complete a Form 2 on any minor changes/modifications/replacements within the treatment plant that do not require a Schedule C application, which are being performed currently and in the future. * Records indicated that the treatment equipment was operated in a manner that achieved the design capabilities required under Ontario Regulation 170/03 or a Permit, Licence or Approval issued under Part V of the SDWA at all times that water was being supplied to consumers. The SCADA trending was reviewed for the inspection period, and all readings were within the required regulatory limits, and all minor exceedances identified were noted in the logbook accordingly (i.e., calibration of equipment, backwash, power spikes/outages,etc) * Records confirmed that the water treatment equipment which provides chlorination or chloramination for secondary disinfection purposes was operated so that at all times and all locations in the distribution system the chlorine residual was never less than 0.05 mg/1 free or 0.25 mg/1 combined. 11 r'):...:: TREATMENT PROCESSES * The Operator-in-Charge had ensured that all equipment used in the processes was monitored, inspected, and evaluated. TREATMENT PROCESS MONITORING * Primary disinfection chlorine monitoring was being conducted at a location approved by Permit, Licence or Approval issued under Part V of the SDWA, or at/near a location where the intended CT had just been achieved. * Continuous monitoring of each filter effluent line was being performed for turbidity. * The secondary disinfectant residual was measured as required for the distribution system. There are two continuous chlorine analyzers on either end of the distribution system, one at the elevated tank and the other is located at the standpipe. Free chlorine readings are also taken weekly at these two sites. In addition, free chlorine readings are taken throughout the distribution system when weekly microbiological samples are taken for analysis. * Operators were examining continuous monitoring test results and they were examining the results within 72 hours of the test. The plants monitoring results are usually reviewed the next day at which time a CT verification calculation is also performed by the operator. * All continuous monitoring equipment utilized for sampling and testing required by 0. Reg. 170/03, or approval or order, were equipped with alarms or shut-off mechanisms that satisfied the standards described in Schedule 6. A review of the data provided indicated that the specific treatment train either shut down and/or alarmed to satisfy the regulatory requirements. An operator responded to the alarm/shut down within a reasonable amount of time. * Continuous monitoring equipment that was being utilized to fulfill 0. Reg. 170/03 requirements was performing tests for the parameters with at least the minimum frequency specified in the Table in Schedule 6 of 0. Reg. 170/03 and recording data with the prescribed format. * All continuous analysers were calibrated, maintained, and operated, in accordance with the manufacturer's instructions or the regulation. An external company also calibrates the continuous analyzers annually. In addition, the hand held units are also calibrated usually at the same time, which is an asset. The analyzers were calibrated April 15 and 16, 2014 and the hand held units were calibrated on March 13, OPERATIONS MANUALS * The operations and maintenance manuals contained plans, drawings and process descriptions sufficient for the safe and efficient operation of the system. * The operations and maintenance manuals did meet the requirements of the Permit and Licence or Approval issued under Part V of the SDWA. 12 ('~ LOGBOOKS * Records or other record keeping mechanisms confirmed that operational testing not performed by continuous monitoring equipment was being done by a certified operator, water quality analyst, or person who suffices the requirements of 0. Reg. 170/ SECURITY * The owner had provided security measures to protect components of the drinking-water system. The water treatment plant is located near the community arena and a park. The building itself is not fenced, but it is alarmed. The treatment plants intake pipe is located just upstream of a marina, and precautions should be in place to ensure that no anchors could affect the raw water supply. According to the operator, the use of chlorine gas for disinfection was switched to liquid, as the tanks may have provided a potential threat to the local landuse activities. The elevated tank located at 3362 Lakefield road within the distribution system is fenced and alarmed. The Strickland Street Standpipe is fenced and alarmed. The Rolliston Street Pumping Station is locked, but not alarmed. The owner/operating authority should consider installing alarms at these locations. CERTIFICATION AND TRAINING * The overall responsible operator had been designated for each subsystem. * Operators in charge had been designated for all subsystems which comprised the drinkingwater system. * Only certified operators made adjustments to the treatment equipment. WATER QUALITY MONITORING * All microbiological water quality monitoring requirements for distribution samples were being met. * All microbiological water quality monitoring requirements for treated samples were being met. * All inorganic water quality monitoring requirements prescribed by legislation were conducted within the required frequency. Inorganic samples were taken August 13, 2013 and August 26, 2014, within the prescribed time limits as required by O.Regulation 170/03. * All organic water quality monitoring requirements prescribed by legislation were conducted within the required frequency. Organic samples were taken August 13, 2013 and August 26, 2014, within the prescribed time limits as required by G. Regulation 170/03. LAKEFIELD DRINKING WATER SYSTEM Page 9 of 14

13 ('~ WATER QUALITY MONITORING * All trihalomethanes water quality monitoring requirements prescribed by legislation were conducted within the required frequency. Trihalomethane(THM) samples were taken as required. * All nitrate/nitrite water quality monitoring requirements prescribed by legislation were conducted within the required frequency for the DWS. Nitrate/nitrite samples were taken as required. * All sodium water quality monitoring requirements prescribed by legislation were conducted within the required frequency. As the operating authority has changed its Alum usage, to a more acidic alum, which seems to provide for better operation of the plant, the sodium levels may be increased as a result. The local Public Health Unit was made aware of this and have requested an increase in sodium testing at the plant. The standard for sodium is 20ppm, and to date, with this modification, no exceedances were reported. Sodium samples are required to be taken every five years, however, sodium sampling has been increased. The following sampling dates for sodium have been observed during the current inspection period; February 18, 2014 May 13, 2014 August 12, 2014 December 2, 2014 * All fluoride water quality monitoring requirements prescribed by legislation were conducted within the required frequency. A fluoride sample was taken August 26, * All water quality monitoring requirements imposed by the Permit and Licence or Approval issued under Part V of the SDWA were being met. * All sampling requirements for lead prescribed by schedule 15.1 of 0. Reg. 170/03 were being met. Two sets of Lead samples and the corresponding alkalinity and ph readings were taken on the following dates; - February 18, August 12, 2014 * Records confirmed that chlorine residual tests were being conducted at the same time and at the same location that microbiological samples were obtained. 14 ('~ t "> vr Ontario WATER QUALITY ASSESSMENT * Records show that all water sample results taken during the review period met the Ontario Drinking Water Quality Standards (0. Reg. 169/03). REPORTING & CORRECTIVE ACTIONS * Corrective actions (as per Schedule 17) had been taken to address adverse conditions, including any other steps that were directed by the Medical Officer of Health. There have been no reported or identified adverse conditions since May 22, * All reporting requirements for lead sampling were complied with as per schedule of 0. Reg. 170/03. * Where required continuous monitoring equipment used for the monitoring of chlorine residual and/or turbidity triggered an alarm or an automatic shut-off, a qualified person responded in a timely manner and took appropriate actions.
