3 1. Purpose and Application This procedure and the HSE Competence Standard it supports establish the minimum HSE requirements for competency and induction. The purpose of this procedure is to ensure all personnel working on, or visiting Arrow Energy workplaces have completed the necessary HSE inductions and hold the required competencies for the tasks they complete. This procedure covers: Induction requirements for Arrow workplaces (section 2) Site notification process (section 3) HSE Competence requirements (section 4) o Driver competencies (section 4.1) o Environmental competencies (section 4.2) o Emergency Management (section 4.3) Safety Critical Roles (section 5) o Technical Authorities (section 5.1) o CGPF Operators (section 5.2) o Permit Holders and Work parties (section 5.3) o Site Safety Managers/Site Managers (section 5.4) o Permit Authorities (section 5.5) o Operational Leadership (section 5.6) Assessment, Verification and Assurance (section 6) Records Management (section 7) Arrow employees and Mode 1 contractors are required to follow this procedure. Mode 2 contractors are required to have a documented procedure that meets or exceeds the requirements of this document and the HSE Competence Standard (ORG-ARW-HSM-STA-00005). The requirements of this procedure and its parent HSE standard are in addition to the legislative requirements for competency and induction. 2. Induction Requirements Arrow has different induction requirements for visitors 1 and workers. Any personnel completing physical work can be considered a worker for the purpose of this procedure. Physical work is where any of the following apply: 1. Any task that requires physical interaction with Arrow infrastructure, any use of machinery or tools, vehicles or other equipment (excluding office equipment in the office environment). 2. Any task that requires being unaccompanied on Arrow controlled sites. 3. Direct supervision of personnel completing the above tasks. Arrow requires mode 2 contractors to establish induction and visitor sign on processes for sites under their control. 1 Office based equipment such as computers, copiers and phones may be used by visitors. Visitors may take photos where required and safe to do so (physical maintenance, manual handling and electrical work should always be considered physical work regardless of location). Visitors may travel as a passenger in Arrow vehicles. Delivery drivers are exempt from the induction requirements outlined in this procedure. The Arrow SM/SSM, Contract Owner or Contract Holder may request delivery drivers to complete inductions if they deem it necessary (for example deliveries to remote worksites in field). Page 3 of 13
4 2.1 Visitor requirements Visitors may not complete physical work whilst on Arrow sites. All visitors are required to be accompanied by an Arrow inducted person. The Arrow inducted person is responsible for the activities and welfare of the visitor(s) whilst on the Arrow controlled worksite. Visitors may conduct non-intrusive inspections and audits. Visitors are required to complete the visitor induction applicable to the site in question. All Arrow personnel are expected to be completely inducted prior to completing any physical work. Arrow staff may be visitors at a particular Arrow controlled site provided they do not have to complete physical work. Arrow staff should consider the impact of visiting sites, staff with a need to frequently visit sites should complete the necessary inductions (and spend time to become familiar with the site and its hazards) to avoid burdening site personnel. When working in an office environment completing office tasks, continuous direct supervision is not required but the inducted Arrow person remains responsible for the visitor and must escort the visitor to the muster point in the event of an emergency Visitor Induction Vistors should complete the visitor induction for the site in question. Where activities for the visit do not fall within the definition of a visitor and is considered as physical work, personnel will need to complete the required worker inductions. The visitor inductions are available online via the Learning Space or the Arrow Escort/Supervisor can arrange for a stand-alone computer for visitors who are not able to access or connect to the Learning Space prior to arrival at site. This induction may be completed offline using the Offline Visitor Induction form (ORG-ARW-HSM-FOR-00212). Once attained this induction does not expire. When visiting Arrow s Corporate Headquarters visitors are required to complete a sign in sheet at reception. 2.2 Worker requirements Workers are required to complete the following inductions: 1. Face to face Industry Safety Induction (ISI) (section 2.2.2) 2. Arrow HSE Induction comprising two modules (section 2.2.3): a. First module - Arrow HSE Induction b. Second module - Arrow Facility/Field site specific induction. The direct induction provision may be used for low risk, short duration work or breakthrough work in place of the above inductions Direct induction Low risk, short duration work (less than 7 days) or breakthrough work may be conducted under a direct induction. Personnel may conduct physical work under a direct induction by the relevant Site Safety Manager (SSM). The SSM or their representative is required to run the inductee through key requirements detailed on the Direct Induction Checklist (ORG-ARW-HSM-FOR-00191). Personnel following the direct induction route may be inducted onsite. The direct induction form must be signed by both the inducted personnel and the relevant Site Manager/Site Safety Manager for the site in question. Page 4 of 13
5 Direct inductions for contractors are only valid at the site of the task and for the duration of the task in question (i.e. record of a direct induction cannot be used to meet induction requirements for future work, even at the same site). Records of all completed/signed off direct induction forms and attachments must be stored in an online folder via Share Point. Link to the folder is indicated in the form. Breakthrough work: direct induction may be used for any task that requires immediate attention such as urgent or emergency priority maintenance and incident response. A failure to plan does not constitute breakthrough work Industry Safety Induction (ISI) Industry Safety Induction (ISI) has been developed by the Competence & Behaviour Working Group of the Queenland National Gas Exploration and Protection Industry Safety Forum (Safer Together). It has been developed in line with a documented Safer Together Industry Safety Induction Standard. ISI is designed to promote strong and consistent safety culture, it outlines industry-accepted working behaviours, agreed standard of safety expectations and requirements and reduce duplication and inconsistency. ISI is a one day face to face session. It is delivered by either of the following two Safer Together approved providers: 1. Australian Well Control Centre (AWCC) Servicing Service Dalby, Roma, Toowoomba & Chinchilla Website: Phone: Wild Geese International (WGI) Servicing Moranbah, Brisbane & Gladstone. Website: Phone: Contractors can book onto the ISI by contacting one of the providers directly. Arrow personnel can request ISI online via the learning space. New Arrow employees must complete the ISI within 3 months of joining Arrow (if you are commencing a site based position then you will be required to complete ISI prior to travelling to site). This training does not expire. Office based contractors working exclusively in the Brisbane office (who do not complete any physical work at site) are not required to complete the ISI. Existing Arrow employees (employed prior to 30 June 2016) who weren t required to complete the Arrow 1 Day HSE Induction do not have to complete ISI. Contractors shall complete ISI unless they have previously completed one of the following: ISI Drilling & Completions General Induction (DCGI) Arrow 1 Day HSE Induction. Page 5 of 13
6 2.2.3 Arrow HSE Inductions (online) First Module - Arrow HSE Induction This module complements the ISI and is structured around general Arrow HSE requirements. It must be completed by all Arrow Employees and Contractor staff who are required to complete the ISI. The Arrow HSE induction module is a one-off requirement and once attained does not expire. The module is available online via the Learning Space and takes approximately one (1) hour to complete. Arrow will recognise 1 Day HSE induction with Baseline (for existing employees and contractors) as equivalent upon presentation of evidence eg. Induction records, cards, certificates and verification of records in the Learning Management System for Arrow staff. Second Module - Arrow Facility/Field Site Specific Induction The completion of this module is a mandatory requirement for Arrow and Contractor staff working in Arrow sites including offices. The module is available online via the Learning Space and will take approximately 30 minutes to complete. It covers functional description of all Arrow Facilities/Fields hazards, risks and controls across assets and emergency management for each facility (including offices). Completing this module will allow Arrow or contractor staff to access any one or all of the Arrow facilities to perform physical work. This induction is a one-off requirement and once attained does not expire. Offices Emergency Response Induction This module is a subset of the Arrow Facility/Field Site Specific Induction module and can be completed online via the Learning Space. It is required to comply with the requirement of the Building Fire Safety Regulation Existing Arrow employees and contractors working in Arrow offices who have completed the modularised site specific induction will need to refresh the emergency response portion of the induction via this 10 minute module every 12 months. Personnel/new starters who have completed the Second Module - Arrow Facility/Field Site Specific Induction are deemed to have completed this induction requirement for the first time Land Access Induction To comply with the requirement of the Land Access Code, this induction is a mandatory requirement for staff who work in the field and directly access land. However, this is not mandatory for staff who only undertake work within the offices at Healy Street and Thorpe Street. This is available online via the Learning Space. Once attained, this induction requirement does not expire. This induction is also mandatory for Visitors accessing land but the method and delivery of this requirement can be: Onsite by relevant Arrow Escort/Supervisor. Visitor Induction North or South online via the Learning Space or the Paper-based Offline Visitor Induction form (ORG-ARW-HSM-FOR-00212). Page 6 of 13
7 3. Site Notification (SN) Travel to Arrow controlled facilities/fields including offices in the Bowen and Surat Basin by Arrow or Contractor employees who not normally based in the facility requires site notification. This is to ensure that the location of personnel in the field is known. As a courtesy to Arrow s operating sites (Moranbah and Dalby) a Site Notification (SN) shall be completed 2 days prior to arrival. Site Notification Form is available electronically via a link in the Reservoir page and via the Supplier Portal. Recurring visits on a regular basis (planned day/time for the duration) can be covered by a single notification. 4. HSE Competency Requirements Arrow has identified specific HSE competencies and requirements which are required to operate vehicles, conduct environmental work and support crisis and emergency management. HSE competencies required for roles identified as safety critical are detailed in section 5 A detailed matrix of HSE competence requirements can also be found in HSE competency requirements table ORG-ARW-HSM-LRM Driver competencies All drivers of light and heavy vehicles are required to hold a current unrestricted motor vehicle license for the class of vehicle they are required to drive (the license shall permit you to drive in Australia). Mode 2 contractors are expected to have processes in place to ensure drivers hold and maintain valid licenses. Arrow personnel are required to notify their manager in the event of license loss. All drivers of Arrow vehicles (Light and/or Heavy) are required to complete the Online Driver Training module which is available on the Learning Space. This Online Driver Training is in addition to the training requirements listed in & below. Drivers of Arrow vehicles will not be issued an IVMS key until all training requirements are satisfied Light Vehicles Drivers of Light Vehicles require the following where applicable: RIIVEH201D- Operate Light Vehicles (1 day course) allows drivers to operate vehicles onroad only (this competency is addressed if the driver completes the competency below). RIIVEH305D- Operate and maintain a 4WD or PMASUP236B- Operate Vehicle in Field this 2 day course allows drivers to operate vehciles both on and off road. Vehicle and Machinery Hygiene online training module where vehicles are to be taken off road Light vehicle drivers who tow trailers should complete an approved training course such as AURTGA3001 Drive and manoeuvre trailers. ATV driver training where required. Page 7 of 13
8 4.1.2 Heavy Vehicles Drivers of heavy vehicles shall meet the requirements of the Heavy Vehicle National Law, including: Work and rest hours fatigue Management including fatigue management training chain of responsibility. Vehicle and Machinery Hygiene online training module where vehicles are to be taken off road Personnel who are involved in the Loading and/or Unloading of Heavy Vehicles shall complete Load Restraint Training. 4.2 Environmental Competencies Environmental regulatory obligations place specific competency requirements on Arrow personnel. The Environmental Authorities (EA) require that suitably qualified persons (based on both qualifications and experience) perform certain tasks including: Certification of plans, procedures and reports ecology surveys and mapping design of regulated dams and linear infrastructure in wetlands and watercourses development of environmental monitoring programs environmental monitoring fauna trapping, handling and relocation. There are other regulatory compliance obligations for competencies associated with Environment Protection and Biodiversity Conservation Act and Nature Conservation Act approvals including for the development of groundwater and biodiversity management plans and programs. Broadly, environmental competency requirements are met through the following processes: Arrow Technical Authority Framework: o HSE TAs (for project and operational environmental management plans, and air quality, greenhouse and noise speciality skills) o Engineering TAs (for regulated dam and linear infrastructure requirements). Arrow Training Programs (available via the Learning Space) including: o Inspections and reporting on embankment dam safety o management of spill control in the workplace o erosion and sediment control o environmental sampling and field testing o complete vehicle hygiene inspections. fauna and protected plants clearing permits issued under the Nature Conservation Act specialty accredited external training e.g. fauna handling registered Professional Engineer Queensland (RPEQ) scheme National Association of Testing Authorities(NATA) accredited laboratories for analysis of environmental monitoring samples Use of external providers with suitable qualifications and demonstrated experience with appropriate certification(s) in writing. 9 4.3 Emergency Management Personnel involved in Arrow s Crisis and Emergency Management response will be appropriately trained and competent to a level deemed appropriate by Arrow Corporate HSE Crisis and Emergency Management Teams (CMT/EMT) CMT and EMT members shall be trained in Arrow s crisis and emergency management response and the set-up and operation of the CMT and EMT rooms respectively. This training will be on appointment to the CMT/EMT and is to be refreshed annually On Scene Commanders Arrow staff determined to be likely to have to act as an incident supervisor will be trained in Arrow s On Scene Command arrangements this position has a mandatory annual refresher Fire warden Arrow staff will be trained as fire wardens for their facilities on an as-required basis. This will be by Base Building for 111 Eagle Street; or by a nationally accredited training organisation for the Thorpe Street and Healy Street Offices. This training must be refreshed every 6 months as recommended by AS Planning for Emergencies in Facilities First Aider Arrow staff designated as First Aiders will be trained in: HLTAID001 Provide Cardiopulmonary Resuscitation; and HLTAID003 Provide First Aid Training must be refreshed in accordance with certification requirements. 4.4 Medical Fitness for Work Field staff will be briefed on the rehabilitation and return to work processes at least annually via Toolbox talks or similar briefing sessions and must have access to a Rehabilitation and Return to Work Coordinator. Rehabilitation and Return to Work Coordinators (RRWCs) will complete training via an accredited training provider. The accredited training under the Australian Qualification Framework is BSBWHS407- Assist with claims management, rehabilitation and return to work programs. There is no requirement for this training to be refreshed. 5. Safety Critical Roles The tasks required to design, operate and maintain the Safety Critical Elements (SCEs) are safety critical and the roles of people who are accountable for these tasks are safety critical roles. Safety critical tasks relating to the operation and maintenance of front line barriers, including SCEs, which if not performed correctly, could lead to a Major Accident Hazard (MAH) are defined in the Hazard Management Guide, ORG-ARW-HSM-GUI Safety Critical Elements (SCEs) Barriers and controls that are in place to manage process safety related MAHs (hardware based pipe work, emergency shutdowns or isolation valves) Safety critical tasks design, operate and maintain the SCEs. Safety critical roles people who are accountable for the safety critical tasks. Page 9 of 13
10 Arrow HSE Competence Standard ORG-ARW-HSM-STA requires that safety critical roles are clearly identified and staffed by people who can demonstrate the required competence. Table 1 maps safety critical roles to positions within Arrow, competency requirements for those positions are described below. Specific requirements for HSE technical authorities are detailed in HSE Competency Requirements Table (ORG-ARW-HSM-LRM-00022). 5.2 CGPF Operators Identification, assessment and assurance of competency requirements relating to the operation of Arrow CGPFs is detailed in Operations Integrity function. 5.3 Permit Holders and Work Parties Arrow or Contractor staff working under the Arrow PTW System are required to go through the process of appointment and verification of competency. To be authorised and entered into the central Permit to Work Register as permit holders, following documents are completed and verified: Permit Holders: Permit to Work Role Appointment Checklist (ORG-ARW-HSM-FOR-00171) Completed Permit(s) and signed off while in training Copies of competencies as per HSE Competency Requirements Table (ORG-ARW-HSM-LRM-00022) Work Parties: Copies of competencies as per HSE Competency Requirements Table (ORG-ARW-HSM-LRM-00022) 5.4 Site Safety Managers (SSMs) and Site Managers (SM) Arrow formally appoints Site Safety Managers (SSM) and Site Managers (SM) with responsibility for managing HSE. SSMs are required under the Queensland Petroleum and Gas Act to supervise activities on operating plant. The regulatory obligations are detailed in the HSE Regulatory Compliance (ORG-ARW-HSM-PRO-00026). Site Safety Managers and Site Managers attend training to gain an understanding of the relevant legislative requirements of the their roles. The minimum competency requirements to meet Arrows obligations are outlined in ORG-ARW-HSM-LRM SSMs and SMs 5.6
11 are required to put together a portfolio to demonstrate attainment of competency. The portfolio goes through the process of assessment and assurance. For Category 2 Consultants acting as SSM/SM on behalf of Arrow for less than 12 months are not required to undergo the full program detailed above. They must complete Competence Assurance form Category 2 contract SSM ORG-ARW-HSM-FOR to provide evidence of competency before appointment. 5.5 Permit Authorities To be authorised and entered into the central Permit to Work Register as a permit authority, following documents are completed and verified: Permit to Work Role Appointment Checklist (ORG-ARW-HSM-FOR-00171) Completed Permit (s) and signed off while in training Copies of competencies as per HSE Competency Requirements Table (ORG-ARW-HSM-LRM-00022) 5.6 Operational Leadership Safety Leadership roles are assessed and assured against the following four competencies: 1. Risk Management 2. Design and Construction 3. Operations Inspection and Maintenance 4. Process Safety Culture. The details can be found in the Operations Competency Assurance Program. 6. Assessment, Verification and Assurance Assessment will, as far as practical and safe, be incorporated into the normal working day and should, therefore, occur as a natural part of the job rather than a special event. The main tool for assessing competence should be observation of staff performing tasks in the workplace by competent assessors. Observation should be supported by conversations to ensure that they can apply relevant skills and knowledge in a full range of circumstances encounted in the applicable role. As a minimum, assessor must possess the following: Hold a recognised assessor qualification (e.g. TAE40110 Cert IV Training and Assessment) Work experience related to HSE Technical competence in the discipline area. Records of Assessor qualifications are to be held in the Learning Space. Verification and assurance of competencies are detailed in the Arrow Competency Framework Manual ORG-ARW-HRM-MAN Where HSE Competencies are required, verification and assurance are conducted by Corporate HSE. 12 7. Records Management Records of verified competencies of Arrow Permit Holders, Permit Authorities are to be maintained in an online register. Records of all completed/signed off direct induction forms and attachments must be stored in an online folder via SharePoint. Link to the folder is indicated in the form. Work party competencies verification will be conducted by Permit Holders and/or Authorities with the help of CPCs as per the requirements in the Arrow Permit to Work process. All records of inductions and competencies are recorded via an online Induction and Competency Tracking Register within the central Permit to Work Register in Share point. To comply with requirements in Sec57AB and 57AC, Electrical Safety Act 2002 and Section 57A, Electrical Safety Regulation 2002, an online Electrical Compliance Register holds details of electrical licenses and electrical competencies of personnel staff both Arrow and Contractor performing electrical works for the business Page 12 of 13
