1 PAGE : 1 OF 8 ANNEXURE XVII INDUCTION PROGRAM SUPPORT STAFF INDUCTION FORM DEPARTMENT : NAME OF EMPLOYEE: JOB TITLE : DATE COMMENCED : This is a checklist of information for Induction which department heads should use with new staff as part of their induction programme at the time of their commencement of employment. The new employee should be asked to tick each subject as he/she has been informed about it, and each section should be signed by the person who has carried out the briefing for that section. The employee shall sign the form upon completion and this shall be countersigned by the department head and passed to HR dept for filing. Please read the guidance notes below before completing this form.

The main groups are detailed below, with particular points to take account of, highlighted. ITEMS SPECIFIC TO THE FOLLOWING GROUPS OF STAFF Support staff with no previous experience These staff may have no previous work experience and will need careful integration into the department. Discuss the following: 1. Role within the department. 2. Reporting responsibilities. 3. Allocation and prioritization of work. Managers and Professional Staff These staff needs a broader induction to put their post in context. 1. Structure and culture of department. 2. Role in relation to Department / Company as appropriate. 3. Training course in supervisory and management skills, if required.

DATE : PAGE : 8 OF 8 REV : 0 ADDENDUM TO CONTRACT OF EMPLOYMENT It is the Company s Policy and objective to firmly support the International Convention For The Prevention of Pollution from Ships (MARPOL) and operate their ships in a safe and efficient manner so as to prevent any pollution as well as protect the marine environment. 1. Employee clearly understands this Company Policy and agrees to comply at all times with all applicable rules and regulations, and with the procedures set out in the Company s manuals to prevent pollution as well as to protect the marine environment. 2. Employee shall not knowingly instruct, advice or encourage any other employee ashore or onboard the company s managed vessels to carry out any act which can or may result in a violation of MARPOL. 3. Employee undertakes to report to the Company immediately in case he observes onboard the vessel or in the office any wrong practices being followed or instructions being given, which can or may result in a violation of MARPOL. 4. Employee agrees that failure to comply with the above requirements will be considered as a breach of Company regulations, and may result in disciplinary action being taken against the employee, including but not limited to instant dismissal and / or legal action. It is clearly understood that the Company will provide all cooperation to Authorities in the prosecution of any Marpol violators.
