1 Healthy Living Pharmacy Assessments HLP Self Declaration Pharmacy Guide Introduction Community pharmacy contractors passing the gateway criteria will receive a Quality Payment if they meet one or more of the Quality Payment criteria. The Department of Health has weighted these based on an assessment of the difficulty of achieving them and the benefit to patients for doing so, with each criterion being designated a number of points. One of the Quality Payment criteria is: On the day of the review, the pharmacy is a Healthy Living Pharmacy level 1 (self-assessment). The Healthy Living Pharmacy (HLP) framework is a tiered commissioning framework aimed at achieving consistent delivery of a broad range of high quality services through community pharmacies to meet local need, improving the health and wellbeing of the local population and helping to reduce health inequalities. The HLP framework is underpinned by three enablers: Workforce development a skilled team to pro-actively support and promote behaviour change, improving health and wellbeing; Premises that are fit for purpose; and Engagement with the local community, other health professionals (especially GPs), social care and public health professionals and local authorities. A variety of paper based self-assessment and compliance assessment tools have been produced to support the process of accreditation. PharmOutcomes offers the contractor the ability to complete the assessments online, adding to the evidence portfolio as this grows. When the assessment is complete, an embedded link will take the contractor to the declaration page at the Royal Society of Public Health to submit a self-declaration of compliance. There are two frameworks that should be completed. Accessing the Frameworks The frameworks can be accessed at If you already have a PharmOutcomes login and your commissioner has elected to make the HLP frameworks available for use, you will be able to access these under the Assessments tab. If you do not have a login please contact the help desk team using the functionality under the Help tab. You do not need to be logged in to send a message to the help desk team, see below Fig 1. 2 From the home page click on the Assessments tab. Fig 2. Accessing the Framework Click on the Assessments tab to access the HLP Frameworks The Healthy Living Pharmacy frameworks will be visible in the main screen under the heading Frameworks. There are 2 views that are available for contractors to monitor progress towards meeting the standards in each framework: Fig 3 Accessing the HLP Frameworks Access assessment here 1. The Healthy Living Pharmacy Frameworks There are 2 of these: i. Quality Criteria Self- Assessment. ii. Assessment of Compliance These are used to record achievement of quality standards and to record evidence 2. The Summary of HLP report that is populated as an overview using the data recorded in the frameworks. Quality Criteria Self-Assessment This mirrors the published workbook but allows an electronic update of status and record of evidence to be maintained as progress is made through the process of accreditation. This is the assessment that records compliance and also evidence to back up compliance. This self-assessment should be completed alongside the assessment of compliance. Assessment of Compliance This is the assessment that matches the RSPH declaration of compliance

3 Completing the Quality Criteria Self-Assessment To record progress towards meeting each standard click the Update icon to the right of the framework title Fig 3. This reveals the framework itself Fig 4 below. The framework sits below the introductory text that provides a brief overview along with clickable links to further resources. Fig 4 Introductory text Introductory text Please note: Use of PharmOutcomes to record your progress towards meeting each criteria will provide information to your LPC who can view a progress overview on their dashboard/licence area. This will enable targeted support to help contractors meet certain criteria. There are 3 sections to the assessment that cover: 1. Workforce development 2. Levels of engagement 3. The pharmacy environment. Each section comprises of multiple parts. The pharmacy team should work through the assessment and answer each set criteria with regard to compliance or noncompliance. When the pharmacy has achieved one of the quality criteria, on answering the compliance question Yes the section becomes green, a No answer will show that section as red Fig 5 Fig 5 Recording compliance Green indicates Criterion met, red indicates Criterion not met

4 Recording your evidence Clicking any of the orange Reveal plans icons will reveal a section of the screen where providers can record their evidence type and location using the top tables - Fig 6. Fig 6 Recording your compliance and evidence Click Reveal Plans to record evidence Record evidence type here Any information recorded here will be visible to the commissioner Record evidence location here Anything recorded in either of these two boxes will only be visible to the pharmacy team. Any information recorded in the Commentary box below these will be visible to the service commissioner, this will be the licence holder in your area e.g. LPC. Clicking the blue Need Help icon will reveal further information for the pharmacy team member. When data has been entered clicking the orange Save button at the bottom of the screen will save all entered information allowing you to enter evidence and plans as each criterion is satisfied. Fig 7 Clicking save will save all entered information After entry of evidence or compliance always click to save The framework can be updated at any time and changes made to the profile will be altered in real time. Saving information will also populate the Summary of HLP report see later.

5 Viewing your status From the assessments page clicking the Status icon next to the framework title will show an overview of status of compliance with criteria set in the framework. Fig 8 below shows a status report for the Self-Assessment Framework. Fig 8 Status overview Links to reports The left hand side bar here shows a variety of links to different reports that can be produced. There are 2 different types of report, each in two different formats Print or pdf. The first report accessed by either of the two top links will generate an output that will show all evidence, action plans and commentary and the two lower links will display and allow printing of a report that shows information entered in the commentary box only see Fig 6 above. These reports are useful to produce at inspection meetings as they will summarise the content of any evidence portfolio detailing all evidence and where in the pharmacy this is stored. Completing the Assessment of Compliance The Assessment of Compliance can be completed in tandem with the Quality Criteria Self-Assessment. Essentially this form mirrors the recently released Declaration of Compliance to be submitted to the Royal Society of Public Health (RSPH) when all HLP standards have been satisfied i.e. all standards that appear in the Quality Criteria Self-Assessment. This framework must be completed in exactly the same way as the Self-Assessment Framework and is in 4 parts: Part 1 - details general standards that must be met Part 2 is split into 3 sections and details standards applicable to: i. Staff ii. Engagement, and iii. Environment Part 3 is the declaration to RSPH and Part 4 is a record of receipt of certification. NB: Some of the Assessment of Compliance duplicates the Self-Assessment Framework but both must be completed.

6 Complete parts 1 and 2 in the same way as the self-assessment. Please note that many criteria require free text entry. This is because the RSPH submission form is structured in this way and will allow copying of entered text into that form for submission. As quality standards are satisfied and completed they will turn green. This is reflected in the status report and summary. When parts 1 and 2 are all green you can make your Declaration of compliance to RSPH Fig 9 Completing the Assessment of Compliance Again any information entered in Our Action Plan or Our Evidence Record boxes remain private to the pharmacy team but any information entered in the Commentary box is visible to the commissioning organisation Record action plans and evidence as before NB: Only enter text when you feel you are compliant title from the Assessments page. A similar status view with links to evidence reports is also available for this framework by clicking the Status icon next to the framework Summary Report The summary report provides a snapshot view of overall compliance using information saved in the Self-Assessment and Assessment of Compliance frameworks. As each of these frameworks are updated the summary also updates. The framework title in this screen is clickable and when clicked will take the pharmacy team member to the framework itself to view recorded information in.

7 Submitting your Declaration of Compliance When your Quality Criteria Self-Assessment is complete, you have recorded all of your evidence and you have completed parts 1 and 2 of the Compliance Assessment you are ready to make your submission to RSPH. Clicking the link in part 3 will take you to the RSPH self-declaration page Fig 11.
