The format of the form should not be altered and data should only be entered in the relevant shaded cell as listed below. 2. Supporting information is welcome as an attachment to the form, bearing in mind the limits on answers to some sections. 3. All fields should be completed and submitted by the closing date. 4. When complete attach the form and to info@afl.com.fj and list the subject as the EOI number followed by your company name (eg EOI No. FJAA/2017 name). One form per .
