2 1. GUIDELINES Response to this questionnaire shall be completed in English by a senior member of your company. All documentation required shall be attached as specified. Responses shall be send on or before 12:00 hours 15-March-2018 to: OLNGSuppliersPre-Qualification@Omanlng.co.om JoelRivera@OMANLNG.CO.OM All information submitted with this questionnaire shall be treated in strictest confidence by Oman LNG. Oman LNG reserves the right to verify any and all information contained in responses to this questionnaire. Oman LNG shall be advised immediately should there be any significant change in the information provided in this document. It should be noted that this document is solely for the Pre-qualification evaluation of the company, for the purposes of inclusion on Oman LNG Tenderers Lists for the stipulated requirements. Submission does not guarantee that the contractor will be invited to Tender. 2. BRIEF DESCRIPTION OF SCOPE The contractor shall carry out the following scope of work, but not limited to: Carry out Engineering, Procurement and Construction (EPC) of the project comprising of the following three locations: Location-1: Car parking at Oman LNG site plant- Qalhat; consist of around 550 car parking slots. Location-2: Car parking at the New School in Al Ghalyla- Sur; consist of around 70 car parking slots. Location-3: Car parking at Hay Al Sharooq (HAS) in Al Ghalyla- Sur; consist of around 200 car parking slots. Carry out site survey and detailed engineering and all required engineering studies,surveys for the Civil scope and Electrical scope (Solar Photovoltaic panels, inverters, tie-in to existing system, transformers, RMU, LV panels and circuit breakers, etc.). Construction of the car park shed and install Photovoltaic Solar Panel at roof top of the shed and feed the solar generated power to the electrical network. Rerouting and termination of existing underground and above ground services. Cable trench excavation, routing of the Power Cables, instrument cables and all wiring connections. Carry out Engineering, Drawings & documents preparation, Materials & Services Procurement, Supply, Manufacturing, Fabrication, Shipping, Transportation, Delivery, Assembly, Construction, Installation, Inspection, Testing, Painting, Certification, Supervision, Management, Commissioning including supply of Testing equipment s, Special tools & Spare parts, Coordination and Training of COMPANY personnel for Operation & Maintenance. Contractor shall obtain all required permits from government authorities to complete the entire work and hand-over. Contractor shall be responsible for all tie-in works to Oman LNG grid for location-1 and to Local Distribution grid for location-2 & location-3 Pre-Qualification Questionnaire Page 2 of 6

3 3. PART I: SCREENING QUESTIONS 3.1 EXPERIENCE Does your company have a minimum of 10 years experience in Civil scope in the particular field being Tendered? Does your company have an EPC ability for the scope being Tendered? Does your company have a minimum of 10 years experience in 11KV Electrical systems scope? Does your company have a minimum of 5 years experience in Solar Photovoltaic panels design & installation scope? Is your company accredited by DCRP for 11KV Electrical systems?, please answer question below. Yes, please submit copy/evidence of accreditation Is your company accredited by DCRP for Solar Photovoltaic panels design & installation?, please answer question below. Yes, please submit copy/evidence of accreditation Do you intend to tie-up, as Joint Venture, with Subcontractor having DCRP accreditation and experience in 11KV Electrical systems scope for a minimum of 10 years in the particular field being Tendered? 4 3.1.8 Do you intend to tie-up, as Joint Venture, with Subcontractor having DCRP accreditation and experience in Solar Photovoltaic panels design & installation scope for a minimum of 5 years in the particular field being Tendered? Yes, please submit Subcontractor profile including list of its previous similar projects. 3.2 RESOURCES Does your company have sufficient resources of manpower, funds, equipment and materials to execute the described scope? Yes, please provide details of resources in tables and below. 3.3 HEALTH, SAFETY & ENVIRONMENT (HSE) HSE Policy: Does your company have a formal/signed HSE Policy document?, please answer questions and below. Yes, please submit a copy of HSE Policy [dated & signed by your company top management] What is your company s HSE Policy? If no HSE Policy exists, explain how you intend to deal with HSE-related issues Submit copy of your company s Organization Chart for this project with HSE Positions included in it. HSE Training: Submit HSE Training Matrix containing list of training programmes for this project / contract and targeted attendees Submit sample records of HSE Training attended by your staff and clearly indicate if these staff(s) will be working on this project. HSE Records: Submit your company s HSE Performance Reports (2 Sample Reports from last 2 years, either 2 monthly reports or 2 yearly reports). Pre-Qualification Questionnaire Page 4 of 6

5 3.4 QUALITY ASSURANCE Does your company have Quality Management System that is certified to ISO or equivalent? Yes, please submit copy of valid certificate for Quality Management System or equivalent. If Yes, the tenderer also confirms that he will apply the above quality management system to this tender. 3.5 FINANCIAL STATEMENT Please submit copy of the last two years Audited Financial Statement of your company and complete the information required in the table below. Current Asset Total Asset Current Liabilities Total Liabilities Year of Account (if Available) Inventory (Stocks), if applicable 3.6 COMMERCIAL REGISTRATION Please submit full copy of your company valid commercial registration. NOTE: Failure to answer or provide details or submit any required documents specified above (especially HSE) may result in disqualification in the Pre-Qualification.
