1 Illinois 4-H Leadership Conference Application for Planning Committee Committee Criteria: Applicants must be 4-H members ages as of September 1, Applicants under the age of 18 must have parental or guardian support. The adult s signature is required on the application. This Committee will be responsible for helping to plan the Illinois Leadership Conference for Illinois 4-H ers, scheduled for January 13-14, 2018 (Committee arrive Jan 12; Conference on Saturday/Sunday). Committee members will be expected to attend the entire event and assist with tear-down. Committee members will be expected to attend at least 75% of the committee meetings, including at least one faceto-face meeting. The majority of the planning meetings will take place via teleconference. The face-to-face meeting will take place during the summer of 2017 and MAY be a 2-day meeting with an overnight stay. Teleconferences will take place in the evenings during the fall. Alternates will be named in case a committee member cannot fulfill these responsibilities. Selection: The committee will consist of 6-7 young adults from throughout the state. Applicants are sought from throughout the state to represent the best of Illinois youth leadership. For the 2018 event, we will be looking for teens who attended the 2017 event. Selection will be based on a completed application, and Reference Evaluations. One reference must be from an Extension staff member OR 4-H volunteer leader; a second must be from some who can testify to the member s leadership abilities in one of the above mentioned competencies and their commitment to following through with responsibilities. Reference evaluations from family members are not appropriate and will not be considered. Completed applications must be received by May 15 in order to be considered. Applications must include the following items: Typed application form Current photo of applicant (A professional photo is not required.) A minimum of two Reference Evaluations (RE), as described on attached. At least one of the RE s must be a current Extension staff member or a 4-H volunteer leader; second must be from some who can testify to the member s leadership abilities in one of the above mentioned competencies and their commitment to following through with responsibilities. Reference evaluations from family members are not appropriate and will not be considered. Recommendation Evaluations s MUST be placed in a sealed envelope by the person completing the evaluation, with the signature of the evaluator across the seal.
