Resource arrangements for special situations such as shift handover or temporary works Is it clear how additional resources may be assigned if necessary? A4 Permits / Plans Have all relevant permits / plans been identified for all tasks that require them? If lifting is involved is the lifting plan referenced adequately and identified in the risk matrix? B Site Details B1 Access For all the locations and tasks, has the Work Package Plan identified all the necessary access points for: Staff Machines / Plant Materials Is there an adequate description for the access points to be identified for planning purposes? Has the way in which materials and resources will be transferred from the access point to the site of work been adequately described? B2 Site Layout Have all the site locations for this Work Package been adequately described for this stage in planning? Are grid references included? Are the layout and limits of the worksite(s) clear? Are any site diagrams or plans included adequate and of value? B3 Control of Site Risks Have the significant site risks been identified for the tasks listed in the matrix columns? Have the risk control measures been adequately described in a manner which is suitable for the person preparing the Task Briefing Sheets? Do all the risks listed relate just to the site? 3 need to protect the excavator from the site risk. If 'No', then what should be moved?) Have the risks been identified as relevant to the tasks by a cross or other kind of check mark? (If descriptions or anything other than check marks have been added in the task group columns, then this should be brought to the attention of the author as being incorrect.) Have all the significant site risks been identified for all the tasks listed? (If 'No', what should be added?). If engineering trains and on track plant are to be used, have the protection arrangements from the risks of these to others been adequately identified? B4 Protection and Isolation Has the need for protection from trains adequately been identified? Have the open line/ safe system of working been identified correctly? Has the planned protection arrangements been adequately described for each of the activities listed in the Work Package Plans? (It is not necessary to list the exact details but to describe the intended protection regime.) Has any further information been included, such as a line diagram, showing protection arrangements Is the need for any traction current electrical isolation adequately identified B5 On-Track Plant and Machinery Movements If on-track plant is to be used has the number, capability and type of machines required for each task, been clearly identified to allow proper planning and resourcing? If machines have to travel some distance to and from a worksite, has the method of transit been addressed? If additional information is required, such as cant, curvature or gradient data, has it been adequately identified or included? B6 Communication and Contact Details Is there a project organisation chart? Have the site personnel contact details been included in this Work Package Plan? Are the supervisory and off site contact details adequate? If protection is required, has the requirement for ES, PICOP and COSS etc. been identified adequately? Has the Network Rail AMCC contact number been supplied? Is there a contact number for each of the contractor companies? Have any relevant utility company contact numbers been included? (E.g. Electricity, Gas and Water etc?) Have any relevant government agency numbers been included? (E.g. Environment, BTP, HMRI etc.) B7 Emergency Arrangements Have all appropriate emergencies and contingencies been identified?
