These customers are classified by First Light as OEMs (Original Equipment Manufacturer s). When First Light establishes products and pricing for our OEM customers we categorize it using an O to denote OEM. O to denote that the Lamps and/or Quartz are produced and sold to First Light s OEM customer for use in the OEM s equipment Unique product produced by First Light exclusively for our OEM customer will not be sold to anyone other than the OEM unless agreed to by our customer. In some cases, the product purchased by the OEM is generic by design and is already marketed widely by resellers and other OEM s. First Light sells products to companies that market our product as after market replacement components for systems build by equipment manufacturers for whom First Light does not have a direct relationship. Products produced by First Light for this market are sold by value-added resellers and select distributors and are categorized three ways: S To denote that the Lamps and/or Quartz are matched to a Specific manufacturer s equipment and can be sold as a direct and compatible replacement. A complete list of S items and prices is available to ours customers upon request. SX To denote that the Lamps and/or Quartz are matched to a Specific manufacturer s equipment and can be sold exclusively by SX Authorized Partners as a direct and compatible replacement. Q - To denote by First Light that the Quartz is generic in nature and can potentially be used to replace quartz in various and different pieces of equipment installed in a variety of applications. ItemID Structure & Definition First Light uses an accounting and order entry system that allows us to create unique product numbers for each product we sell. Since we manufacture to order and each lamp and quartz can be made with unique characteristics for our customer (technical specifications, etch, onlay, base color, packaging specifications, validation, etc), we create a unique ItemID for each product each of our customers purchase. Below is a grid that provides you a broad overview of our numbering scheme so that you can review your order acknowledgements with confidence. Custom Lamp Etches and Charges Standard Lamp Etch - First Light Technologies, as a standard, etches each UV lamp with a Manufacturer s Date Code, a Mercury and Mercury Disposal Warning, marking and a Caution to protect from UV: Maximum Etch Size.59 x 2.92 CAUTION: PROTECT EYES FROM RAYS There are no additional charges for First Light s standard etch. First Light s Manufacturing Date Code YY =2 Dig Year, WW = Week # 1-52 (Example: F01-11) Made in the Symbols alert to proper handling and disposal of a lamp containing Mercury Custom Etches - Customer Specific & Item Specific Etch Option. As a value-added service and at no additional cost to our customer, First Light will provide customers with a custom laser etch for each lamp type manufactured. Etches provide important information that allow you to easily and effectively capture repeated business. First Light has incorporated the use of Laser Etch Profiles for each of our customers. The Laser Etch Profiles allow our customers to provide clear instruction to First Light on the information that is to be etched on each lamp. Each customer s Laser Etch Profiles will be maintained in a database and clearly communicate essential information about the characteristics of the etch: Key: P/N CAUTION: PROTECT EYES FROM RAYS A distributor customer may likely have multiple profiles, one with their company logo and one for each of their customers ordering lamps with a private label. LE1 Used to identify a custom etch (ie private label, generic layout, etc) LE2 Used to identify a second custom etch (ie private label, generic layout, etc) LE3, LE4, LE5 etc. may be used to uniquely identify additional custom etches required by any one customer. LEF Used for all First Light Branded Lamps (additional info follows) Each product you purchase from First Light has a unique Item ID in the First Light system. Each ItemID will have a corresponding Laser Etch Profile Number providing clear direction on the Laser Etch to be applied to the product. ItemID Specific Markings will be easily and accurately maintained in a corresponding Customization Database so the etch on all your lamps with the same Laser Etch Profile will look the same except for the number you use, if you choose to, to uniquely identify each of your products. Below you will find an example of the Laser Etch Specification Form that our Laser Etch Production Operator will see when preparing to etch your lamp. Please notice that it is specific to a unique ItemID. The customer specific Laser Etch Profile number is included in the Product Description which appears on all Sales Order Acknowledgements and Invoices allowing the customer to confirm that the etching instructions and information are correct. sales@firstlightusa.com Page 4 of 9

First Light Branded Products - Custom Etches are not available on an exclusive and limited group of First Light Lamps and Quartz that directly replace the original manufacturer s aftermarket lamp/quartz. First Light branded products will be etched using the LEF Laser Etch Profile which will be unique to this First Light Branded Lamps. This represents a unique opportunity for First Light and our partners and branding as such will reinforce the markets acceptance of quality and compatibility. For these products, First Light will use a First Light Etch. A list of these lamps is available, to our partners, upon request. Individual Packaging First Light will, upon request provide special packaging allowing our customers to purchase lamps and quartz pre-packaged for individual sale. This value added service can include product labeling. Below please find the standard additional charge, per item (lamp/quartz), for individual packaging. This box is used by First Light to safely ship small quantity shipments of long lamps and/or quartz via small package carriers (UPS, Federal Express, DHL, etc.). With the Safeguard box and the small package carrier of choice (based on the destination and level of service(s)), First Light has been successful in reducing the overall shipment cost. The analysis of the overall cost savings realized by our customers includes the additional charge of the Safeguard Box. ItemID Description Price Safeguard Box Safeguard Box $75.00 A Safeguard Box fee is charged when it is determined based on the size of the order, and most cost effective, to direct a shipment to UPS, FedEx or select trucking companies. Product shipments with a length of 52 are packaged using a certified safeguard box to ensure against breakage for lengthier shipments. Alternative crating and skidding of shipments result in higher freight charges and longer transit times. If Sales knows that a Safeguard box is needed, then the fee will be added to the Sales Order. Otherwise shipping makes that determination. Unless otherwise instructed, First Light will make the determination to utilize the Safeguard Box based on the characteristics of the shipment. This decision will be made to assure the safety of the products being shipped, minimize the transit time and reduce the overall cost of our customer s shipment. When this determination is made, First Light will include the Safeguard Box as a line item on the invoice and separate from any applicable freight/shipment charges. First Light will not require our customers to modify their purchase orders to include this incremental Safeguard Box charge and will expect that any applicable Safeguard Box charges invoiced by First Light per this policy will be paid without unnecessary delay. sales@firstlightusa.com Page 6 of 9

7 Validation Service Policy The validation of ultraviolet output is a process by which UV lamps undergo an extended 60 hour, factory based, burn-in process. After the 60 hour burn-in the UV output of a sample quantity of the lamps is measured and validated to be within a specified range. The output is measured under specific testing criteria based on the characteristics of the lamp. Testing criteria include: radiometer, input voltage, ballast, distance from the lamp, timing of the output reading. Validated Lamps will be etched with a validation number, placed directly above the date code. The Val# is unique to the production order. A sample of the etch placed on a validated lamps is shown below: Val# P/N CAUTION: PROTECT EYES FROM RAYS A Validation Report will be shipped with the order and will be attached to the order packing slip(s). Reprints of the report are available upon request and may be provided in hardcopy or digitally. First Light will validate lamps being ordered for a fee of up to $10.00 per lamp. Customers will be required to order a minimum of 12 validated lamps per lamp specification and will still be required to meet the minimum order quantity, per lamp specification, inclusive of the 12 or more validated lamps. Order Acknowledgements will display both the validated and non-validated lamps separately with pricing being increased by up to $10 for each of the validated lamps. Sample Lamp Policy In an effort to support our customer s requirements for sample germicidal lamps and remain efficient and profitable, First Light has developed the following policy: Any standard germicidal lamp with existing manufacturer specifications will be provided to the customer in 30 piece quantities for sampling at standard volume pricing determined by First Light. This will exclude any existing customer s proprietary lamps and (or) custom types unless pre-authorized by that customer. Any lamp, which does not have an existing specification and requires R&D, may be subject to a production lot charge of $ This fee will include the start of (12) production lamps and any R&D, which may be required. 8 ending lamp totals may vary by 10% depending upon manufacturing shrinkage. This $ charge will be credited in the future once the customer reaches a minimum purchased amount of $ for that lamp type. Sample lamps will be manufactured with standard lead-time of 4 to 6 weeks to maximize production efficiency. These samples may be available earlier depending upon inventoried items and production schedules of similar lamp types. Lamp Warranty First Light Technologies uses only certified materials in the manufacturing process of its germicidal lamps. guarantees that all merchandise shipped from our Poultney, Vermont facility meets any and all industrial standards specified regarding ultraviolet lamps and their germicidal effectiveness. guarantees all lamps against defects in quality and workmanship. Our lamps will equal or exceed the performance of any other manufacturer. All Coated Lamps have a rated life of hours unless otherwise specified. Rated average lamp life is defined as the elapsed operating time under essentially continuous operation (one on/off cycle every 8 hours) for the output to decline to 80% of the output the lamp had at 100 hours. Lamps which are defective should be returned to in accordance with Return Policy (see Order Policies). Credit for defective lamps will be issued on a prorated basis. The manufacturer s liability is limited to the lamp, and shall not exceed the cost thereof. First Light will include the CE marking on the lamp etch for all First Light Branded lamps. (see section labeled First Light Branded Products) All lamps manufactured by First Light are NOT CE marked as many lamps are etched and labeled for exclusive distribution by companies other than First Light Technologies, Inc. Lamps displaying identifiers (Company Name and/or Registered Trademarks) other than First Light Technologies, Inc s information will only be CE marked at the written request of the First Light customer who s markings are displayed on the lamps as it is this/these companies that will be responsible** to demonstrate conformance and maintain a Technical File. ** - The manufacturer has sole and ultimate responsibility for the conformity of the product to the applicable directives, whether he designed and manufactured the product himself or is considered as a manufacturer because the product is placed on the market "under his name".
