All locations are expected to produce a similar document, with the relevant contact details and information duly filled in for quick reference in case of an emergency There can be many types of emergencies that an agent may be expected to encounter. This chapter deals with some of the most likely emergencies that can occur on board ships. Name of Master, Owner, Operator, and P+I club of the vessel. C. Last Port departure date and time, ETA arrival Port D. No of stowaways discovered. E. Name, Nationality and passport number of each stowaway, if known. F. Any other travel / Identification document found in their possession. G. Does Master intend to hand them to authorities here or take them back to country of stowaways' domicile? Advise Master not to include the stowaways' names in Crew/ Passenger list and to advise above details in writing (if not done already). II) Advise General Manager and Assistant Port Manager, if not notified yet. Advise of above details on phone, followed by letter (Official letter-head) to: A. Marine Police of the port B. Port Immigration Authority C. Concerned Embassy / Consulate - - D. Harbour Master (if any) III) Advise above details to Owners/ Operators; advise them the local regulations pertaining to stowaways. In case additional funds will be required to attend / make arrangements, request principals for the same. IV) Keep in regular contact with Marine police / Immigration authorities for all arrangements including repatriation if possible. Ref: ISS/F/003 Revision 04/04 Page 4 of

(Most of the information required will be available within the earlier communication with the vessel, documentation on arrival) Vessel in Territorial waters / At Berth B. Name of Master, Owner, Operator, and P+I club of the vessel. C. Last Port departure date and time, ETA arrival Port D. Name of Cargo, Position of spill, approximate amount of spill, If spill is contained / stopped. E. Current Position, Course and Speed of vessel. Present weather conditions F. Any other information pertinent to the situation. Advise modes of contacting us (VHF, Tel, Fax, telex and Emergency Response No.). Master to be given a single point contact number. III) Advise of above details on phone, followed by letter (Official letter-head) to: A Port Control Tower B Harbour Master C Coast Guard / Marine Police D Terminal Control IV) Advise vessel's owners / operators / Principals (be precise in reporting) of incident. V) Ask Master if vessel needs any assistance (e.g. communication, etc). Arrange to place a local mobile phone on board. VI) Keep in close touch with the situation (without burdening vessel with excessive communication) and maintain log of each and every activity / event. VII) Keep Owners / Operators / Principals advised of the current situation at all times. Ref: ISS/F/003 Revision 04/04 Page 5 of

Position and time of incidence. C. Name of Master, owner and P+I club of the vessel. D. Ship & or cargo damaged (Type and extent of damage), if ascertainable. E. Any casualty or immediate danger foreseen to ship's staff, ship or cargo. F. Location of helicopter landing area on board, if available. G. Any back up or assistance required. H. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). I. Any distress, urgency or safety message transmitted to shipping in vicinity. Advise modes of contacting us (VHF, Tel, Fax, telex and Emergency Response No.) Master to be given a single point contact number III) Advise of above details on phone, followed by letter (Official letter-head) to: A Port Control Tower B Harbour Master C Coast Guard / Marine Police D Terminal Control IV) Advise vessel's owners / operators / Principals (be precise in reporting) of incident. V) Ask Master if vessel needs any assistance (e.g. communication, etc). Arrange to place a local mobile phone on board. (If possible and requested for) VI) Keep in close touch with the situation (without burdening vessel with excessive communication) and maintain log of each and every activity / event. Present position, direction and speed of vessel. (If applicable) C. Name of Master, Owner, Operator and P+I club. D. Name of ship's section & or cargo under fire (Type and extent of fire). E. Any casualty or immediate danger foreseen to ship's staff, ship or cargo. F. Any back up or assistance required, including for fire fighting. G. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). Advise modes of contacting us (VHF, Tel, Fax, telex and Emergency Response No.) Master to be given a single point contact number III) Advise of above details on phone, followed by letter (Official letter-head) to: A Port Control Tower B Harbor Master C Coast Guard / Marine Police D Fire Brigade E Terminal Control IV) Ask Master if vessel needs any assistance (e.g. communication, etc) and if any distress, urgency or safety message transmitted and any responses to it. V) Advise vessel's owners / operators / Principals (be precise in reporting) of incident. Name, Call Sign and Port of Registry of vessels involved in collision. B. Position of incidence, and if both ships still at the same sight. C. Name of Masters, Owners, Operators and P+I clubs of both ships involved in collision. D. Ships' sections & cargoes damaged (Type and extent of damage to ships). E. Any casualty or immediate danger foreseen to ship's staff, ship or cargo. F. Location of helicopter landing area on board. G. Any back up or assistance required. H. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). I. Any distress, urgency or safety message transmitted to shipping in vicinity. Advise modes of contacting us (VHF, Tel, fax, telex and Emergency Response No.). Master to be given a single point contact number Advise of above details on phone, followed by letter (Official letter-head) to: A Port Control Tower B Harbour Master C Coast Guard / Marine Police D Terminal Control III) Ask Master if vessel needs any assistance (e.g. communication, etc) and if any distress, urgency or safety message transmitted and any responses to it. Present position, direction and speed of vessel. (If applicable) C. Name of Master, Owner, Operator and P+I club. D. Name, Nationality, Sex, Passport no of casualty. E. Nature and extent of injury. F. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). Advise modes of contacting us (VHF, Tel, fax, telex and Emergency Response No.). Master to be given a single point contact number. III) In case casualty needs immediate medical attention and doctor to go on board contact any of the following doctors: Hospital Name Address Telephone Fax Sulmaniya Hospital Manama, Bahrain American Mission Hospital Manama, Bahrain IV) In case casualty needs to be evacuated: A. By Launch Advise launch (by VHF and/or phone) to immediately proceed to the ship. Once underway, the details of ship's name, position, casualty to be passed on by VHF/Phone etc. B. By Helicopter Full Helicopter evacuation service contact details V) Advise Master to send us summary of situation and nature of assistance required on a telex/fax and in case of evacuation advise Master to land casualty's passport and seaman's book (along with casualty). Keep Master advised of assistance sent and ETA of launch/helicopter, etc. VI) Make all arrangements ashore for receiving the casualty, immigration clearances, paramedics etc VII) In case of evacuation by launch, arrange an ambulance to transport casualty from launch station to designated hospital. VIII) On discharge of casualty from the hospital, all arrangements with immigration authorities to be completed, to repatriate to hometown if required. IX) Request principals for funds required. C. Name of Master, Owner, Operator and P+I club of the vessel. D. Name, Nationality and passport no of person feared dead and his next of kin. E. Time of incidence / discovery of incidence. F. Any causes known or injury visible on body. G. Name and address (phone, fax, telex) of crewing agent, if applicable. H. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). Advise modes of contacting us (VHF, Tel, fax, telex and Emergency Response No.). Master to be given a single point contact number. Advise of above details on phone, followed by letter (Official letter-head) to: A Marine Police B Immigration Authority C Harbour Master D Port Security Manager KBSP,Hidd E Concerned Embassy / Consulate III) Advise these details to vessel's owners/ crewing agents. Please be very precise in the message text and request them to advise next of kin and advise by return the disposal instructions for the body (send home or local burial/ cremation). In case of local burial/ cremation, a letter authorizing the same will be required from next of kin/ embassy/ consulate. IV) Advise Master to prepare the following: A. Incident report - and keep it ready for police. B. Prepare a list of all items belonging to person feared dead (witnessed by two persons). C. Pack belongings in airworthy units (fit to be air freighted) with packing lists. V) Police along with court official will investigate the death and then the body will be transported to Government hospital / police mortuary (please keep log of each event and keep owner / crewing agent advised of situation). VI) After a police formality are over, the body will be handed over for burial / cremation, sending back to the next of kin, with all relevant documents. Description of Pirates and their vessel if any. D. Name of Masters, Owners, Operators and P+I club. E. Any casualty and nature of same. G. Back up and details of assistance required. H. Modes of contacting vessel in case it is required (Inmarsat, VHF, etc.). I. Any distress, urgency or safety message transmitted to shipping in vicinity. Advise modes of contacting us (VHF, Tel, fax, telex and Emergency Response No.). Master to be given a single point contact number Advise General Manager and Assistant Port Manager and owners /operators of the vessel, if not notified yet.
