1 QUESTIONS AND CLARIFICATIONS ITB Ref. UNFPA/CHP/ITB/15/ Is there a requirement of specific color for the samples? There is no standard requirement for foil color and the standard color for condom procured by UNFPA is plain foil. Any special packaging including color may be requested at the PO stage to meet a specific customer requirement. 2. Is there any specific requirement about the printing on the condom samples? I.e manufacturer name, lot number, etc. Please submit samples that are compliant with WHO/UNFPA specification for individual packaging detailed on pg. 33 of the WHO/UNFPA Male Latex Condom Specification. 3. May our company's laboratory test results be taken as independent lab test results? No. Independent laboratory test results are not a mandatory requirement but should be submitted with the samples if they are available. 4. The sample inner box is just one box for 144 pieces. Shall we add another sample inner box for 100 pcs? Please send only a sample box for only 144 pcs. 5. Page 16 clause Registration details of the company - Could you please clarify whether this clause is requiring the company's registration license? Yes or any similar document including the name of the manufacturer, company registration number in the country of origin and the product being manufactured. 6. Page 13 clause 16.1 "Bids must remain valid for 120 calendar days after the Bid submission deadline" while annex A "We agree to abide by this Bid for a period of 90 days from the date fixed Information in Annex A has been corrected and document uploaded in UNFPA etendering Platform. As per clause 16.1 Bids must remain valid for 120 calendar days after the bid submission deadline. 7. Even if no logo or brand name should be printed on the outer foil, we understand you re looking for a private label already registered in that country. Is this correct? UNFPA cannot procure private commercial brands but only non-branded condoms. UNFPA considers a country registration valid when the product is registered by the bidder and/or a third party who allows use of this registration by UNFPA without cost. Naturally, this valid registration shall enable UNFPA s country offices or Third Party Client to import the goods to the destination. 8. List of the countries required Registration for condom. We don't have information about countries that required registration for condom. Would you please share this information? It is bidder s decision/strategy to decide in which countries to register. However, in the bid document Annex G. UNFPA indicates a list of countries where registration is a priority for the organization.

2 9. As manufacturer, we were audited and approved for prequalification by UNFPA, but at that time, we only applied and got approval for 52 mm natural condom without flavor. Our product width range is from 52.4 to 53.0 mm. We would like to ask if we are eligible to offer for 53 mm condoms considering our current pre-qualification status of 52 +/-2 mm? Can we also offer for other similar condom types, like 49 mm natural color, condom with flavor or scent, ribbed and dotted condoms? If the offered variation was not part of the prequalification submission, it should clearly state in the bid that this variation was not originally declared in the product dossier and the manufacturer should submit recent internal and external test reports (from ISO accredited laboratory) for at least 3 Lots that show that they meet the WHO/UNFPA requirements. Please note that UNFPA may request additional information or clarification during the evaluation. 10. Price Structure. For 100 pcs box: We have noticed that in price structure you have written price per gross, however the unit of measure is 100 pcs. In this case do you mean gross box here it means 100 pcs box (& not relates to 144 pcs)? Price structure needs to be submitted in the Price Schedule Form Annex D tab Price Structure. The price structure needs to reflect different costs embedded in the price of the condom (for example, what percentage of the price per condom refers to raw materials, electricity, labor, operational costs, etc.). 11. Annex VII. Countries of registration. Please advise us if the products are not registered in some of the countries? In the document Countries of Registration, suppliers need to indicate Yes, No or In Progress for each country. In case the bidder marks yes it is necessary to submit a copy of the registration. In case the registration belongs to a third party client the supplier needs to submit a letter confirming the registration can be used by UNFPA without any cost. Any other remark can be included in Supplier Comments. 12. How many samples do we need to submit? In the bidding document pag. 17 clause 20.1 says 4 samples while in the Annex F Product Overview says 10 samples? Maximum 4 pieces/design. Document Annex F Product Overview has been corrected and uploaded to UNFPA etendering Platform. 13. There is a difference between the name of Section VI. Annex A on clause and the name of Section VI. Annex A on clause Annex A is Bid Submission Form while Annex B is Bidder Identification Form. Please disregard Section VI Annex A Bid Submission form in clause Most favored pricing. Is there any certification from bidder need to be submitted to prove this? There s no need to submit any document but suppliers commit when signing an LTA and bid offer. 15. How to state the valid of this bid for 120 days? Bidder needs to submit and sign document Annex A. Bid Submission Form.

3 16. What content should we include in the financial bid except the Annex D. Price Schedule Form? The financial bid must include only the Annex D. Price Schedule Form. The Annex D needs to be submitted to the secure address indicated in the ITB document. 17. We can use unregistered winrar to compress the bidding documents into a.zip format. Could you please check whether it can be opened successfully? Bidders are allowed to use any programme to compress the documentation submitted for the technical section. UNFPA can only see the documentation uploaded in UNFPA etendering system once the bid deadline has finished. In case UNFPA experiences problems to access/open any document we will inform supplier accordingly. 18. Can we submit warehouse and waste treatment system improvement and up grading from the last qualification? Documents must be in line with ISO ISO or ISO within a 6 months of bid submission is a mandatory requirement to sign a Long Term Agreement with UNFPA. Suppliers need to demonstrate the continuous improvement on waste water treatment systems. 19. Shall we submit 4 pieces of samples for each item (including condomize) for evaluation? Please refer to Question and Answer 12. Bidders do not need to submit samples of Condomize design. 20. Annex C. The word format attachment we downloaded from the e-tendering platform is slightly different from its version included in the ITB the ITB UNFPA/CPH/ITB/15/041.pdf, kindly confirm which version we shall use. Please use the amended word document uploaded in UNFPA etendering platform. 21. In regards to Condomize Designs: 1) 53mm, black condom, LOGO CONDOMIZE B/W, can we provide W52mm, Length >170mm to represent the W53mm, length >180mm samples? 2) 49mm lemon scent, yellow condom, can we use banana flavor to represent the lemon flavor considering that we have not lemon flavor in stock presently? Please, refer to question We have noted that you have specified that the gross box without plastic lining will have the condoms stacked vertically. The photograph shows only one layer. Would it be possible to indicate the size of the gross box and the size of the shipping carton so that to a great extent the packing will be standardized for all suppliers? The condoms will be packed in a single layer. The dimensions would be: Length to 235mm Height height of a strip of 3 condoms + maximum 10 mm buffer

4 Width length of a strip of 3 condoms + maximum 5mm buffer Condoms have to be not packed in a manner that they are not overloaded nor leave too much empty space in the gross box and there should be space for 10 folded leaflets. The gross boxes could be of Snap bottom or five panel folder. Where there are flip over flap panels they should be of adequate area to support the contents of the gross box. 21. UNFPA_CPH_ITB_15_041_for_male_condoms_and_lubs.xml Event bid package ; we cannot downloaded nor open it. Please kindly send us directly this file to this address or give us instructions. Kindly note the file with extension.xml is created by default by the system and is not necessary to submit this bid. 22. We would like to express the interest of participating in male latex condoms bidding exercise. We would like to l hearing more from you about the quantities. Are you required to supply the condoms manufactured by the factories known by WHO? Please refer to the section III Spend analysis and demand forecast to have an estimated idea about the procurement volumes and forecast. Regarding the eligible bidders for procuring, kindly refer to section 2. Eligible Bidders. 23. According to ISO4074:2015 latest requirement, the individual package shall mark at least: manufacture's name, lot number, expiry date while according to Clause (Page 34 of UNFPA/CHP/ITB/15/041): Individual package materials and markings No logo or brand name should be printed on the outer foil unless otherwise stated in the Purchase Order and specifically requested by UNFPA. The individual package should adhere to requirements in the latest version ISO 4074 standard and the latest WHO/UNFPA Male Condom Specification and should have the following markings: - Manufacturers name and registered address - LOT number, (printed at the time of packaging, not pre-printed) - Manufacturing date (year and month). - Expiry date (year and month). - Shape, texture and color if condom is not natural, smooth or parallel sided 1)if jet-printing on side of foil, this could meet ISO4074:2015 requirement Company s name LOT:XXXXXX RIBBED MFG:MM/YYYY EXP:MM/YYYY 2) if jet-printing on both of foil, this might meet requirement of clause front foil:

5 Company s name City, country back foil : LOT:XXXXXX RIBBED MFG: MM/YYYY EXP: MM/YYYY 24. We want to know whether the above both labels meet this bid requirements? The bid documents state the registered address of the manufacturer this is an address which give the actual location of the manufacturing site. Regarding jet-printing, in addition to the labeling requirements in the bid it must comply with current ISO Can UNFPA accept the samples with not- fresh manufacturing date, is there requirement about samples' manufacture date, such as from 2015? Yes it is acceptable, as long is the stock is not older than 12 months from date of the bid publication. 26. Should we submit all the documents with our company letter head? It is not a mandatory requirement to include company s letter head in the documentation submitted. 27. Is it possible for the documents to be uploaded and saved in the e tendering platform and finally submitted when all the documents are ready before the due date of submissions? Kindly follow the guidelines indicated in the clause of the ITB document and upload the compressed file once your technical bid is completed. 28. Glows in the Dark design appears in Annex F. Product Overview but not in Annex D. Price Schedule form. Please clarify. The design Glow in the Dark has been deleted from the form Annex F. Product Overview to be aligned with Annex D. Price Schedule Form. 29. We have capability to produce Condoms listed in the tender except Glow in Dark. Do we need to provide sample specific as per the list provided or can we provide samples on Plain White Foils or can we provide samples from our own Brand current production run? Kindly note Glow in the Dark design condom is not requested in this bidding exercise. Secondly, please provide the samples as they would be for regular procurement- plain foils. If this is not possible then provide explanation and commitment when the appropriate samples will be submitted. 30. In case of multiple Factories producing Condoms, do we need to submit our Bid separately for each Factory or will one submission be sufficient. Similarly, also in the case of sample submission do we need to submit samples separately from each Factory?

6 In case of being a male condom manufacturer with different factories UNFPA/WHO prequalified you have 2 different options: 1. You can submit only one bid as a group including both factories. However, only one Price Schedule Form can be submitted with a single and same set of prices, meaning that ALL factories need to offer the same prices for each variation of condom offered. Technical information needs to be submitted for each factory but only a unique set of same prices applicable to both manufacturing sites is acceptable. i.e. Factory A 53 mm male condom needs to have the same price as Factory B 53 mm male condom. 2. You can submit two independent bids for each company/factory. This would be the way to go for a Group which has more than one WHO/UNFPA Prequalified sites but wants to offer a different set of prices for each factory. Each company/manufacturing site should submit its own bid. Each bid submission needs to include the Technical documentation and the Financial Offer. I.e Factory A submits a Bid for its products and Factory B submits another bid. In this case Factory A can submit a different price as Factory B as they are completely different bids and as such will be evaluated. 31.All required documents returned with the Technical bid should besubmitted in PDF version. But in it also indicates that the technical bid should be submitted in PDF version and /or excel version. And al so in Section VI: BidandReturnable Forms, Annex D to Annex I preferred file for submission in PDF & Exce l. Please refer to Section VI. Bid and Returnable Forms in the ITB document. 32.In Section I, A. Male condom manufactures, it should provide the summary data on the last 30 lots of products manufactured at that site. The data should be from at least My question: On the last 30 lots means the latest 30 lots condom we manufactured or it can include the condoms we manufactured in 2014 to 2015? This 30 lots summary data should be from 2014 or it can also provide from 2015? Can we provide this summary data from UNFPA orders products? Yes, you are free to provide data on UNFPA orders. It should be from one year only e.g. if you manufactured more than 30 lots in 2015 then use the 2015 data BUT if you manufactured less than 30 lots in 2015 then use for 2014 and 2015 data. 33. In Section I, : Product catalogues containing pictures of the condoms offered, Can we provide the samples pictures which we will send them to UNFPA. Or it needs more information? We would like to have pictures where we can see the product and not only when it s already been packed in the foil. 34.As each gross box shall include instruction for use, we also need to send the inner box and the sampl es to UNFPA, should we also need to provide the instruction for use leaflet in gross box to UNFPA?

7 One sample of the standard condoms packed in the gross boxes as intended to be supplied. The information and user instruction as per the WHO/UNFPA specifications 2010 page 35 need to be submitted with the samples. 10 leaflets per gross box. 35.In excel Annex G Bid Scoring Form, No.4. Supplier capabilities of WHO/UNFPA prequalified manufact uring sites operated by the supplier, we would like to know what is the scoring criteria for this content In the document Annex G. Bid Scoring Form criteria 4. Supplier capabilities, the supplier is obtaining 30, 50, 80 or a maximum of 100 points depending on the number of different designs the bidder offers in the Price Schedule Form. 36. As for the samples submission, we are able to submit the various types of condoms including the 49 mm COLOR but we could not come up with the tender samples for this type as we need to dip for a very little quantity, please advise. If this color has been declared in the product dossier at the prequalification stage, this is acceptable. However, please note that you will have to provide samples to UNFPA QA at the purchase order stage. 37. ITB A. article 2: Summary data on the last 30 lots of products manufactured at that site. Do it means the newest 30 lots of condoms that manufactured according to the WHO2010 or ISO 4074: 2015 spec? Yes, for manufacturers that have been WHO/UNFPA prequalified and have supplied condoms to the WHO/UNFPA 2010 specification please supply the information as per WHO2010 (with the correngendum). Yes, for manufacturers that have been WHO/UNFPA prequalified and have not supplied condoms to the WHO/UNFPA specification please submit the information as per or ISO4074: 2014/2015. In this case please provide reasons for producing condoms to the WHO/UNFPA specifications Lot Sequence No.: does it means the consequence of the lot in total 30 batches? If so, shall we fill 1, 2, 3 UP to 30? Yes. The spirit of the sequencing is to enable UNFPA to assess whether the Lots were manufactured in sequence. 38. In which Column can we fill the number of nonconforming products? i.e. Can we fill Failed sample qty/total tested qty in column of pinhole, air inflation, package integrity and visible defeat? In instance: 1/315 means: 1 non conformity in total 315 pcs of samples tested Yes, failed sample quantity/total tested quantity format is acceptable, e.g. for FFH if two condoms had holes out of a 315 sample then it is presented as 2/ Shape & Texture, color, scent: shall we fill the exact character of the products (plain surface, red, yellow or strawberry flavored)or just fill pass/fail data?

8 Pass/fail data. 40.A list of similar contracts executed for other clients, including contract details. Can we provide the domestic family planning commission relevant bids contracts for this requirement? In clause we intend to see that the bidder has the adequate capacity and expertise. Therefore, kindly send all documentation you might consider fulfils this request and in case we need additional information we will approach the bidder during the clarifications stage. 41. Event Details (file attached). We don t see any explanation on how to fill in and how to submit the Event Details part but I find it in the e-tendering after clicking Bid on Even there are lists of Event questions. In each question, there is the Add Comments or Attachments. Should we attach the corresponding document to each question at this point of event question? Or, do we need to provide the documents as one.zip file as Technical Bid. Please refer to the UNFPA etendering Bidder Guide Submitting a bid. 42. The event question includes the final part of pricing. Do we need to fill it or just leave them blank? DO NOT SUBMIT any prices for male condoms or lubricants in the UNFPA etendering Platform Only Technical bid needs to be uploaded in UNFPA etendering Platform. The Financial offer needs to be sent to the secure address specified in the ITB document. 43. Technical Bid. How to upload the file for technical bid into the e-tendering? We have gone through the UNFPA etendering Manual for Bidders but don t see the instruction for the upload of the Technical file separately. Please refer to the UNFPA etendering Bidder Guide section 3. Submitting a bid. First, you need to answer all the responses in the event questions with the ideal response and then you will be able to upload your technical offer.

9 44. UNFPA etendering Manual for Bidders VS. UNFPA etendering User Guide. We received from the solicitation time 2 files concerning the How to use etendering. One is mentioned as UNFPA etendering Manual for Bidders and another one is UNFPA etendering User Guide. Please confirm our understanding that we should only focus on the Manual for Bidders one. Correct. Use only the UNFPA etendering Manual for Bidders 45. Price Schedule Form for box of 100 pieces. We see it is mentioned INFORMATION USED FOR INFORMATION PURPOSE ONLY so Is it necessary to fill in this form for box of 100 pieces? Yes Is the sample of box of 100 pieces be required? No 46. In Annex VII: Countries of Registration. Is it be necessary that we include the certificate of registration for the countries we mention as registered? Yes it is a mandatory requirement to consider the registration valid and acceptable. 47. For country that we are processing the registration, will there be necessary to include the evidence of registration application? UNFPA will only consider a registration valid and acceptable during the technical evaluation when the bidder has marked YES and the documentation submitted proves that UNFPA can use this registration. Other else is for information purposes. 48. Is it necessary that the condoms registered in each country have exactly the size of either 53mm or 49mm. (For example, if we have registration for size 52mm)? Kindly submit and inform in the column Supplier comments. 49. In case the certificate of registration does not mention the size of condom, is it o.k.? Kindly submit and inform in the column Supplier comments. 50. Bid Scoring Form. Referring the event question and it mentions Have you completed and updated the Bid Scoring Form? Kindly confirm if we need to fill column [c] Points obtained by the Bidder and mark on countries details. Complete column C and submit in excel and pdf format. 50. Bid Scoring Form, if the registration is for the brand owned by our customer for supply from our factory, could we count as our registration? Please refer to Annex E. Countries of Registration form. 51. Section VI Price Schedule Form - In case we would like to bid as one bidder but supply from 2-3 manufacturing sites, should we submit our Financial Bid with Price Schedule Form for each manufacturing site? Please refer to Question and Answer 30

Technical Bid - In case we would like to bid as one bidder but supply from 2-3 manufacturing sites, should we attach all required documents within the Technical Bid of all manufacturing sites in the same Event Questions? Or should we have to register each manufacturing sites as a Supplier User in the e- tendering system? Please refer to Question and Answer Registration details of the company, kindly clarify which details of registration to be provided under this clause. Please refer to Question and Answer Evaluation, how UNFPA will decision if bidder/supplier get low score on number of having countries registration, number of products and quality (as specified in bid scoring form) at low bidding price while other supplier/bidder get higher score on having number of countries registration but their price is higher, for example; - Company A. - Bid Scoring form Points 500 Points -price $3,60/gross - Company B. -Bid scoring form Points 400 Points -price $3.40/gross How will the scoring form points will be used with bid prices to shortlist suppliers for LTA award The technical evaluation is done separately from the financial evaluation. Criteria such as countries of registration, performance and total manufacture production capacity are part of the technical evaluation, whereas the price is assessed in the financial evaluation. The technical evaluation will be done with basis on the criteria set out in Section I, of the ITB. The maximum number of points a company can obtain in the technical evaluation is 500. The financial bids will only be opened for those bidders who obtain at least 300 points in the technical evaluation. The financial bids will then be evaluated with basis of their responsiveness to the Price Schedule Form. The maximum number of points a company can obtain in the financial evaluation is 500. The total score for each bidder will then be the weighted sum of the technical (60%) and financial (40%) scores. 55. Is there any weightage given to the track record on quality and service? The only criteria considered in this bidding exercise are the ones included in the ITB document and in the Bid Scoring Form. 56. Enter bid line responses How do we respond to this event? We understood that we have to send Financial Bid contains the price of each types of condoms and lubricant separately to address Bidtender@unfpa.org.

Please refer to clause 13. Incoterms in the ITB document. 58. Do we need to send samples of those variants which are not mentioned in the price schedule form and Product Overview Form? Please refer to clause 20.1 in the ITB document. 59. Please let us know where we can find the UN Supplier Code of Conduct because it is not possible for us to check this code by clicking at the link on Clause 2.6. "UN Supplier Code of Conduct". Link Please advise us whether or not UNFPA will notify all amendments on this bid by to all prospective bidders if some clauses or others are amended regardless of E-tendering platform. It is bidder responsibility to check all the documents and amendments of the event in the UNFPA etendering platform 61. Should we prepare samples with all colors or all flavors we can manufacture? Please refer clause 20.1 in the ITB document. 62. Before the later stages during the technical evaluation, should we send UNFPA a sample inner / gross box with samples of condoms Please refer to Clause 20.1 in the ITB document. 63. We accepted this bid invitation on UNFPA e-tendering platform. But we do not know how we can acknowledge receipt of the Invitation to Bid through UNFPA e-tendering platform no later than 10 March 2016 and indicate whether or not a bid shall be submitted as stated in the 2nd page of the ITB. Please guide us more detailed information. Please refer to the UNFPA etendering Bidder Guide section 2.2. Accepting Bid Invitation. 64. Clause says For easy evaluation, all required documents within the Technical Bid should be submitted in PDF version and/or excel version and be compressed in a.zip file and uploaded in UNFPA e-tendering platform.can we use a.rar format instead of.zip format? Please refer to question and answer Each event question has its own Add Comments or Attachments. If all required documents should be compressed in a.zip file, can we upload it to the 1st question and comments on other question that please refer to attachment of the 1st question?

12 Please refer to questions and answers 41 and 42 and review UNFPA etendering Bidder Guide Section What is the limited size of each attachment uploaded in UNFPA e-tendering platform? (i.e. 8MB) Unlimited capacity 67. Clause says "Written Confirmation from the Bidder that they are not listed in any of the banned/suspended supplier lists. (Section VI-Annex A. Bid Submission Form)". Do we need to submit a separate Written Confirmation together with Bid Submission Form? Annex A. Bid Submission Form is sufficient. 68. Clause Copy of last 3 years audited company Balance and Financial Statements. Whereas, UNFPA e-tendering platform mentions Have you submitted a the last 2 years audited Balance and Financial Statements? Please clarify. Please submit copy of the last 3 years audited company balance and financial statements. 67. Clause "If no instance of previous past performance has resulted into advers actions, this must be clearly indicated in the Bidder's bid." Do we need to submit a Written Document (i.e. Statement) for this requirement? Yes 68. Clause "A complete list of registration by product and subsequent updates shall be the Supplier for coordination purposes."do we need to submit a separate complete list of registration for male Condom as Technical Bid documents? No, this is requested to awarded suppliers only. 69. Clause "The bidder warrants that the cost of packing and palletizing is included in the cost offered for the items." Do we need to submit Written Warranties (or Statement) for that? No statement is needed. Bidders need to consider that cost of packaging and palletizing must be included in the cost offered for the items. 70. Should we submit only ONE Annex D: Price Schedule Form mentioning the Prices per Gross (not the Prices per 100 pieces)?

13 Please refer to Section VI. Suppliers need to submit only ONE Annex D including the information for the Sales of Unit Gross of 144 and Box of 100 units, if available. Only Price per gross will be considered in the Financial Evaluation. 71. According to Section IV: UNFPA Special Conditions of Contract, ACESSRH. Do we need to submit written Confirmation for that as Technical Bid document? It is implicit when submitting and signing Section VI- Annex C 72. Could you please let us know the condom type for the following items? * Item number 2: 53mm COLOR - Condoms, male, according to UNFPA/WHO specification 2010, revised April 2013, and ITB UNFPA/CPH/12/026. * Item number 7: 49mm COLOR - Condoms, male, according to UNFPA/WHO specification 2010, revised April 2013, and ITB UNFPA/CPH/12/026. * Item number 12: 53mm CONDOMIZE 2 DANCERS - Condom 53mm, two dancers front foil, UNFPA logo back. Condoms, male, according to UNFPA/WHO specification 2010, revised April 2013, and ITB UNFPA/CPH/12/026. * Item number 13: 53mm, black condom, LOGO CONDOMIZE B/W, white writing on black background) reg. size, black condom - normal lubricant. Condoms, male, according to UNFPA/WHO specification 2010, revised April 2013, and ITB UNFPA/CPH/12/ Where can I find the required templates or draft documents used for responding to a condom tender? My team needs them to review to familiarize ourselves with what is required and the process. Please refer to the event 546 in UNFPA etendering Platform. 74. Are there any form documents or draft documents that are preferred by the UNFPA for Joint Bids? Please refer to the section 2. Eligible Bidders. 75. We do not know which are the documents we need to submit to this bid neither where to find them. Please advise or suggest the proper manner to handle. Please refer to question and answer number We note that the name of manufacture and registered address are required to be printed on the foil, for preparing the samples, do we require to print full name and full address? Yes, It should be the name of the manufacturer and the registered address. 77. Water Based Lubricant. 14 It should be safe for use with latex, polyurethanes and synthetic materials. Please note that the lubricant may be used by uses of female condoms which are varied in the materials used. 78. Refer to Technical Bid Clause and Section VI Annex I: Bid Reporting Template. Will the summary data on the last 30 lots be from any product size, any texture, any color, and or any scent? Do we need to submit 30 lots based on samples of condoms given? The summary of the 30 lots has to be from the condoms size stated in the bid and should be for lots of the same size, same shape, same color and same surface texture. 79. Clause 20 - Samples required. For Samples required, Can we send samples from our current production lot in any foil? Or should we produce new samples packed in foil with exact printing requirement i.e. name of manufacturer and registered address, lot number, Mfg. Date, Exp. Date, etc.? Samples should be as per the UNFPA requirements of foiling and labelling. If this is not possible, please provide what is available and provide a justification for the deviation from the requirement. 80. Clause Does UNFPA require any special format for manufacture date and expiry date? i.e. mm/yyyy or yyyy/mm As stated under section of the bidding documents it should be year and month (yyyy/mm) 81. Foil design Centering. In case we have capacity limitation for doing centering as per artwork mentioned in bidding document, can we continue participate in bidding? Yes, your company can still participate in the ITB. Kindly submit a quote for the condoms variations/designs you can manufacture 82. Section VII Lubricant shall we offer 4.5 gram per sachet instead of 2ml-5 ml? The sachets have to be of the volume 2-5 ml. If you choose to offer based on weight, then please provide the density of the lubricant as well as the conversion calculation of the 4.5gm to milliliters.

15 83. Although we have not manufactured the lubricants, we have contracted with the manufacturer of the lubricants in our country. In this case, kindly advise us whether or not we can participate in this bid for both the male condoms and the lubricants if the manufacturer of the lubricants are eligible to supply their lubricants to UNFPA through our company. Please refer to 2. Eligible Bidders and 33. Award of Long Term Agreement clauses. 84. Please advise us if we should prepare 4 pieces for each samples per each type / design / variation. If yes, should we prepare samples with all colors or all flavors we can manufactured? Samples have to be on the types/designs/variations that were included in the dossier at the time of prequalification. 85. Before the later stages during the technical evaluation, should we send UNFPA a sample inner / gross box with samples of condoms? Please refer to clause 20. Samples required about delivery deadline. 86. Do we need to provide each proposed type/design/variation of the male condoms' samples together with our Internal/In house Certificate of Analysis for all the samples (batches) and independent laboratory test results, if available? Please refer to question and answer Do we need to provide Control Chart together with Annex I: Bid Reporting Template? Yes 88. May we submit only one gross box with (or without) printings at the actual dimension together with samples of each 4 pieces? If box is not printed, please provide a justification and a date when the gross box with printing will be provided. Regarding dimensions, please refer to question and answer 20.
