Each prime bidder is responsible for distribution of information conveyed by this Addendum to its subbidders and suppliers. Attendees included Kraig Beilharz (Beilharz Architects, Inc.), Greg Williamson (LLSD Superintendent), David Miller (LLSD Treasurer), David Grismore (LLSD Building & Grounds Superintendent), Tim Frost (Baker-Shindler), Andrew Reinhart (McDonalds Design & Build), Tim Schroeder (Schroeder Masonry), Bob Luttfang (Schimmoeller Construction). ITEM NO. 2: Bid Form Replace the Bid Form in the Project Manual with the attached revised Bid Form. Refer to Section item below for revision of Alternates. ITEM NO. 3: Section Alternates Paragraph 1.02.C: Revise Alternate 3 to Alternate 3A. Revise description of alternate as follows: 1. Remove light switch and wiring to approved junction box location above ceiling. 1. Relocate diffuser and grille to new SAT ceiling; provide ductwork as required; refer to 3/M Existing individual shower rooms: Remove four switches and wiring to approved junction box location above ceiling. ITEM NO. 6: Sheet A-2 Schedules and Details Room Finish Schedule: 1. Floor Material Legend, mark E: Refinishing and sealing of exposed concrete shall include removal of existing paint. 2. Shower 104: Change south and east wall finishes to GB (new); remove existing Glazed Block and install new GB matching new GB on other walls. Patch ceiling at removed areas; paint. 4 4. Bids to be good for 60-days J. PROJECT DESCRIPTION & SCOPE OF WORK 1. Project estimate: $265,000 (all Primes) 2. Scope of Work: See Section K. CONTRACT TIME SCHEDULE 1. Notice to Proceed: a. After Board Meeting early April. (Board meeting is April 8, 2013 at 7:00 PM.) 2. Substantial Completion: July 1, Final Completion: 7 days after Substantial Completion 4. Liquidated Damages: see Section L. OWNER-SUPPLIED PRODUCTS (see Section ) Material receipt, storage, installation, etc by Contractor 1. Lavatories and faucets 2. Toilet paper, sanitary napkin, soap dispenser, paper towel dispenser and sanitary napkin dispenser 3. Note: Owner supplied products are only applicable to the alternate bids; otherwise they are furnished as scheduled under the base bid) M. WORK RESTRICTIONS 1. Interior work in existing building areas during school hours shall be strictly limited to the works and staging areas. 2. Interaction between contractors (including subcontractors) and School staff, students and personnel is strictly prohibited. (There will be zero tolerance regarding this matter) 3. This is a tobacco-free campus! 4. Loud music is prohibited during school hours 5. Hours of non-school activity: 6. Use of Site; limited to staging area. Parking as approved by School. a. All access / parking shall be by the SW corner of the work area adjacent to locker room 102. b. Owner will provide access to the school; no keys will be issued to contractors. c. There shall be no trailers utilized as existing area in the school has been designated for contractor use. d. Dumpsters shall be located in the parking area designated, however it shall be removed one week prior to graduation (May 19, 2013) and placed back in this location afterwards. e. Work hours shall be as follows: (unless otherwise agreed to by the School) i. 5:00 AM to 11:00 PM until May 22, ii. 6:30 AM to 4:00 PM for the remainder of the project. f. Attempt to avoid loud tools and equipment during school hours; however it is not a requirement. School starts at 8:00 AM and is out at 3:04 PM. g. Motorized vehicle / machinery are not permitted. Provide ventilation for any motorized power tools. N. PROJECT MANAGEMENT AND COORDINATION (see Section ) 1. The General Contractor is designated as the single prime and shall have responsibility for coordinating all contractors. 2. The GC is the custodian of the building and premises; protect existing finishes 3. The General Contractor shall conduct all construction progress meetings. 2 P a g e

Shop drawings for Special joists shall include loading conditions, stress diagrams, calculations, member sizes, types and sizes of gussets (if any), connectors, and an analysis of all combined compressive and tensile forces QUALITY ASSURANCE: A. Perform Work in accordance with SJI Specifications and tables, including headers and other supplementary framing. B. Erector Qualifications: Company specializing in performing the work of this section with minimum five years documented experience. C. Design Special joists, drift loads, concentrated loading conditions, field connections, and other special conditions indicated on the Drawings, under direct supervision of a Professional Structural Engineer experienced in design of this work and licensed at the place where the Project is located. 1. Live load deflection for Special joists shall not exceed SJI Specifications. Maximum allowable total load deflection shall be as follows: a. Roof Joists with Gypsum Board or Plaster: 1/360 of span. b. Other Conditions: 1/240 of span DELIVERY, STORAGE, AND HANDLING: A. Deliver, store, protect, and handle products to site under provisions of Section and in accordance with SJI requirements. B. Coordinate delivery schedule of built-in items, such as bearing plates and anchors, with installer. PART 2 PRODUCTS 2.01 FABRICATORS: A. New Columbia Joist Company. B. Open Web Joist Members: SJI Type K, parallel chord; underslung ends with top chord bearing. B. Fabricate to achieve end bearing of 2½ inches on steel; 4 inches on masonry. C. Develop camber in accordance with SJI Specifications for all joists. Camber is not optional. D. Provide bridging for all joists in accordance with SJI Specifications for size, type, and spacing. 1. Provide bridging anchors at ends of bridging lines FINISH: A. Prepare joist component surfaces in accordance with SSPC procedures. B. Shop prime joists. 1. Do not prime surfaces that will be field welded, or in contact with concrete or grout. 2. Do not prime surfaces indicated to receive spray-applied fireproofing. PART 3 EXECUTION 3.01 EXAMINATION: A. Verify existing conditions prior to beginning fabrication or erection. B. Verify that supports are properly installed and braced prior to beginning erection ERECTION: A. Coordinate placement of anchors in masonry construction for securing bearing plates. B. Erect and bear joists on supports. C. Erect joists in accordance with SJI Specifications and OSHA requirements. D. Allow for erection loads. Provide sufficient temporary bracing to maintain framing safe, plumb, and in true alignment. E.
