H RENOVATION OF THE VALLEY STREET STUDENT CENTER FOR DENTAL HYGIENE ADDENDUM #1 PART 1 - GENERAL Project: Open-End Agreement for Misc. Projects. Pkg. H Renovation of the Valley Street Student Center For Dental Hygiene [B.C. # ] Owner: Gadsden State Community College Architect: Aho Architects, LLC Date Issued: 04/16/2019 This Addendum is directed to all prime Contract Bidders, and all others to whom Drawings and Specifications have been issued by the Architect. This Addendum forms a part of the Contract Documents. The following conditions take precedence over any conflicting conditions in the Drawings and Specifications. The Drawings and Specifications are hereby amended in the following particulars: 1.1 GENERAL A. Regarding sales tax exemption certificate, the College will issue a Letter of Intent after the Construction Contract has been executed by the College, but prior to full execution by the DCM. This will allow the Contractor to apply for this exemption prior to the issuance of the Notice to Proceed. B. A Pre-Bid Conference was held per the Legal Advertisement on April 11, 2019 at 10:00 a.m. local time in the Valley Street Student Center. A copy of the minutes and sign-in sheet are attached to this Addendum. 1.2 REVISIONS (DRAWINGS) A. Revise drawings to include attached supplemental drawing ASD-1 dated 04/16/2019. Revision shows design for plumbing chase to be installed under the elevated portion of the building. 1.3 ATTACHMENTS ASD-1, Pre-Bid Meeting Minutes, Pre-Bid Sign-In Sheet END OF ADDENDUM #1 JOB NO PAGE 1 OF 1

Projects, Package H Renovation of the Valley Street Student Center for Dental Hygiene BC# ; Aho Architects Project Number: PRE-BID CONFERENCE MEETING MINUTES April 11, 2019 at 10:00 a.m. Pre-Bid 1) Sign-in on the sheet being circulated. 2) The Design Professionals involved on this project are: Tim Aho, Project Principal, April Cain, Project Manager, and Stephanie Vaughn, Business Manager. Your primary contact will be with the Project Manager. For contracts, pay requests, etc. contact the Business Manager. 3) The Client Representatives involved on this project are: Mr. Stewart Davis, and Mr. Cory Carter. Do not contact the Owner directly. 4) Upon completion of the Pre-Construction Conference, a site tour and overview of the project scope will be conducted by the Project Manager. 5) If additional site visits are required, coordinate with the Architect for availability, contact persons, etc. Please be respectful and minimize disruption of the Owner s ongoing operations. 6) Bid Documents are available for Pre-Qualified General Contractors, Subcontractors, and Suppliers through Alabama Graphics Digital Plan Room. Documents have also been posted to plan room services such as isqft, Reed, etc. See legal advertisement for more info. 7) Instructions to Bidders and Special Instructions to Bidders should be read by each bidder prior to submitting his/her bid. Among other requirements, please note sales tax avoidance procedures, requests for substitutions, verifying existing conditions, permit fees, etc. Bids are to be held open for 60 days. 8) Every General Contractor and every Subcontractor should read and be familiar with all of the front-end documents and all of Division 1 Specifications of the Project Manual in addition to the work they are bidding and other work they have to coordinate with. 9) All comparable products from the manufacturers listed in the Project Manual, requests for substitutions, additional manufacturers, pre-approved subcontractors and suppliers may be submitted no later than 2 working days prior to the Bid Date per the Special Instructions to

4 Bidders (Monday, April 15th). This is a change from the Project Manual which lists 5 working days prior to bid. 10) Insurance requirements should be read by each bidder and should be provided to each General Contractor s and Subcontractor s insurance carrier for review. Insurance requirements are indicated in the General Conditions. 11) Contractor shall be familiar with the requirements in Specification Section Supplementary Conditions pertaining to compliance with H.B Alabama Immigration Law (E-Verify MOU and Contract language). 12) Contractor shall note the requirements in Specification Section for Allowances. Allowance #1 is a $5,000 Contingency Allowance 13) Contractor shall be responsible for verifying existing conditions and field dimensions at the site prior to bid and start of work. 14) This project will include some Selective Demolition. Refer to Specification Section and the Drawings for more information. 15) Contractor shall be familiar with the requirements of Specification Section Closeout Procedures. 16) Requirements for quality control, testing, and inspections are located throughout the project manual, and should be reviewed so that they are understood prior to bidding the project. 17) All bid related RFI s shall be submitted to the Project Manager no later than 12:00 p.m. local time on Tuesday, April 16 th. 18) No Addenda have been issued to date. An Addendum will be issued with minutes and a copy of the sign-in sheet from this pre-bid conference. All Addenda will be issued via Alabama Graphics. Bid 19) Per the legal advertisements, the Bid Date is Thursday, April 18, 2019 at 2:00 pm (local time), in the same location as this pre-bid conference. (It is possible the bid date will change due to the project being resubmitted to the DCM for an additional review). Any further bid date changes will be changed only by an official addendum issued by the Architect through Alabama Graphics. 20) A copy of the Proposal Form C-3 is included in Project Manual. Loose and/or electronic (PDF) copies are available from the Architect. Contact the Business Manager for these. 21) The Owner is a tax-exempt entity. Sales and use tax avoidance procedures will be utilized on this project. Bids shall not include sales tax. Form C-3A Accounting of Sales Tax Attachment to ABC Form C-3 Proposal Form must be submitted with bid. See Division 1 Specification Section Sales and Use Tax Savings for more info.

5 22) The Contract Time for this project is 61 consecutive calendar days from the Notice to Proceed to Substantial Completion. See Proposal Form C-3. 23) Submit list of subcontractors, major suppliers, etc. along with the project s superintendent s resume within 24 hours (2 p.m. local time on 4/19/2018) after receipt of bids. Pre-Construction 24) A Pre-Construction Conference is required for this project. Other mandatory inspections involving the DCM will include an above-ceiling and final. The Permit Fee must be paid to the DCM by the General Contractor prior to scheduling the Pre-Construction Conference. Construction 25) A Project Sign is not required for this locally-funded project. No subcontractor signs will be allowed. 26) The Contractor s schedule must coordinate the overall construction schedule of the project. Watch for long lead-time items. Established Contract Time allows for this. 27) Contractor shall have access to available existing utilities (water, electric power, etc.) onsite for reasonable use paid by the Owner. Restroom facilities shall be provided by the Contractor. Project Completion 28) Liquidated damages as indicated in Article 49 of the General Conditions may be made against the Contractor for the entire period after expiration of the Contract Time that the Contractor fails to achieve Substantial Completion of the Work. 29) Final payment will not be made until the requirements of the Specifications, Drawings, and General Conditions are completed. 30) A final report of sales taxes avoided is required by ADOR and is a condition prior to final payment.
