1 I. Base Bidding Requirements This is not a base bid project. II. Work Included The following items are site specific requirements of the project. Contractor to repair, replace and install the necessary materials to complete the work identified below. Remove and replace approximately 625 ft. of existing 6-inch diameter vitrified clay sanitary sewer pipe with 6 SDR 35 PVC sewer pipe from manhole No. 5 (located 10 out from rest area building) to manhole No. 3. Remove existing clay pipe at three (3) manholes, core bore if required and install flexible boot style connectors for the new PVC sew pipe installation. Reestablish vegetative ground cover in all areas of lawn disturbance. Materials o Sewer Pipe Polyvinyl chloride (PVC) with integral-bell gasketed joints, conforming to ASTM D-3034, SDR 35. o Fittings Conforming to the same applicable ASTM Specification requirements as specified for the pipe. o Joints Push-on with elastomeric gasket, ASTM D 3212, and ASTM F477 for gasket specifications. o Pipe Bedding Material AASHTO No. 8 (Penn DOT No. 1B) coarse aggregate conforming to PennDOT Publication 408, Section o Pipe-Manhole Connectors Sewer pipe to manhole connections shall be flexible, resilient, watertight connectors meeting the requirements of ASTM C-923, Resilient Connectors Between Reinforced Concrete Manhole Structures and Pipes. Connectors may be any of the following types: Flexible, watertight, banded-boot and compression type connectors with stainless steel expander ring/sleeve, similar to NPC Kor-N-Seal as manufactured by Trelleborg Pipe Seals Milford, Inc.: PS: Direct Drive by Press Seal Gasket Corporation or a Department approved equal. Waterstop grouting rings similar to NPC-Waterstop Grouting Ring as manufactured by Trelleborg Pipe Seals Milford, Inc.; WS-30 Waterstop by Press Seal Gasket Corporation or a Department approved equal. 2 o Bonding Agent Two-component, 100% solids, moisture tolerant epoxy resin grouting/bonding agent similar to Sikadur Hi-Mod as manufactured by Sika Corporation; F 752 HydroEster by Fox Industries; Sonobond by Sonneborn Building Products or Department approved equal. o Non-Shrink Grout BASF Masterflow 713; Master Builders s Embeco 636; SiksGrout 212 by Sika Corporation or Department approved equal. Sanitary Sewer Construction o Take precaution to protect the public and property by furnishing and maintaining temporary fencing, barricades, lights, and other appurtenances as necessary. o Excavated material not reused for backfill shall become the property of the Contractor and be removed from the construction area and legally disposed. o Provide shoring as required in compliance with all applicable Occupational Safety & Health Administration (OSHA) regulations and safety provisions for excavation and trenching operations. Remove all shoring from excavations before backfilling. o The Contractor is solely responsible for the conditions and results of the excavation work. Disposition of Underground Utilities o Contractor is responsible to identify any overhead or underground utilities including all utility company owned or Department owned utilities. o Before beginning work, ascertain the existing underground utilities which may be encountered during trenching operations and verify their depth and location. Take precautions to protect and clear any such utilities within the work area. o Be advised of the following underground utility lines that are in close proximity to and parallel alignment with the existing sewer line. Primary electrical line to rest area building transformer Secondary electrical distribution line from rest area building to sewage treatment plant. Telephone service. o Comply with Pennsylvania Underground Utility Line Protection Law, PA Act 287 by telephoning Pennsylvania One Call System, Inc. at least three days prior to the start of excavation. o Adequately repair or replace any utilities damaged during the course of construction. 3 Trench Excavation o Excavate the pipe trench true in alignment with that of the existing sanitary sewer line so as to permit the new pipe to be laid in the approximate center of the trench. Ensure the trench is wide enough to provide a free working space on each side of the pipe. Trench width from the trench bottom to a point 12 inches above the top of the outside barrel to the pipe shall not exceed 24 inches. o Remove the existing vitrified clay pipe and excavate to a depth of 6 inches below the pipe to allow for the placement of bedding material. If the trench foundation is yielding, over excavate and stabilize the trench. o Keep the trench free from water during installation operations. Pipe Bedding o Provide the replacement sewer pipe with a crushed stone aggregate bedding foundation placed on the bottom of the trench to a minimum depth of 6 inches and along the sides of the pipe to a depth on one-half the pipe diameter. o Bedding material shall provide adequate and uniform support under the pipe. Ensure that sufficient granular bedding material is worked under the haunching of the pipe to provide adequate side support. Pipe Installation o Construct the new 6 inch diameter sewer along the same uniform line and grade as that of the existing sewer line. Lay pipe with a firm and even bearing along the barrel of the pipe. o Commence laying the pipe beginning at the low point of system and proceeding upgrade with smooth, unbroken continuity of invert. Lay pipe with bell end upgrade. o Maintain the existing manhole inlet and outlet invert elevations. o Do not lay the pipe in water or when trench conditions or weather conditions are unsuitable for such work. Manhole Connections o Ensure the watertight connection of replacement sewer pipe to existing manholes. o Pipe-to-manhole connector installation: Install banded-boot and compression type connectors by core drilling the manhole in strict accordance with the manufacturer s specifications. Recore the manhole using a larger diameter hole to accept the required connector size recommended by the manufacturer for use with the new pipe. Page 3 of 8 08/23/2012

4 Position the water-stop grouting ring around the outside diameter of the pipe so that the ring will be in the middle of the manhole s wall thickness. Secure the ring to the pipe with stainless steel take-up pipe clamps in accordance with the manufacturer s specifications. Prepare the opening by thoroughly cleaning and applying a high-quality bonding agent to the opening. Apply and compact non-shrink grout around the water-stop and between pipe and opening, taking care to fill all voids. o All pipes are to be flush with the inside face of the manhole wall. Do not allow the pipe to protrude into the manhole trough. o Reshape the bottom of the manhole as necessary to accommodate the invert of the new sewer pipe and maintain the manhole channel flow line as a smooth continuation of the pipe. Testing o Before backfilling, inspect and test the sewer line. After each manhole-tomanhole section of sanitary sewer is replaced, plug the pipe manhole and perform a low pressure air test. o Tests shall be performed in accordance with the current provision of ASTM F using the time-pressure drop method. All 6 inch pipe sections tested shall not exceed a0.5 psig air pressure drop (from 3.5 to 3.0 psig) for a minimum of 2 minutes and 50 seconds. o All pipe sections failing the test shall be repaired or replaced and retested. Backfill o Backfill the pipe only after is it inspected, tested and accepted. o Use the same granular pipe bedding material for the initial backfill from the bedding (centerline of the pipe) to a depth of 12 inches above the top of the pipe. o Place the initial granular material backfill in uniform 6-inch layers and, using mechanical methods, thoroughly tamp and compact to eliminate the possibility of settlement, pipe misalignment, or damage to joints. o Backfill from 12 inches above the top of the pipe to the surface with suitable excavated trench material. o Use mechanical tampers to compact trench backfill material and consolidate the backfill in such a manner as to ensure minimal possible settlement. 5 Topsoil, Seeding and Soil Supplements o Material Topsoil In accordance with Section of PennDOT Publication 408 Specifications. Seed Formula B in accordance with Section 804.2(b) of PennDOT Publication 408 Specifications. Soil Supplements Pulverized agricultural limestone and fertilizer in accordance with Section of PennDOT Publication 408 Specifications. o Seeding Operations Place and spread topsoil on the prepared areas to a uniform 4-inch depth. Rake topsoil and remove debris, foreign material and stones larger than 2 inches in any dimension. Final grade shall be reasonably free from rocks, roots and other debris. Do not place topsoil in a wet or frozen condition. Prepare areas for seeding by uniformly applying supplements. Blend the initial supplements into the soil as least 2 inches on top-soiled areas, by raking, disking, harrowing, or other acceptable methods. Apply soil supplements in accordance with Section 804.3(c) Publication 408 Specifications. Apply seed at the specified rates and sow uniformly on the prepared areas by broadcasting or hand seeding methods. Place mulch uniformly in a continuous blanket immediately after seeding is completed. END OF SECTION Page 5 of 8 08/23/2012

6 Submittals and final Deliverable items o The Department will provide the awarded contractor a complete breakdown of the required submittals based off of the System identified, I.E. Roofing System o Contractor to provide three (3) copies of the entire project documentation in three (3) ring Binders. o Binders to include the following items: Bid Documents & Specifications All project meeting minutes (pre-bid, pre-job, Interim, and Final inspection) Correspondence Submittals L&I approved Engineered Drawings (Paper/CD) L&I Building Permits, Inspection Logs, Occupancy Permit, Other-File Number is Certifications Certified Payrolls (Prevailing Wage rates) Waste receipts Photos Before, during and after o A sufficient amount of Photos is required for all phases of construction. Operation and Maintenance manuals Contractors Warranty Manufactures Warranty I. SUBMITTAL PROCEDURE A. Submit Two (2) copies of all items shown on submittal schedule except for "samples". B. Identify project number, project name and location, and contractor's name and address on all submittals. C. Provide space for review stamps of Contractor and Department. Put contractor stamp of approval on each item submitted. D. Revise and resubmit submittals as required, identify all changes made since previous submittal. Page 6 of 8 08/23/2012

7 E. No work shall be performed until all submittals are approved by the Department. Work performed prior to submittal approval is performed at the Contractor's risk and may be subject to repair, replacement or other corrective measures as deemed necessary by the Department with no additional contract time or money. F. Review of shop drawings will be general and for the limited purpose of checking for general conformance with the design; and shall not relieve the Contractor from the responsibility for proper fitting and construction of the work, nor from furnishing materials and work required by the Contract which may not be indicated on the shop drawings when reviewed. G. The submittals required by this contract are subject to reproduction for the Department's internal use only. By signature on this bid proposal, both the Contractor and the equipment manufacturer authorize the Department to reproduce any materials including printed literature and training aids, submitted in fulfillment of this contract. II. MANUFACTURER'S INSTRUCTIONS A. When specified in individual specification sections, submit manufacturer's printed instructions for delivery, storage, assembly, installation, start-up, adjusting and finishing, and maintenance, in quantities specified for product data. B. Identify conflicts between manufacturer's instructions and contract documents. III. MANUFACTURER'S CERTIFICATES A. When specified in individual specification sections or on submittal schedule, submit manufacturer's certificates to for review. B. Indicate that material or product conforms to or exceeds specified requirements. Submit supporting reference date, affidavits and certifications as appropriate. C.
