1 Invitation to Bid Duplin County Refrigeration Project for Villari Brothers Foods 1012 US Hwy 117 South Warsaw NC Duplin County is seeking bids from qualified contractors for providing materials and labor for the Villari Brothers Foods refrigeration project located at 1012 US Hwy 117 South, Warsaw NC The work will be performed in accordance with plans and specifications prepared by Villari Brothers Foods and as approved by Duplin County. Copies of the plans and specifications are available for inspection between the hours of 8:00 am 5:00 pm in the Duplin County Managers Office located at 224 Seminary St. Kenansville, NC Bidders are encouraged to visit the site and verify working conditions prior to submitting a bid. The work must be completed by September 28, The County reserves the right to reject any and all bids and to waive any and all technicalities or informalities. Time is of the Essence and Liquidated Damages will apply. Bids will be received at the Duplin County Administration Building located at 224 Seminary St. Kenansville until 12:00 p.m. August 27, 2015 at which time they will be opened and read aloud. The successful bidder will be required to show the following: Workers Compensation Insurance Coverage under this section shall be as required by federal and state Workers' Compensation and Occupational Disease Statutes, and shall have minimum limits as follows: Coverage A: Statutory, State of North Carolina Employers' Liability: Each Accident $1,000,000 Disease - Policy Limit $1,000,000 Disease - Each Employee $1,000,000 General Liability Insurance. The policy must be written on an Occurrence basis, with the following minimum Limits of Liability: General Aggregate per project $1,000, Products/Completed Operations Aggregate $1,000, Bodily Injury and Property Damage csl/each occurrence $1,000, Personal Injury and Advertising Injury $1,000, Certified ammonia welders. P&ID drawings. Additional ammonia charge if needed.

3 Bid Documents Duplin County Villari Brothers Foods Refrigeration Project The following documents constitute the entire agreement between the Parties and supersede all prior oral or written statements or agreements. Invitation to Bid General Specifications Bid Submittal Form Required Contract Provisions: Indemnification Clause, Warranty, Time of the Essence, Insurance Requirements, Jurisdiction of Duplin County, and Liquidated Damages. 4 Refrigeration Project General Specifications The project consists of supplying and installing refrigeration systems as noted. Any modifications or suggestions must be approved by Villari Brothers Foods and Duplin County. The project includes minimum specifications and requirements for the referenced work. Bidders must follow procedures in accordance with NCGS which includes: Minority and Female Employment Requirements, and Disadvantaged Business Enterprise Requirements. Successful bidder must supply proof of Liability and Worker Compensation Insurance Coverage. Bidders are required to provide all services by September 28, Bidders are responsible for conforming to all County and OSHA safety requirements while performing the work.

5 Bid Submittal Form Proposal of (hereinafter called Bidder ), organized and existing under the laws of the Sate of North Carolina and doing business as with the County of Duplin North Carolina (hereinafter called Owner ) In compliance with the Invitation to Bid, the bidder hereby proposes to perform and provide all labor and materials for the construction/installation associated with the Villari Brothers Foods Refrigeration Project in accordance with contract and design documents supplied by the Owner. By submission of the bid, each bidder certifies that this bid has arrived independently, without consultation, communication or agreement as to a matter relating to this bid with any other bidder or competitor. The bidder hereby agrees to complete work by September 28, Base Bid Villari Brothers Foods Refrigeration Project includes labor, materials and equipment to install refrigeration equipment as shown on the referenced project plans and specifications. 7 Refrigeration Project Warsaw, NC: All Bids must be delivered to Duplin County Economic Development in Kenansville, NC before 12 noon on August 27, The successful Bidder will be notified August 28, All work to be completed by September 28, Low Temperature System: Provide and Install (6) Vilter Ammonia 36 ton evaporators or equivalent evaporators: Model HPS /2-RA-HGP for three blast chillers. Provide and install condensate drain lines from the evaporators down to the floor. Provide and install Logix control upgrades and control wiring. Provide and install all necessary ammonia control valves and hand valves. Provide tech for (1) pump down to tie in the main headers to the existing system. Provide and install all ammonia pipe insulation and jacket. The jacket outside will be aluminum and all inside jacket will be white PVC. Provide a tech for system startup. Provide ammonia pipe markers and install. Provide and install pipe stands. Provide and install ammonia leak detection in the new areas. Provide and install heat trace for blast cooler evaporator drains. Provide and install blast cooler drain line insulation. Provide and PSM program assistance. Medium Temperature System: Provide and install one (1) Frick XJS12OL Compressor. Provided and Install two (2) Evapco NTW P2 evaporators or equivalent evaporators. Provided and Install two (2) Evapco NTW P2 evaporators or equivalent evaporators. Provide and install condensate drain lines from the evaporators down to the floor. Provide and install Logix control upgrades and control wiring. Provide and install all necessary ammonia control valves and hand valves.
