1 July 7, 2017 Forsyth County Procurement Donna Kukarola, CPPO, CPPB, Procurement Director ADDENDUM #2 BID For: Providing all labor, materials and equipment for the construction of the Brookwood Road widening project from McGinnis Ferry to S.R.141; project includes widening of Brookwood to a four lane median divided highway, construction of a 5 wide sidewalk along the east side and a 10 wide multi purpose trail on the west side, and Caney Road to be realigned away from S.R. 141 and signalized at Brookwood. This addendum supersedes and supplements all portions of the bidding documents and becomes part of the contract documents for the above-referenced project. Where any item called for in the specifications or indicated on the drawings is supplemented hereby, the original requirements shall remain in effect. Where any original item is amended, voided or superseded hereby, the provision of such item not so specifically amended, voided or superseded shall remain in effect. Response to questions received: (response in italics) 1. Half of the typical sections show a 12.5mm topping and the other half show a 9.5mm asphalt topping. There is only a pay item for 12.5mm listed on the bid form. Will the county please add a 9.5mm asphalt pay item or revise the typical sections? 12.5 mm will be used for the surface course on all typical sections, the bid item is correct. 2. After a field visit to this project it was noted that Forsyth County has a new fire station at roughly sta LT just past the graveyard. On the plans it doesn t look like this fire station has been accounted for. The plans show to install a multi-use trail and curb and gutter right across the fire stations driveway. Would the county please review this parcel #68 concern? Valley gutter quantity has been provided to tie the Fire Station back to the project. In the plans on index drawing it states summary of quantities will be from to In the plans provided there is only and Would the county please provide the other two summary of quantity plan sheets? Plans are complete, no other summary quantities will be provided. 4. Does the county have any soil boring information to provide? No. 5. This project shows a pay item for WATER QUALITY INSPECTIONS 18 EA. Typical GDOT pay items break out WATER QUALITY MONITORING & SAMPLING EACH AND WATER QUALITY INSPECTIONS MONTH. Will the county consider breaking out the water quality into the GDOT pay items? Contractor will perform the water quality inspections and County will perform the water quality monitoring per the special conditions, no change is needed. 6. On the bid form item # Mulch shows Tons in the description and CY for the unit cost. Would the county please clarify which unit of measure is correct? Unit Cost will be changed to CY to match item. Revised Bid Schedule attached and is to be used. 7. On the bid form item # Fiber Optic Splice, Fusion does not have a quantity listed. Would the county please provide a quantity on the bid form? It is 2 each. 8. On the bid form a lot of the traffic signal conduits, luminaires, pull boxes, and some strain poles are not listed as a pay item. Would the county please look over the signal plans and compare them to the pay items? The pedestrian lighting, installation and wiring is included in the mast arm and lump sum pricing line items. The lighting will have integrated photo cell and be powered from the signal cabinet main to each pole. Bid conduit as shown in plans, HDPE can be used in place of PVC The staging plans show temporary storm drain that will be required. On the bid form this item shows a quantity of 2 EA. Would the county please clarify which quantity is correct? Please use the bid quantity. 11. In the utility plans it shows pay item WATER METER, 3/4" with a quantity of 1 EA. On the bid form this item shows a quantity of 2 EA. Would the county please clarify which quantity is correct? Please use the bid quantity. 12. On the bid form it shows pay item WATER MAIN, 10 IN DIP CLASS 350" with a quantity of 320 FT. On the utility plans this item is not listed. Would the county please clarify if this is required? Line 118 for item Water Main, 10 DIP class 350, 320 Ft. will be removed. 13. On the bid form it shows pay item VALVE MARKER" with a quantity of 34 EA. On the utility plans this item is not listed. Would the county please clarify if this is required? Please use the bid quantity. 14. In the utility plans the following items are listed, but there is not a pay item established: Water Service Line, ¾ 265 LF Water Service Line 1 55 LF Water Service Line LF Price for meter installation, include service line connections. Remove Existing Water Valve, Incl Box 20 EA Items called out to be removed are to be included in Grade Complete Remove Existing Fire Hydrant 6 EA This is be included in the price for Grading Complete. 15. GDOT requires a post installation inspection of storm drain pipes with the use of laser profile technology. Will the City require this same inspection? 4 Page 4 Addendum #2 Bid Brookwood Road Widening 16. Bid Item #15 shows DOT # which is 4 median but you are spelling out 6 median, which is correct? Please bid Pay Items 22,23,24,25, and 26 all pertain to retaining walls of different types but do not appear to be the correct lengths of any of the wall listed on the plans. Could we get a clarification of which of the four walls in the plans are of which type and of which pay item they correspond to? The wall pay items are correct for each type of wall listed; the walls are not priced per foot/per wall but are priced per foot/per wall type.
