1 CONTRACT PROPOSAL/CONTRACT ACCEPTANCE PROJECT # 2016-NC-002 For Construction of Bloomfield EMS Station located in Bloomfield, Kentucky 02/12/2016

2 REQUEST FOR BIDS Bloomfield EMS Station Building The City of Bloomfield will accept sealed bids for furnishing all labor, equipment, and material to construct a new Emergency Management Service (EMS) Station to be located off Perry Street (Corners of Barneys Alley and Carr s Alley) in Bloomfield, Kentucky. The Project involves the supply of all materials, equipment, and labor for the construction of an 1,100 square foot commercial type building. Sealed Bids shall be submitted by and will be opened on Friday, March 11, 2016 at 9:00 A.M. at the Bloomfield City Hall, P.O. Box 206, 141 Depot Street, Bloomfield KY Contract bid forms, plans, and specification can be obtained at the Nelson County Government s website at nelsoncountyky.com. Hard copies of the contract plans and documents may be obtained from GBA Printing, 622 North 3'rd Street, Bardstown KY ( ) at a cost to the bidder of $ All questions or concerns regarding this project may be addressed to the City of Bloomfield, The City of Bloomfield reserves the right to reject any and/or all bids and waive formalities. 3 CONTRACT PROPOSAL/CONTRACT ACCEPTANCE PROJECT # 2016-NC-002 for Construction of EMS Station City of Bloomfield located in Bloomfield, Kentucky

4 This CONTRACT PROPOSAL/ACCEPTANCE by and between the City of Bloomfield (herein called the owner) and (hereinafter called the Contractor). The OWNER and the CONTRACTOR shall, in consideration of the mutual covenants hereinafter set forth, agree as follows: 1. WORK The CONTRACTOR shall complete all work as specified or indicated in these CONTRACT DOCUMENTS and SPECIFICATION. The work is generally described as follows: furnish all labor, equipment, and material to construct a new EMS Building including foundation work, framing, roof, siding, drywall, doors, windows, electrical and HVAC. 2. CONTRACT DOCUMENTS & PLANS/SPECIFICATIONS The EMS Station building shall be constructed accordance with this Contract Proposal and Plans (with specifications) entitled EMS Station, City of Bloomfield. The Contractor shall be responsible for all items in these Contract Proposal Documents. 3. CONTRACT TIME: The time period for completion of this contract shall be 150 calendar days from the Notice to Proceed. A Notice of Award shall be issued to the Contractor once Bids have 4

5 been reviewed and Approved by the City of Bloomfield. The Notice to Proceed shall be issued after the contractor has provided all performance bond and insurance documents. Reference Section 7, for Performance Bond requirements. A signed copy of this contract agreement will be mailed to the Contractor with the Contract Notice to Proceed. 4. CONTRACT INSPECTION: Inspection for this contract including submittal of material used, requests for payment, and acceptance of work completed will be performed by - City of Bloomfield, personnel and/or Nelson County Government Project Manager 5. PAYMENT PROCEDURES. Payment for work completed will be made once per month. Invoices for work completed shall be submitted to the Project Manager for approval by the Second (2nd) Wednesday of each month. Submit payment request to Attn: James F. Lemieux, PE, PLS c/o Nelson County Road Dept. 820 West Stephen Foster Bardstown, KY UTILITIES/STORAGE SPACE Storage space at the site for trailers is available. The contractor shall coordinate/obtain approval from the City of Bloomfield for storage. If offsite storage on adjoining property 5

6 is necessary, then the contractor shall obtain approval from adjoining property owners.. Equipment/materials may be stored at the site at the contractor's own discretion within the construction site area (unless written authorization obtained from adjoining properties owner). The contractor shall be totally responsible for all materials stored at the site. No utilities for use during construction are available at this site. All Temporary utility connections (electric) shall be the responsibility of the contractor to coordinate hook up. 7. BONDS: Approved security such as a performance bond shall be required of the contractor by the OWNER prior to issuance of the Notice To Proceed. Within two weeks of issuance of the Contract Award, the bond/surety shall be required. If the Bond is not provided as stated, then the CONTRACTOR'S bid may be considered invalid and the contract terminated at no cost to the owner. The Performance Bond shall be in a penal sum at least equal to the Amount of the Contract bid Price, as security for faithful performance of all the CONTRACTOR=s obligations. Alternatives to Performance Bonds may be submitted of which the OWNER may or may not accept as a surety of Performance. 6

INDEMNIFICATION/INSURANCE AND LICENSE REQUIREMENTS: The Contractor (including sub-contractors) shall purchase and maintain at the Contractor's expense for the full contract period such insurance as is appropriate for the work being performed. The Contractor shall purchase and maintain at the Contractor's expense for the full contract period such insurance as in the following minimum amounts: Workman's Compensation: Statutory General Liability General Aggregate $2,000,000 Bodily Injury Liability Personal Injury Liability $1,000,000 Each occurrence $1,000,000 Each occurrence Property Damage Liability $500,000 Each occurrence Comprehensive Automobile Liability Combined single Limit Bodily Injury Liability $1,000,000 Each occurrence $500,000 Each person Property Damage Liability $500,000 Each occurrence Stored Material Insurance -shall be the contractor s responsibility. The Contractor shall submit a Certificate of Liability Insurance document to the City of Bloomfield which shall list as a certificate holder (additional insured) the City of Bloomfield, 141 Depot Street, Bloomfield, KY

8 The insurance shall cover claims which may arise from the Work performed with this contract which includes Claims under Worker's Compensation, disability benefits, and claims for damages because of bodily injury, personal injury, sickness or disease, or death of any of the CONTRACTOR's Employees or for any person other than the CONTRACTOR's Employees who may be hurt/injured/killed as a result of the Contractor's Work on this contract. *** The Contractor shall provide proof of insurance within two weeks of Notification of Award The Contractor shall indemnify and hold harmless the OWNER and the ENGINEER against all claims, damages, losses and expenses, arising out of or resulting from the performance of any work on this contract. 8

9 9. Special Conditions: A. Project Contract Plans - Entitled EMS Station, City of Bloomfield have been designed/prepared by Simic Design, International, Inc., Ben Palmer Ball Jr. Architect. Site (grading) Plans prepared by Horizon Engineering. The Plans are listed as follows: G-1.0- Cover Sheet G-1.1- General Notes G-1.2- Construction Notes (includes MATERIAL SPECIFICATIONS) A-1.0- Floor Plan A-1.1- Foundation, Roof Framing Plan & Details A-1.2- Interior Elevations & Details A-2.0- Lighting & Roof Plans & Details (includes ELECTRICAL REQUIREMENTS A-3.0- Sections A-3.1- Sections A-4.0- Exterior Elevations P1.0- Plumbing Legend, Notes, and Schedules P1.1- Plumbing Details P2.0- Sanitary Piping Plan-Ground Floor & Riser Diagram P3.1- Domestic Water System Plan- Ground Floor Site Layout- (by Horizon Engineering) Sheet 1 of 2 Site Grading and Drainage Plan (by Horizon Engineering) Sheet 2 of 2 B. Bid Items: All items in the Project Contract plans shall be bid as the BID ITEMS listed as follows. Bid item # 1: EMS Station Building-Complete- Per Contract Plans- Includes Bldg Foundation grading work, the supply, installation, and construction of all materials listed and required (ref. Sheet G-1.2 for materials/specification) in the contract plans including HVAC And Electrical Work and front and back 3' ft wide Concrete porches. (Does not Include Bid Items # 2 thru # 6A) 9

10 The following Bid Items shall be Bid Separate and shall not be included in Bid Item # 1 Above. These Bid Items may or may not be utilized at the discretion of the City of Bloomfield. (The Contractor shall take this into account in bid preparation): Bid Item # 2: Site Finish Site Grading Plan - Includes seeding and straw Bid Item # 3- Concrete Drive & Conc HC Parking- (front of building) 6" DGA and 6" Concrete w/wwf (reference: Site Grading Plans and Sheet A-1.0- Key Note- # 2). Bid Item # 4 - Gravel Parking lot (rear of building ~ 41'x 42') 6" DGA- (reference Site Grading Plans-Horizon Engineering) Bid Item # 5- Flag Pole- (Ref Sheet A-1.0) Bid Item # 6 - Backup Generator 14 kw, Liquid Propane fuel, with Whole house 200A (SED) Automatic Transfer Switch (ATS), Corrosion Proof Enclosure, Automatic Operation (sensing utility s voltage Drop restoring power within 10 seconds and automatic exercising features) - Supply and Install Complete Kholer 14 kw,(w/200 SAL), Generac Guardian 16kW System(200 A Serv Disconnect), Or Equal Bid Item # 6-A- Automatic Transfer Switch (ATS) -Whole house 200A (SED) wired Complete From Electric Utility Service Provider Meter between and to Main Distribution Panel. 11 C. Electrical: Minimum Required Electrical Service shall be a 200 AMP, 120/240 Volt, 40 slot panel installed at location shown on the plans. All Work shall comply with the National Electric Code. All electrical fixture locations and materials shall comply with the plans (ref. A-2.0) The contractor shall coordinate Final electrical Hookups with Kentucky Utilities. This work shall be included in Bid Item # 1 D. HVAC : The Contractor shall be responsible to provide design and construction of the the HVAC system (unit and ducts) by a licensed HVAC Contractor. The minimum HVAC System shall be a 3 ton sized Heat Pump with LP Gas backup heat. This work shall be included in Bid item #1 E. Water & Sewer: The City of Bloomfield shall provide water and Sewer Service to within five feet of the Building. The Contractor shall be responsible for all sewer water construction in accordance with the plans. 11

12 F. Permits: The Contractor shall be required to obtain all Building permits Required for this project.
