1 Addendum I For Residential Reroofing at Multiple Public Housing Developments Town of Chapel Hill, NC BID: Q TO: FROM: SUBJECT: ALL PROSPECTIVE BIDDERS ZAKIA ALAM, PURCHASING & CONTRACTS MANAGER DATE: This Addendum modifies the bid submission format, additional conditions and addresses the following questions submitted since the release of the Request for Qualifications: 1. The Bid Proposal Form (page 7 of the bid package) is amended as shown below. Please disregard the form in the Bid Package, and submit this form and its completed attachment with your bid. 2. Please note permits must be obtained when required; also please note that all Town of Chapel permit fees will be waived as the work performed benefits affordable housing. 3. Rain gutter guards are not included in the line item budget/scope; the Town will not be addressing them at this time, but may request pricing at a future date.

2 Amended Bid Proposal Form BID PROPOSAL RESIDENTIAL REROOFING at MULTIPLE PUBLIC HOUSING SITES BID: Q SPECIAL NOTE: The name of the firm listed below MUST be the same company name that will be listed on insurance forms and the contract. In addition, the company listed on the bid form must be registered with the NC Secretary of State. Bidder: To: Purchasing & Contracts Manager Town of Chapel Hill 405 Martin Luther King Jr. Blvd. Chapel Hill, NC Bid Date: The undersigned, as Bidder, proposes and agrees if this proposal is accepted to contract with the Town of Chapel Hill for the furnishing of all materials, equipment, and labor necessary to complete the construction of the work described in these documents in full and complete accordance with plans, specifications, and contract documents, and to the full and entire satisfaction of the Town of Chapel Hill. Complete and include Attachment A, Residential Roofing Line Item Unit Price Budget, for removal of existing roofing and replacement and other work items as noted, with this. Include in your price replacement of any damaged or deteriorating materials, sheathing, fascia, or framing members. The contractor is responsible for providing all necessary protection from the weather during all phases of roofing operation. All materials, installation, and inspections shall be in accordance with the current edition of the North Carolina Building Codes. 3 Residential Roofing Line Item Unit Price Budget A. ROOF 1. Laminated Dimensional Shingle Roof- OAKWOOD Remove existing roof cover down to the deck. Ensure sheathing meets current North Carolina Building Code of at least 5/8 thickness. Replace any sheathing that is damaged and/or does not meet thickness standard; nail entire deck to current North Carolina Building Code standards using at minimum 8d nails installed 6 /6 spacing with no nail misses. Reinforce or replace damaged rafters with at least No.2 grade Douglas Fir or Southern Yellow Pine. Install one layer of No. 30 underlayment. Replace damaged overhang wood under soffits and fascia; replace any damaged soffit screen. Install shingle roof according to manufacturer s instructions and current North Carolina Building Code. Shingles shall be fungus resistant, be tested, and approved for ASTM D3161 or ASTM D7158 for wind resistance and UL 2218 for impact resistance, and carry a manufacturer warranty of at least 25 years. It is the contractor s responsibility to ensure that roofing installation and materials comply with the manufacturer s instructions and the current North Carolina Building Code. The contractor must request required inspections at appropriate intervals. Painting shall be done in accordance with Article P, Paint & Primer of this document. Indicate here how much wood replacement is included in the base price square 2. Fascia Board- OAKWOOD Replace all damaged or missing fascia board. Replace with pressure treated wood. Prime and paint as indicated in Article O, Paint & Primer. 3. Soffit- OAKWOOD Replace existing damaged/deteriorated/missing soffit and ventilation as needed to match existing. Prime and paint as indicated in Article O, Paint & Primer to match existing. Install according to the current edition of the North Carolina Building Code. 4. Gutter System- OAKWOOD Replace damaged/deteriorated/missing gutters and downspouts to match existing. Sizing of gutter system and locations of downspouts shall permit proper drainage and meet manufacturer s recommendations and current building code requirements. New gutter system shall be at least 5, 27 gauge seamless aluminum gutters, downspouts, leaders, drops, and accessories including a splash block at each downspout. Gutter system shall have a baked on enamel finish. Owner shall select a standard color.
