1 Documents Required for Registration Sr. No Application for registration in Form A under Schedule 2 of these Regulation along with a fee of Rs. 100, as in Schedule 3 Provide a self attested declaration of adherence to the basic hygiene & safety requirements with the application in the format provided in Annexure- 1 under Schedule Documents supporting proof of Total Turnover from business. 4. Source of water supply. 5. Source of Electric Supply used for business (If yes, state the exact horsepower or sanctioned Electric Load). 6. Proof of Possession of Premises (Sale Deed, Agreement, Electricity Bill). 7. NOC from Municipality 8. State Pollution Control Board clearance certificate 9. Water Analysis Report Documents to be enclosed for New Application for License to Central Licensing Authority Sr. No. 1. Form-B Duty completed and signed (In Duplicate) by the Proprietor/Partner or the Authorized Signatory 2. Blueprint/Layouts plan of the processing unit showing dimensions in meters/square meters and operation-wise area allocation. Kind of Business Re-labelers, Sale & 3. List of Directors with full address and contact detail. 4. Name and list of Equipments and Machinery along with the Number, Installed Capacity and Horse Power used. Re-labelers

2 5. Photo I.D. with Address Proof issued by government authority of authorized signatory with designation 6. List of food category desired to be licensed 7. Authority Letter with name and address of responsible person nominated by the applicant along with alternative responsible person indicating the powers vested with them viz assisting the officers in inspectors, collection of samples, packing and dispatch 8. Water analysis report a) Analysis Report (Chemical & Bacteriological) of water to be used as Ingredient in Food b) Pesticide residues report for Units Manufacturing Packaged Drinking Water, Packaged Mineral Water And/or Carbonated Water from a recognized/public health laboratory to confirm the potability indicating the name of authorized representative of LAB who collected the sample and date of sample & location of source of water Manufacturer 9. Proof of possession premises (Sale Deed / Rent agreement / Electricity Bill, etc.) 10. Partnership Deed/ Notarized Affidavit/Memorandum & Articles of association towards the constitution of the firm or copy of certificate obtained under Coop Act-1861/Multi State Coop Act in case of cooperatives. 11. NOC From Manufacturer Re-labelers 12. Food Safety Management System Plan or Certificate 13. Source of Milk Procurement including Location & Capacity of Milk cum chilling centre 14. Source of Raw Material for meat and meat processing units including veterinary inspection (anti mortem/post mortem inspection) Dairy Units in Manufacturers Meat Units in Manufacturers 15. Food Recall Plan Re-labelers And Importer 16. NOC's From Municipality / Local Body 17. State Pollution Control Board clearance except in case of notified industrial area. Manufacturer 18.
