Placing Orders All orders for stock lenses must be placed using ebuy at eyefinity.com. Please select Plexus Optix, Inc. Fulfillment Center from the drop-down menu. VSP In-Office Finishing Program Plexus Optix TM Single Vision Stock Lenses for VSP Claims This catalog can be used to purchase stock lenses for all of your patients. As a VSP provider, you can earn additional revenue by finishing lenses in-office for your VSP Signature Plan, VSP Choice Plan, and VSP Advantage Plan SM patients. Included in this catalog are two columns specific to VSP patients, a finishing service fee and additional revenue earned by the VSP Network providers. To download the Plexus Optix for VSP Claims, choose the ebuy tab from the Eyefinity menu at eyefinity.com, then click on the Plexus Optix logo. earned calculations are based on the VSP Signature Plan VSP In-Office Finishing Program Schedule effective June 1, Prices are subject to change without notice. Returns, Credit, Cancellations, and Warranty Terms Returns Excess inventory of stock lenses purchased from the Plexus Fulfillment Center may be returned for credit, and are subject to the 15 percent restocking fee. Lenses returned for credit must be received in resalable condition within 30 days of the purchase date, with a copy of the original invoice and a stock lens return form. Returns should be sent to: Plexus Fulfillment Center Attention: Returns Department Trade Center Drive, Suite 130 Rancho Cordova, CA Cancellations Orders can be canceled upon request. However, if the request is made after fulfillment, but prior to shipping, we will process it as a lens return. Scratch-resistant Warranty The Scratch-resistant Warranty applies to scratches resulting from normal use. Lenses that are abused, extremely scratched, chipped, broken, or damaged during processing aren t covered under this warranty. All scratch-resistant coated lenses are covered under a Scratch-resistant Warranty one time, for one year from date of purchase. Anti-reflective Warranty The Anti-reflective (AR) Warranty applies to scratching, peeling, or crazing under normal use. Lenses that are abused, extremely scratched, chipped, broken, or damaged during processing aren t covered under this warranty. Standard (Category A type) AR coatings are warranted one time, for one year from date of purchase. All other (premium) AR coatings are warranted for two years from date of purchase. Manufacturer s Defect Defective stock lenses may be exchanged on a one-for-one basis or returned for a full refund. Shipping Orders received by 3:00 p.m. Monday Friday, excluding holidays, are processed and shipped within two business days.
