For a long time, stroke was simply divided into the two main categories: the hemorrhagic and the more often ischemic stroke. With the help of and developments in modern brain imaging techniques ischemic stroke can be described very precisely in terms of stroke causes and individual disease patterns. This deep look 4

5 inside the individual pathology even in the very acute stroke period allows the development and use of individual treatment concepts. Intravenous thrombolytic therapy with rt-pa is officially the only proven therapy in patients with acute ischemic stroke today. However, in comparison to placebo there is only a 7% greater chance for a better clinical outcome with iv-rt-pa alone. Endovascular catheter based recanalization therapy for acute ischemic stroke treatment with different recanalization techniques (intra-arterial thrombolysis, neurothrombectomy) offers the possibility of greater recanalization rates than iv-thrombolysis. Within the very last year s new promising endovascular techniques (stent-retriever) became available and there is an overwhelmingly promising debate about the importance of this new exciting therapy. However, even with the availability of these new options unsuccessful treatments occur daily and raise the question for further improvements. Nanotechnology may offer the opportunity for new therapeutical approaches or even improvements of existing therapies.
