1 1- Project Team Design Solution: A) Provide a DESIGN SOLUTION that addresses the vision OVERALL CONCEPT PLANNING AND BASE PROGRAM SCOPE OF WORK for the Project. Specifically describe how the Proposal addresses the Prerequisites and Priorities of the Design Criteria Package and an explanation of the intent of any modifications, revisions, deviation or innovations noted. Indicate for each item the purpose, anticipated benefits, and anticipated costs or savings in DESIGN AND CONSTRUCTION. Provide Outline Specifications in a narrative style, organized according to the Specification Section. 3 2- Sustainable Strategies: A) Provide a written description of all sustainable design approaches for the project. In areas of the project that will not be LEED Certified, indicate where and how the County s Go- Green Certification and Seal of Sustainability will be applied. For more information see Exhibit D - Sustainable Strategies, page Project Management Plan (PMP): A) Provide a Project Management Plan (PMP) that describes how the Design-Build team will meet the Project objectives including but not limited to, maintaining ongoing operations on the Project site(s) within the Park, phasing and staging of the work. The Project Management Plan shall specifically address how the Design-Build Team will meet the County s requirement that construction be substantially complete, facilities operational and able to support Park s activities by December Include a proposed Project Schedule (Gantt chart) indicating the duration and milestones for Project activities. Identify Design-Build Team Members personnel and describe their roles and responsibilities along with the other principal participants and named sub consultants having a direct role in the Project as it relates to design and construction activities. Organizational charts shall include the following: A) A Project Management Plan (PMP) that describes how the Design-Build team will meet the Project objectives was provided. For more information regarding the PMP see Exhibit G - Project Management Plan (4 pages). For more information regarding the Project Schedule see Exhibit F - Project Schedule (6 pages). Provide construction organization indicating the responsibilities and structures of the construction staff. For more information see Exhibit G - Project Management Plan, page Workload of the Firm: For the prime Vendor only, list all completed and active projects that Vendor has managed within the past five years. Please make it clear which Projects are Active and which ones are completed. In addition, list all projected projects that Vendor will be working on in the near future. Projected projects will be defined as a project(s) that Vendor is awarded a contract but the Notice to Proceed has not been issued. Identify any projects that Vendor worked on concurrently. Describe Vendor s approach in managing these projects. Were there or will there be any challenges for any of the listed projects? If so, describe how Vendor dealt or will deal with the projects challenges. The contractor has completed seven (7) projects with the total construction value of $15.2M in the last five years. Currently the contractor is working on two (2) projects with combined construction value of $17.8M. One of the current projects valued at $14.3M is projected to be complete by March Page 5 of 6

6 5- Location: Refer to Vendor s Business Location Attestation Form and submit as instructed. A Vendor with a principal place of business location (also known as the nerve center) within Broward County for the last six months, prior to the solicitation submittal, will receive five points; a Vendor not meeting all of the local business requirements will receive zero points. The following applies for a Vendor responding as a Joint Venture (JV): if a member of the JV has 51% or more of the equity and meets all of the local business requirements, the JV will receive three points; if a member of the JV has 30 to 50% of the equity and meets all of the local business requirements, the JV will receive two points; and if a member of the JV has 10% to 29% of the equity and meets all of the local business requirements, the JV will receive one point. 5- See Purchasing Director's Memorandum. 6- Project Price: Total Project Price for your Design Solution, Design Documentation and Construction Cost 6- See Purchasing Director's Memorandum.
