Please note that the mandatory pre-bid meeting will be held on either Thursday, January 30, 2014 or Friday, January Due to patient care needs, the date/time of the pre-bid meeting has yet to be determined. A follow up will be sent to confirm the date/time early next week. The bid due date will be either 2:00 p.m. Thursday, February 13, 2014 or Friday, February 14, 2014 depending on the date of the pre-bid meeting. Confirmation of the bid due date will also be provided early next week.

2 SECTION INVITATION TO BID PROJECT Provide all labor, supervision, equipment, and materials necessary to complete interior renovations related to the removal of existing Linear Accelerator equipment, and replacement with a new Elekta Versa HD. Work shall occur in Room B-192 of The Harry and Jeanette Weinberg Building located at 401 N. Broadway in Baltimore, Maryland. Renovations include work in a 520 sq. ft. treatment room, and 157 sq. ft. control room. (Note: room dimensions are approximate.) General Construction Work includes management of all trades, and life safety implementation and environmental protection. The scope of work includes, but is not limited to the following: selective demolition, concrete, miscellaneous steel, carpentry, architectural millwork, interior finishes, and mechanical, electrical, plumbing (including low voltage). The existing concrete floor will require surveying to confirm exact locations of fire sprinkler and/or medical gas lines. Existing radiation protection around ductwork shall be salvaged and reinstalled. Documentation of how radiation protection was previously installed must also be maintained. The successful bidder will be required to utilize Constructware, a web-based project management tool, for the duration of this Project. **Effective July 1, 2013: All projects require appropriate Personal Protective Equipment ( PPE ). Please refer to Attachment A for the JHHS Facilities Design & Construction PPE Policy. BIDS DUE Bids will be received until 2:00 p.m. local legal time on either Thursday, February 13, 2014 or Friday, February 14, 2014 and will be privately opened. Bidders are permitted to submit electronic bids to Rebekah Tate at rtate3@jhmi.edu provided they are received by 2:00 p.m. on 2:00 p.m. Thursday, February 13, 2014 or Friday, February 14, Only bids from Invited Bidders will receive consideration. 3 INQUIRIES Inquiries concerning this Bid shall be directed to the Facilities Design and Construction Office, Attention: Rebekah Tate via at QUALIFICATIONS Only previously qualified contractors in the trades provided for in the JHHS Facilities Bidders List, shall be permitted to take part in the bidding of this Project. A listing of previously qualified contractors will be issued with the Bid Documents. For trades not listed in the JHHS Bidders List, Bidder may utilize any firm they deem fit, subject to Owner review and approval. Bidder may propose the usage of MBE firms for listed trades, even though not currently listed on the JHHS Bidders List, to meet their Base Project MBE Requirement (defined below). For any proposed MBE firm, not currently listed on the Owner s prequalified Bidders List (and which is performing construction services within the listed trades), Bidder shall submit a standard AIA A305 document with their Bid. The AIA A305 document is available from Owner at Bidders request. PRE-BID MEETING A pre-bid meeting and site visit will be held on either Thursday, January 30, 2014 or Friday, January Due to patient care needs, the date/time of the pre-bid meeting has yet to be determined. Confirmation of the exact Pre-Bid Meeting date/time will be provided early next week. This meeting will be mandatory for those firms invited to bid on the Project. The pre-bid meeting shall be held at the following location: Pre-Bid Meeting Location: TBD ACCEPTANCE OF INVITATION TO BID Invited Bidders that accept this Invitation to Bid, shall provide written notification, transmitted by (rtate3@jhmi.edu), to the Facilities Design and Construction Office, Attention: Rebekah Tate, on or before 2:00 p.m. on Tuesday, January 28, Bid Documents will be distributed only to those Invited Bidders that provide this notification. PUBLIC BID SITES/CONSTRUCTION PERIODICALS Be advised that all Bid Documents related to this, and all other Projects managed by the Johns Hopkins Health System Facilities Design & Construction department ( JHHS FD&C ) are the sole property of JHHS FD&C. JHHS FD&C does not authorize the publication of Bid Documents, Project Information, Bidder s Lists, Contract Procedures or any other projectspecific information in any construction periodical or on any website (e.g. Bid Clerk). This includes internal company websites that are not protected by a User-ID and password. Any Bidder that deviates from this policy will be disqualified from bidding this and any other Project. 4 MINORITY, WOMEN AND DISADVANTAGED BUSINESS ENTERPRISE PARTICIPATION It is the policy of the Owner to encourage the highest percentage of Minority, Women and Disadvantaged-owned Business Enterprise ( MBE/WBE/DBE ) participation possible. To this end, Bidder shall achieve Fifteen percent (15%) participation of MBE/DBE/WBE in the construction of this project. 5 ATTACHMENT A PPE Policy

6 Construction Site PPE Policy July 1, 2013 Johns Hopkins Health System This policy reflects the minimum requirements. Special conditions will require the appropriate PPE. Mandatory 1. Class A Hard Hat Hard hat must be without cracks, chips or looking dried out. 2. Safety Glasses Safety glasses must be clear enough to see through them without excessive scratches and grime. 3. Safety Vest with company affiliation printed on the back with letters a minimum of 3/8. 4. Leather footware No soft soles, high heels or open toed shoes. Shoes or boots must be sturdy enough to withstand rough and/or uneven surfaces. 5. Long pants No shorts, capris, dresses or skirts. 6. No loose clothing and accessories No scarves and wallet chains. Non breakaway lanyards and ties must be tucked in. 7. No radios, ear buds, ear phones. Conditional 1. Ear plugs or headphones When exposed to loud noise. 2. Dust masks When exposed to dusty or high chemical concentrated air. 3. Gloves suited to the application Protection from cuts, bruises chemical irritation, extreme hot or cold, electrical shock. 4. Goggles Eye protection from flying particles (sparks or dust), chemical or biological splash, getting poked. 5. Harness Fall protection risk
