1 APPENDIX C SAMPLE ASBESTOS ABATEMENT SPECIFICATION The following appendix includes information and standard drawings that are diagrammatic and are used only to convey intent.

2 Guidelines for Use of Sample Asbestos When to use this specification: - University Project Coordinators, Facilities Maintenance staff, and/or Environment, Health, & Safety staff shall use this sample specification for all asbestos abatement projects that are solicited independent of other contracts. - A/E s and/or contractors should reference this document when preparing their specifications for subcontractors. This sample is intended as a template. Sections with italicized red letters shall be replaced with project specific information. Page 2 of 13

3 PROJECT TITLE ASBESTOS ABATEMENT SPECIFICATION PART 1 GENERAL 1.1 ABATEMENT CONTRACTOR'S RESPONSIBILITY The Abatement Contractor shall perform removal of all Asbestos Containing Material (ACM) identified in SPECIAL PROVISIONS 2.1 SCOPE OF WORK. The ACM removal work shall be conducted by a licensed and qualified asbestos abatement contractor. 1.2 ABATEMENT PLAN AND SCHEDULE The Asbestos Abatement Contractor shall plan and schedule asbestos abatement so that the LEAST NUMBER of regulated areas are created as is practicable for project flow. This requirement shall be confirmed by submission of an Abatement Plan prepared by the Abatement Contractor. See Abatement Contractor Submittal requirements in PART 2, SPECIAL PROVISIONS. 1.3 ACCESS DURING ABATEMENT The Asbestos Abatement Contractor shall have access to all abatement areas at the job site. 1.4 UTILITIES FOR ABATEMENT The building's utilities, including power, water, and sanitation may be used during abatement; however, no guarantee as to the reliability of those utilities is made. The Asbestos Abatement Contractor shall be responsible for establishing and maintaining utilities necessary for completion of abatement. PART 2 SPECIAL PROVISIONS 2.1 SCOPE OF WORK The Asbestos Abatement Contractor (Contractor) shall furnish all labor, materials, services, permits, equipment, utility and transportation services required and necessary for the removal, transportation and disposal of ACM and asbestos contaminated materials as specified herein. The asbestos abatement removal items are listed below: Insert abatement items here. Page 3 of 13

4 Abatement shall be completed in conformance with the time schedule requested by the University Project Coordinator. The Contractor shall be responsible for field verifying all conditions and quantities that may affect his work prior to bidding this project. Key items to consider in bidding this work follow: Insert job specific conditions here. Examples include limitations to work hours, occupancy of building or adjacent areas, and unique site conditions. 2.2 BASIS OF BID This is a lump sum project, with the Owner providing opportunities for bidders to access the site during the bid period. The successful bidder shall provide unit prices for all individual abatement items after being notified that they are low bidder. These unit prices shall be used for progress payment purposes and for establishing increases or decreases in the Lump Sum Bid Price due to discovery of changed conditions and/or quantities when the project is proceeding. 2.3 CONTRACTOR QUALIFICATIONS The Owner may accept those Contractors as being qualified if they are licensed as Asbestos Contractors by the State of Nevada, State Board of Contractors, for the appropriate class of work, and who have the appropriate insurance. 2.4 PRE-ABATEMENT CONFERENCE AND JOB BOARD The Contractor s supervisory personnel shall attend a pre-abatement meeting with the Owners Representative at the time and location designated by the Owners Representative. At this meeting the Contractor shall provide three copies of bound submittals, including but not limited to the following: - Schedule: A schedule indicating the approximate dates of all key functions for each abatement area (preparatory work, abatement, clearance, etc.), including shift times. - Abatement Plan and Procedures: The Contractor shall provide an abatement plan with methods and procedures covering each of the identifiable abatement areas. All procedures used shall provide containment of asbestos debris and not allow airborne contamination of adjacent areas. The Abatement Plan shall be approved by the Project Coordinator and the University Environmental, Health, and Safety Department (EH&S) before abatement work may commence. Abatement Plans shall, at a minimum, contain: scope of asbestos related work, containment methods, safe work practices, protection of workers, products to be used, air sampling (personal and area), waste disposal, certification of supervisors/workers, and description of clearance of the abatement area, to include the independent party that will perform clearance inspection and air sampling if removing friable ACM materials greater than ten square feet. The Abatement Plan shall be submitted to the Project Coordinator at least one week prior to commencement of work. If the Pre-abatement conference is less than one week before scheduled work start, the plan must be submitted in advance of the conference. Page 4 of 13

5 - Waste Disposal: The Contractor shall provide the procedure to be followed for handling asbestos waste on removal, packaging, transport and disposal at an approved public asbestos waste disposal site meeting the requirements of CFR 40, Part 61, Subparts A and M. - Emergency Procedures: Emergency procedures, which will be followed at the site in the event of: -- Fire. -- Life-threatening bodily injury inside the containment area. -- A major breach in the containment barrier. -- The detection of unexpected airborne contamination in other parts of the building. -- Splitting or spilling of asbestos debris bags en-route to the dumpster or waste truck. - Safety Data Sheets and Product Data: Contractors shall submit Safety Data Sheets (SDS) and manufacturer s literature for all products they intend to use on the project. - Insurance: The Contractor shall submit insurance certificates verifying insurance coverage as required in Section 2.4, for the Owner's review and approval. - Other Postings: Emergency numbers (24 hours), safety procedures, air monitoring results, and the name and license number of lab performing testing. Dioctylphthalate (DOP) test certifications for Differential Pressure Units (DPUs) and High Efficiency Particulate Air (HEPA) vacuums, shall be conspicuously placed within the near vicinity of containment areas, as directed by the Owner or Owner's Representative. - Project Office Layout: The Contractor shall provide a layout sketch of the project office area which must be located near the Clean Room of the Decontamination Unit if at all possible. The layout shall indicate location of the sign-in log, the job board with posted emergency procedures and the location of a workstation for the Owner. The following documents must be maintained at the site of a project for the abatement of asbestos for the duration of the project and retained by the contractor -- The contract or specifications for the project. -- The plan of the project and the estimated amount of asbestos involved in the project. -- The scheduled and actual dates for commencing and completing the project. If the actual date for completing the project differs from the date originally scheduled, a statement of the reasons for the difference must be included. -- Documentation of compliance with all regulatory requirements related to asbestos. -- Copies of all correspondence with regulatory agencies concerning the project, such as permits for building or demolition or notices of violations. -- The name and address of the authorized disposal facility to which the materials containing asbestos were taken, evidence of the amount of asbestos received for disposal and confirmation that the disposal was made pursuant to regulations adopted by the State Environmental Commission. -- The method by which samples of the air were tested during the abatement process, test results, name and license number of the consultant hired to perform the sampling, and name of the laboratory employed to analyze the testing. 6 -- A complete list of the names of abatement workers, supervisors and other employees or agents participating in the project. The Contractor shall provide a report on his staff organization, including licensure, individual experience, training and the chain of command to be utilized on the worksite. -- A description of unplanned releases of asbestos and accidents which occur at the work site. -- A log containing the names of persons who entered and exited the containment area and the times of their exit and entry. - The Contractor shall provide a description of respirators, protective clothing, and Personal protective Equipment (PPE) to be provided by the Contractor to his employees and all visitors to the work site. The description shall include brand names and specifications. - The Contractor shall also provide the step-by-step procedure he will employ and enforce for all individuals to enter and exit the wash area through the decontamination facility. Decontamination procedures for tools and equipment shall also be provided. 2.5 ASBESTOS CONTRACTOR INSURANCE Comprehensive General Liability Insurance: Contractor shall purchase and maintain during the course of this agreement Comprehensive General Liability Insurance which is occurrence basic including coverage for: Premises - operations Products - completed operations Blanket Contractual Broad form property damage Independent Contractors Automobile liability Personal injury Asbestos risks Limits for said insurance policies shall be for amounts of not less than those set forth below. GENERAL $2,000,000 PRODUCTS-COMP/OP $1,000,000 PERSONAL & ADV INJURY $1,000,000 EACH OCCURRENCE $1,000,000 All insurance required under this specification shall be equal to or exceed an A, AA, or AAA rating as listed in Best's Insurance Guide's latest edition. 7 In the event that asbestos risks are excluded from the Comprehensive General Liability, the Contractor must provide an additional policy insuring asbestos abatement liability that does not exclude asbestos abatement, asbestos disease or transportation to an Environmental Protection Agency (EPA) approved landfill, with limits of not less than One Million Dollars ($1,000,000.00) per occurrence. Insurance shall be occurrence basic. University of Nevada, Reno and their allied entities shall be named as additionally insured under this policy. With respect to the liability of any such additional insured arising out of this work for that insured by the Asbestos Abatement Contractor or for the Asbestos Abatement Contractor, this insurance is primary. 2.6 UTILITIES It is the responsibility of the Contractor to negotiate with the Owner for the use of water and to provide transportation of the water and the utilities for construction purposes, including but not limited to, electrical power, gas, telephone and sanitary facilities. The Contractor shall pay all costs involved in securing such utilities. 2.7 POST-ABATEMENT SUBMITTAL Upon completion of the work, and prior to the project being considered complete, the Contractor will prepare a report and submit it to the Owner. The report will contain: - A copy of all notifications to or permits received from Federal, State and local agencies for this project. - A copy of the signed waste manifests indicating the place, time and exact quantity of asbestos, received by an approved landfill. - A description of all problems encountered during the work. - A copy of the log maintained at the job site throughout the work containing the names of persons who entered and exited the containment area and the times of their exit and entry. A list of location and quantity of asbestos containing material not removed from the work area. - A copy of the log for any air monitoring performed by the Contractor, and the lab s analysis, including the location and credentials of the laboratory performing the analysis. - The name, title, and signature of the person who prepared the report. The Contractor will certify in writing that the work has been completed in accordance with the job scope given by the UNR Project Coordinator. The Post-Abatement Submittal will provide verification documentation that the project has been completed in accordance with the Contract Documents, and the Submittal shall be provided to, and approved by, the Owner as a project requirement prior to processing any final or phase completion pay request. 8 PART 3 TECHNICAL SPECIFICATIONS 3.1 PERSONAL PROTECTIVE EQUIPMENT (PPE) Contractor shall provide all workers with PPE in strict conformance with the OSHA Asbestos Construction Regulation (29 CFR ) requirements. Authorized visitors or inspectors must have fit tested PPE before being granted access to the abatement work area. 3.2 DECONTAMINATION SYSTEM AND WASTE LOAD-OUT Prior to beginning any pre-cleaning activities, Contractor shall construct a worker decontamination enclosure system connected to the existing work area. OSHA Class I abatement area decontamination enclosures will include the following features: An equipment or dirty room with two doorways, one to the work area and one to the decontamination room. A decontamination room with two doorways, one to the equipment room and one to the airlock/clean room. The decontamination room shall contain at least one shower with hot and cold water. Careful attention shall be paid to the shower enclosure to ensure against any leaking and the shower s water source should be disconnected and turned off at the end of each shift. Soap and shampoo will be available in the shower room at all times. Clean and dry towels will be available for employees and any Owner authorized visitors and personnel. A three-stage water filtration system will be employed prior to release of shower water into the local sewage system. An airlock/clean room with one doorway into the shower room and one to the outside area. OSHA Class II abatement area decontamination enclosures will include the following features: An equipment or dirty room with two doorways, one to the work area and one to the clean room. A decontamination room with one doorway into the dirty room and one to the outside area. Wash water, clean and dry towels will be available in this room for employees and any Owner authorized visitors and personnel. A three-stage water filtration system will be employed prior to release of water into the local sewage system. The contractor shall ensure employees enter and exit the regulated area through the decontamination area. Waste load-out shall take place through a two-stage waste load-out enclosure system constructed prior to waste load-out at the end of the abatement project or at the end of daily work. No gross abatement shall be performed while load-out is taking place. If the Contractor requests to use the worker decontamination facility for waste load-out, approval will be based on a no impact judgment on the protection to facilities or the final project provided to the Owner. 9 Bags of ACM will be wet wiped, removing all visible material from the exterior of the bags outside of the dirty room. Each bag will then be passed into the dirty room where it will be bagged again with a bag from the decontamination room. Load-out of the bag can then take place, after the bag is inspected to insure no contamination has taken place. Evacuation of excess air by HEPA vacuum is recommended. The waste transfer unit shall be fully lined (except ceiling) with one layer of 6-mil poly. The Contractor shall test the unit leak-tight and lockable. 3.3 ASBESTOS REMOVAL WORK SHALL NOT COMMENCE UNTIL: Arrangements have been made for disposal of waste at an EPA approved dumpsite. Arrangements have been made for containing and/or disposal of wastewater resulting from showering and other abatement activities. Containment, decontamination enclosure system, and waste load-out area are appropriately constructed and approved by the University EH&S Department. Tools, equipment and material waste receptacles are on-site. Suitable barricades and warning signs are established to restrict access to the work area to abatement associated personnel only. All other preparatory steps have been taken and applicable notices posted, and permits are obtained. A visitor and employee log-in log-out system is in place at the job site. All persons entering the site will be required to sign in and sign out. Sufficient differential pressure is established inside containment per Technical Specifications. A minimum of 0.02 column inches of water pressure differential, relative to outside pressure, shall be maintained within the Negative Pressure Enclosure (NPE) as evidenced by manometric measurements for all class I containments. A formal pre-abatement safety and procedure meeting has been held at the site with the University EH&S Department and Contractor's employees present. At this time the Contractor's final abatement plan must be submitted for each abatement area. Written notice to proceed has been given by Owner acknowledging that items above have been accomplished 3.4 MAINTENANCE OF ENCLOSURE SYSTEMS Ensure that barriers and plastic linings are effectively sealed and taped. Repair damaged barriers and remedy defects immediately upon discovery. Visually inspect enclosures and differential pressure units at the beginning of each work period or shift. Details of the inspections are to be included in the Contractor s daily log. Page 9 of 13

10 Use smoke methods to test effectiveness of barriers and the differential pressure containment at each shift, per OSHA regulatory requirements. 3.5 PROHIBITED PRACTICES No high speed abrasive saws without HEPA filters. No compressed air. No dry sweeping, shoveling or other dry clean-up. 3.6 CONTAINMENT CLEARANCE Removal and detail cleaning work will be considered complete when all surfaces inside containment areas have no visible debris or asbestos residue and have passed final clearance visual inspection by the Owner's Representative. The inner poly wall and floor shall be removed after the visual inspection is approved. Any contamination that penetrated the inner poly layer shall be thoroughly decontaminated and cleaned. If the outer poly layer was punctured or material leaked through, then the outer poly shall be removed in the contaminated areas and all contaminated materials cleaned or removed as contaminated materials. Aggressive air samples will be taken for clearance after a visual inspection has been passed. The Contractor will be allowed two visual inspections, and if work is still required after two inspections the Contractor shall pay for any costs associated with the additional inspections. All removals of friable ACM greater than ten square feet shall have aggressive air clearance by a third party consultant per the Nevada Administrative Code. Phase Contract Microscopy (PCM) air clearance testing will be performed by the Owner's Representative collecting a containment size appropriate number of samples inside the work area each with an air volume of at least 1200 liters taken at 10 liters per minute or less. The samples will be analyzed by PCM National Institute of Occupational Safety and Health (NIOSH) Method Any alternate sampling method shall be approved in advance by University EH&S. PCM clearance will be achieved if the average of all five inside sample results are equal to or below 0.01 fibers/cc. If clearance samples fail, the Contractor is responsible for all subsequent consulting and testing costs until the clearance is achieved. The Contractor shall remedy the situation that caused the failure, i.e., filter outside dirty air, re-clean inside surfaces or other, and then the Owner's Representative shall retest the containment area. Once the Visual Inspection and PCM Air Clearance is achieved encapsulation can occur. After encapsulation dries and passes inspection the Contractor shall remove all barriers and equipment. 3.7 DISPOSAL As the work progresses, and to prevent exceeding available dumpster storage capacity on site, remove asbestos waste and dispose of at an EPA approved disposal site in accordance with the requirements of the disposal authority. Submit documentation regarding disposal to the Owner. All ACM and asbestos contaminated material shall be double bagged (or wrapped) in 6-mil bags. ACM shall be wet/damp inside of collection bags. Bags shall not be dropped or handled roughly. If chutes are used, chute angle shall be sufficient to prevent breakage. All bags shall Page 10 of 13

11 be labeled per OSHA and have proper generator labels and information per EPA National Emissions Standards for Hazardous Air Pollutants (NESHAP). All ACM or asbestos contaminated material shall be damp when delivered to the disposal site. All ACM containers which also contain metal or other sharp objects shall be protected from punctures by placing in fiber drums or approved alternative. Sealed plastic bags may be dumped into the burial site unless the bags have been broken or damaged. Damaged bags shall be sealed by placing them in another properly labeled 6-mil bag. Each load must be accompanied by a Uniform Waste Manifest or equivalent and any other certificate required by state or local agencies. Copies of the Waste Manifest shall be provided to the Owner. The Contractor shall be responsible for the safe handling and transportation of all waste generated by this contract to the designated waste disposal site. The Contractor shall bear all costs for all claims, damages, losses and expenses against the Owner, Consultants or the Owner's representatives, including but not limited to attorney's fees arising out of or resulting from spills en-route to the waste disposal site. 3.8 RESTORATION WORK Damaged areas will be repaired or replaced to the satisfaction of the Owner. Any ground areas that may become contaminated with asbestos shall be decontaminated at the Contractor's expense. Decontamination procedures shall require full containment procedures. Removal of soil to a depth of four (4) inches, which shall be disposed of as asbestos waste. All other surfaces shall be free from dust, debris, and contamination. The Contractor shall encapsulate all decontaminated surfaces. 3.9 ASBESTOS ABATEMENT ADDITIONAL REQUIREMENTS A. Containment Full containment shall be established for all work (except glove bag or glove box procedures). Completion of full containment shall include the establishment of critical barriers over grilles, vents or any other penetrations, mechanical and electrical equipment, windows and doors. Critical barriers shall be installed on HVAC system ducting passing through the work area to other building areas. Where ducts exist at the perimeter of containment and interior ducting is to be removed, the remaining duct at the perimeter shall be capped with solid plywood or sheet metal barriers, caulking and any other material necessary to achieve an airtight seal. The airtight seals shall be maintained until clearance is achieved in the work area. The continued use of an adequate number of Differential Pressure Units (DPU) is also required. Air from the work area shall be continuously exhausted, through HEPA Page 11 of 13

12 filtration, to the building exterior via ducting secured by plywood window inserts. The Contractor shall remove exterior windows as necessary to install plywood inserts. Removed window areas shall be boarded up upon removal of containment. DPUs shall be installed immediately at the start of the work and operated continuously thereafter until final clearance is obtained. The Contractor acknowledges that it is his responsibility to install critical barriers and seals, smoke test all barriers daily, and immediately repair any leaks or breeches in these seals to the satisfaction of the Owner or Owner's Representative. The Contractor shall provide additional moveable DPUs inside containment, which shall be located in each active work area during all preparation and abatement work. During above ceiling critical barrier installation, DPU intakes shall be located in close proximity to work with flexible intake ducting. B. Decontamination and Waste Load-out Units Three-stage or two-stage decontamination and waste load-out units shall be constructed with rigid framing where they are exposed to the public. Surfaces exposed to the public or exterior building shall be 1/2-inch thick plywood. Shop drawings of each unit, including connection of shower and drainage filter for contaminated water, shall be made available to the Owner or Owner's representative. C. Electrical The Contractor is responsible for securing the electrical service necessary for completion of abatement activities. All electrical work and materials shall be installed in accordance with the national electric code and all other applicable codes. The Contractor shall retain the services of a licensed electrician who shall be responsible for identification, coordination, installation and maintenance of temporary ground-fault protected electrical power to equipment and circuits inside the work area. The Contractor shall supply power for the Owner's Representative, as necessary, for area and clearance air monitoring. D. Safety/Emergency Exits Emergency exits shall be clearly marked in work areas. Emergency flashlights and signal horns shall be located within containment areas. The Contractor shall provide daily project safety meetings on-site. E. Other Requirements A "Job Board", with SDS, emergency numbers (24 hours), safety procedures, air monitoring results, and DOP test certifications for DPUs and HEPA vacuums, shall be conspicuously placed within the near vicinity of containment areas, as directed by the Owner or Owner's representative. The Contractor shall post perimeter barriers around staging areas with signage indicating, CONSTRUCTION AREA KEEP OUT, and post containment entrances and waste containers as required by OSHA.
