1 Addendum No. 2 (i) Issued January 23, 2018 REQUEST FOR PROPSALS FOR GENERAL CONTRACTOR SHSU CJC AHU Replacement RFP No.: H Notice to All Respondents: The following is Addendum No. 2 to the Request for Proposals (RFP) ESBD posting No H which was posted on January 4, 2018 Addendum No. 1 was posted on January 19, 2018 I. Clarification: A. See attached questions and answers that were presented for the RFP as well as the Asbestos Abatement Project Design document.

2 January 23, 2018 ADDENDUM 1 CJC AIR HANDLING UNIT REPLACEMENT A WING This ADDENDUM shall modify, amend, amplify, extend and become a part of the CONTRACT DOCUMENTS dated DECEMBER 22, 2017 prepared by RAMIREZ-SIMON ENGINEERING, LLC in accordance with the terms therein, as if written out in full therein and bound originally therewith. Questions concerning this ADDENDUM or the CONTRACT DOUMENTS, should be addressed to STEVE SIMON by , sjsimon@ramirez-simon.com. In cases of conflict, the ADDENDUM shall take precedence over previously issued CONTRACT DOUMENTS. Subcontractor should acknowledge receipt of this ADDENDUM when they submit their proposal. Questions: 1. Drawing MO.1 Specifications, Thermal Insulation, chilled water and condensate piping insulation calls for closed cell elastomeric insulation, Armaflex. However, no thickness requirement was given. Please specify. (Vaughn) 2. Drawing M1.1 Key Notes 2&3 call for new louvers to be installed. Is there any structural enhancements that will be required? Please advise. (Vaughn) 3. Drawing MO.1 Specifications, Piping, heating water piping is to be Type L Copper. Would 3 welded pipe be acceptable with the runouts to the VAV boxes being Type L Copper? Please advise. (Vaughn) 4. Acceptable Air Handler Manufactures - Clarification 5. It was stated at the pre-bid meeting that no temporary air would be required for bidding purposes. Please clarify. (Vaughn) 6. It was stated at the pre-bid meeting that the Owner would temporary remove a light pole and trim some trees near a dumpster area or parking area off of 16 th Street to facilitate lifting the HVAC unit. Please Clarify. (Vaughn) 7. It was stated that the pre-bid meeting that the question deadline for the project would be January 25 th. Please confirm as it states in the RFP January 18 th. 3 9. It was stated at the pre-bid meeting that there would be asbestos abatement on the project. Could an asbestos report be given prior to bid? (Vaughn) Responses: 1. M0.1 Mechanical Specifications - Modify sheet by adding under Thermal Insulation, Quality Assurance section: A. 4. Provide 1.5 thickness for pipe greater than 2 in diameter, and 1 for pipe less than 1-1/2 in diameter. (RSE) 2. Pending (RSE) 3. 8 PART 1 - GENERAL 1.1 DESCRIPTION This section covers labor, materials, facilities, equipment, services, employee training and testing, permits and agreements necessary to perform asbestos abatement in accordance with the plan of action, including but not limited to any other federal, state or local government regulations. Whenever conflict or overlap of regulations exists, the most stringent rules apply. 1.2 SCOPE OF WORK Location of Asbestos Abatement Project: A300U1 Chill Water Supply/Return, Chill Water Supply/Return, Hot Water Supply Insulation Associated With AHU-A1, AHU-2, AHU-3, AHU-A4 and FA-RA-1. Approx. 650 LF. See attached piping drawing. Duct Insulation Associated With AHU-A1, AHU-2, AHU-3, AHU-A4 and FA- RA-1. Approx. 1,300 SF. See attached HVAC duct drawing. The above materials shall be removed licensed by asbestos workers in personal protective equipment (PPE) inside negative pressure containment using wet removal methods and proper waste disposal. All abatement activities must be performed in accordance with applicable Department of State Health Services (DSHS) Texas Asbestos Health Protection Rules (TAHPR), EPA NESHAP and applicable OSHA regulations. OSHA approved glove bag methods may be used on pipe insulation only as needed Work area shall be equipped with the following: Regulated areas within which asbestos abatement is to be conducted shall be separated from adjacent areas by impermeable barriers of four (4) mil polyethylene sheeting attached securely in windows, doorways, corridor entrances, ventilation openings (both air supply and return air), drains, ducts, grills, grates, diffusers and skylights, shall be sealed with a minimum of a single layer of six (4) mil polyethylene sheeting. All penetrations that could permit air infiltration or air leaks through the barriers shall be sealed, with exceptions of the make-up air provisions and the means of entry and exit All asbestos shall be placed into labeled, 6-mil poly impermeable bags (double bagged) or wrapped in two layers of 6-mil poly. Waste may be disposed of at any approved landfill that accepts asbestos-containing materials (ACM) The abatement contractor shall be responsible for securing the work area from unauthorized entry for duration of asbestos abatement project.

11 1.6 SAFETY REQUIREMENTS Fire Safety: The number, type and location of fire extinguishers in work areas satisfying the requirements of OSHA 29 CFR "Portable Suppression Equipment". Each extinguisher shall be maintained in a fully charged and operable condition Electrical Safety: All active service lines within the regulated and containment area shall be connected through ground-fault circuit interrupter (GFCI) units. 1.7 UTILITIES Owner shall provide temporary electricity and water sources sufficient in size, power and capacity to complete project Use only grounded extension cords and use "hard services" cords in areas where cords will be exposed to abrasion and traffic. 1.8 EMPLOYEE TRAINING Supervisors and foreman shall have training and knowledge of applicable regulations and expertise in safety and environmental protection as evidenced by participation in and successful completion of a training course offered by an EPA endorsed educational institution and satisfying requirements of OSHA 29 CFR At least one person "competent person" shall be trained in the provisions of National Emission Standards for Hazardous Air Pollutants (NESHAP) Workers and abatement supervisor shall be DSHS licensed to perform asbestos abatement work and have training equivalent to the EPA Model Accreditation Plan for asbestos abatement workers, 40 CFR, part 763, Subpart E, appendix C All personnel must undergo and pass an annual medical surveillance by a licensed physician. 1.9 RESPIRATORY PROTECTION All asbestos abatement personnel shall be provided personally issued and marked respiratory equipment approved by NIOSH or OSHA. New filters shall be provided as needed by the personnel The minimum respiratory protection required for this project shall be a 1/2 face air purifying respirator (APR) equipped with high efficiency particulate air (HEPA) filters.

14 a) Lockdown agents (encapsulants) must be EPA approved b) Encapsulants shall not be noxious or toxic to application workers and shall possess high flame retardant characteristics as well as low smoke emission ratings. c) Encapsultants shall exhibit some water vapor permeability to prevent condensation accumulation and common cleaning agent solution resistance. 2.2 TOOLS AND EQUIPMENT Transportation Equipment: Transportation equipment as required shall be suitable for loading, transport and unloading contaminated waste without exposure to persons or property Vacuum Equipment: All vacuum equipment shall include HEPA filtration systems Water Sprayer: The sprayer shall be adequate to keep material wet Other Tools and Equipment: The Abatement Contractor shall provide suitable tools for striping, removal, encapsulation and disposal activities. Spray foam shall not be used to seal openings due to potential fire hazards. PART Ill- EXECUTION 3.1 PRE-ABATEMENT PREP ERA TION Install critical barriers per section Surfaces shall be Hepa vacuumed and wet wiped prior to post abatement visual inspection. Install 2 layers of 4 mil poly sheeting on walls. Protect floors with 2 layers of 6 mil poly sheeting. 3.2 ASBESTOS REMOVAL All asbestos-containing material that will be abated must be sprayed with water containing a removal agent during the abatement process. Water shall be applied as a fine low water process. Water shall not be bagged under any circumstances. Standing water shall be filtered using minimum of a 5 micrometer filter. 3.3 FINAL INSPECTION Upon completion the Asbestos Project Manager shall conduct a visual inspection of the abated areas to ensure all material in scope of work has been removed and no visible ACM debris remains.

17 4.3 PRE-CONSTRUCTION CONFERENCE: Schedule an initial progress meeting, recognized as Pre-Construction Conference Prior to start of any work, meet at project site with CONTRACTOR's Superintendent, OWNER, CONSULTANT, and other entities concerned with Asbestos Abatement Work. Record discussions and agreements and furnish copy to each. 4.4 REPORTING Daily Log: a. b. C. d. e. f. g. h. i. j. Maintain within the Decontamination Unit a daily log documenting the dates and time of, but not limited to, the following items: Meetings; purpose, attendees, discussion (brief) Visitation; authorized and unauthorized Personnel; by name SS# entering and leaving the work area Legible Asbestos Workers DSHS license, Worker's Acknowledgement, Fit Test, and Physical Examination. Inspection of work area preparation prior to start of removal and daily thereafter. Removal of any polyethylene barriers CONTRACTORs daily manometer readings Removal of waste materials from work area Decontamination of equipment (list items) CONTRACTORs Daily Project logs Provide two (2) copies of all close-out documents at final closeout of project for use by the OWNER and CONSUL TANT.
