Vulcan County W W4 04-Mar Amendment No. 6 issued to EPO-2015/02-SSR inserting after Clause 2(f) of Amendment No. 5: "(g) The Company shall, at the same time as conducting the groundwater monitoring and sampling required under the Revised (January 18, 2016) In-Situ Groundwater Delineation Proposal, conduct surface water sampling and analysis by carrying out at least all of the following activities: (i) Surface water quality grab sampling and analysis of the Little Bow River, (A) Directly adjacent to the Site, in line with the location where the Substance entered the Little Bow River from the Site; (B) 100 metres upstream of the Site; and (C) 100 metres downstream of the Site for (D) all the same parameters as groundwater, as described in clause 2(d) of Amendment No. 5"; and deleting Clause 4(q) of Amendment No. 5 and replacing with: "A discussion of how all the surface water samples described in clause 4(n) compare to each of the following: i. the Environmental Quality Guidelines for Alberta Surface Waters July 14, 2014, as amended, and ii. all surface water samples taken under the requirements of this Order, as amended, including, but not limited to, the November 30, 2015 Short Term Monitoring Report." 3 of 12

5 Owens Corning Insulating Systems Canada GP Inc. Edmonton SE W4 Enforcement Orders 13-Jan (e) Owens Corning Insulating Systems Canada GP Inc. holds Approval No for the operation of an insulation manufacturing plant located at 831 Hayter Road in the City of Edmonton, Alberta. On January 4, 2016 the Company experienced a fire which caused damage to the Ceilcote wet scrubber, a piece of pollution abatement equipment associated with plant. The Approval currently does not allow them to operate unless and until the pollution abatement equipment associated with the process is operational. The Director is of the opinion that the Company has contravened section of the Approval by operating the plant without the secondary Ceilote wet scrubber in place. Owens Corning is working with Alberta Environment and Parks to design a protective air monitoring program which will be required during operation of the plant until the secondary scrubber is again operating. Releases from the W2 Curing Oven Exhaust Stack shall not exceed the Approval Limits; The Company shall monitor and immediately notify the Director if any significant deviation from normal performance parameters used to monitor and control Ducon wet scrubber operations occurs; Operation of the W2 plant process equipment shall cease immediately if the air emission limits in section of the Approval are exceeded; The Company shall perform Additional Manual Stack Surveys in accordance with the Approval; Develop, Submit, and Implement the Ambient Air Monitoring Plan; Notify the Director and the Strathcona Industrial Association membership if the Alberta Ambient Air Quality Objective for particulate matter is exceeded; Develop and submit a written Remedial Plan within 60 days of the date of this Order, and; Implement the Remedial Plan as authorized by the Director. Weder, Cristoph and Weder- Bueschi, Erika Birch Hills County SW W6 NE W6 SW W6 NW W6 SE W6 NW W6 SW W6 NE W6 SE W6 SE W6 NW W6 NE W6 SW W6 SW W6 29-Feb-16 PLA The lands are owned by the Crown in right of the Province of Alberta, and are 47(1)(b), "public lands" within the meaning of section 1(p) of the Public Lands Act. The 54(1)(a.1), Public Lands Act requires a person to obtain a disposition, or other form of 54(1)(a.2), authorization from Alberta Environment and Parks ["AEP"] to use or occupy 54(1)(c), 54(1)(e), public lands in Alberta. Temporary Field Authorization within GRL (1)(f) was issued for the specific purpose of fence line and trail clearing and included specific conditions as to how this activity was to be Undertaken. An inspection by AEP found approximately 15 kilometres of trail had been constructed; trail construction outside the areas identified in TFA155134; construction of trails were done with a crawler tractor and not the authorized mulcher; fence lines and trails exceeded the maximum width of 3 metres; woody debris and leaning trees were not slashed, limbed or bucked flat to the ground as required, and; soil and vegetation disturbances were noted within the 100 metre buffer of the Peace River. Reclamation work is required to restore the disturbed areas to predisturbance contours and conditions. The Parties shall immediately vacate the 5 of 12

6 SW W6 SE W6 NW W6 NE W6 SW W6 SE W6 SE W6 NW W6 SW W6 SE W6 SW W6 NE W6 NW W6 Enforcement Orders Crown Lands, and cease entering the Crown Lands, except to undertake the reclamation works that are required pursuant to this Order; Submit the Reclamation Plan prepared by, or under the direction of a Registered Professional Agrologist or Registered Professional Forester; Submit a proposed Schedule of Implementation; Reclaim the Crown Lands in accordance with the Reclamation Plan approved by the Director; Provide the Director with at least two business days written notice prior to the commencement of the reclamation work; Submit a written final report within 15 days of the completion of the work in the reclamation plan. A & T Construction and Transit Mix Ltd., Tkachuk, Brian and Tkachuk, Wade Lac La Biche County SW W4 23-Mar-16 36(1), 142(1)(h) A & T Construction and Transit Mix Ltd. ["the Company"] is the registered owner of the lands and Brian Tkachuk and Wade Tkachuk are the sole Directors and have at all material times made the decisions in respect of the issues which form the subject matter of the Order. The Company owns and operates a concrete producing plant on the lands and there is a fen [the "Wetland"] adjacent to the Plant which lies between two Crown owned water bodies. Aerial and ground Inspections by Environmental Protection Officers observed a large mass of concrete and concrete slurry approximately 450 square metres in size in the Wetland, and a truck bearing the Company's logo was observed releasing concrete slurry into the Wetland. The placement of the concrete mass and concrete slurry in the Wetland has altered of may become capable of altering the flow or level of water in the Wetland, and has caused or may become capable of causing an effect on the aquatic environment. The Parties shall immediately cease all unauthorized activity on the Lands, including any further placement of concrete, fill or any other material in the Wetland; Submit for Directors approval, the name and qualifications of a Qualified Wetland Science Practitioner to prepare the Wetland Restoration Plan; Include a Schedule of Implementation; Implement the work described in the Wetland Restoration Plan under the supervision of the QWSP; and within 6 months of completing the requirements of this Order, submit a final written report describing the work undertaken to comply with this Order. 7 Richard Repair Services Ltd. and Richard, Robert Lac La Biche County SE W4 Environmental Protection Orders 05-Feb Richard Repair Services Ltd. is the owner of lands legally described as SE W4M, in Lac La Biche County, Alberta. Robert Richard is the Owner, Operator, and sole Director and Shareholder of Richard Repair. On August 10, 2015, Alberta Environment and Parks ["AEP"] received a complaint regarding waste deposited within the Lands. An Inspection by AEP found a release of motor oil within 50 meters of fish bearing Plamondon Creek. AEP contacted Robert Richard with respect to the release and Mr. Richard agreed to clean up the substance. The Director is of the opinion that a release of a substance into the environment has occurred and that the release may cause or is causing an adverse effect on the environment. The Parties shall prepare and submit a remedial plan incorporating Alberta Tier 1 Soil and Groundwater Remediation Guidelines; upon approval, implement the work identified in the remedial plan; ensure that all work executed in the approved plan is directly supervised and documented by a qualified environmental specialist; and submit written reports confirming that the work identified in the remedial plan is being completed. 7 of 12

8 Prosecutions Wallace, Clarke Damian 01-Mar-16 : $10, (e) Count 4: "Co-accused Mantai, Samantha Lynn Sharon" On or between the 1st day of February, 2013 and the 11th day of March, 2013, at or near Langdon, in the Province of Alberta, did contravene a term or condition of an approval; to wit, Approval No , Condition which states: Collection, preservation, storage, handling and analysis of samples and reporting shall be conducted in accordance with the following unless otherwise specified in writing by the Director: (a) the Standard Methods for the Examination of Water and Wastewater published jointly by the American Public Health Association, American Water Works Association and the Water Environment Federation, as amended; and/or (b) the Methods Manual for Chemical Analysis of Water and Wastes, Alberta Environmental Centre, Vegreville, Alberta, 1996, AECV96-M1, as amended and did thereby commit an offence contrary to section 227(e) of the Environmental Protection and Enhancement Act, evidence of the offence having first come to the attention of the Director on March 11, Pled guilty and was sentenced to a fine of $10, of 12

Strohschein Farms Ltd Wetaskiwin County 22-Feb-16 Strohschein Farms Ltd conducted unauthorized ditching on or prior to March 26, SW W4 36(1) Works rendered ineffective by infilling the drainage ditch and restoring capacity of the land has been completed. Greenfield Developments Inc. Crossfield NW W5 23-Feb-16 36(1) The Company conducted an unauthorized activity at the Westgate Subdivision, the disturbance of 0.23 hectares of a Class IV wetland (Wetland #2) within the project area as discovered in April Water Act Approval was issued in November 2015 for the disturbance to Wetland #2 and compensation for wetland loss was paid to Ducks Unlimited Canada in January of 12

11 Lac La Biche County Lac La Biche County SW W4 Warning Letters 02-Mar-16 36(1) Investigation undertaken at the Beaverhill Road upgrade identified work conducted within a water body without the required Water Act Approval. A remedial plan was requested from the County to assess the impact of the activity and provide mitigation options. The plan was received and reviewed, and after several revisions accepted by Alberta Environment and Parks. Alliance Ready Mix Concrete Ltd Edmonton Plan , Block 2, Lot Mar (e) COPCP; 6(3)(c) The Company contravened the Code of Practice for Concrete Producing Plants by failing to control fugitive dust emissions at its facility, as confirmed during an inspection on June 5, Compliance has since been achieved through various dust mitigation measures. MD of Greenview W6 10-Mar-16 FRSTA 60/1973; 100(1)(a) The Company conducted an unauthorized activity, the construction of a road in block and on or about the 11th day of February Tolko Industries Ltd. High Level SW W5 15-Mar-16 FRSTA 60/1973; 100(1)(a) The Company did not conduct timber operations in accordance with their approved annual operating Plan, being, the opening of a seismic line without prior approval between block 1400 and 1461 on March 11, Erisman, David Leduc County NE W4 22-Mar-16 36(1), 142(1)(h) Mr. Erisman conducted an unauthorized activity in October, 2015, being, the removal of a dam on neighbors adjacent property and the removing and disturbing of ground and vegetation in or on any land, water or waterbody, which may alter the flow, the level, or cause siltation of the water. Goodliffe, Justin Westlock County NW W4 22-Mar /96; 23(1) Mr. Goodliffe conducted an activity, the disposal of domestic sewage in a place other than an authorized waste management facility on August 22, Compliance achieved with the commitment from Mr. Goodliffe that all future disposal will be done at a registered wastewater lagoon. 11 of 12

12 Williams, Brian Keith; Williams, Shelly; Skoreyko Crushing Ltd., and Skoreyko, Travis Lac La Biche County SW W4 Water Management Orders 23-Mar-16 99(1), 97(1)(c) Brian Keith Williams and Shelly Williams are owners of the lands in which Skoreyko Crushing Ltd. [the "Company"] operates a gravel pit and Travis Skoreyko is the sole Director of the Company. The Beaver River transects the Lands and is a fish bearing river that flows throughout the year. On March 15, 2016, Alberta Environment and Parks ["AEP"] received a complaint of a crossing that was built over a river on the Lands and that the river was filled with gravel. AEP found no record of authorization and an inspection identified a bridge had been built over the Beaver River to access an area where it appeared gravel extraction was occurring or had occurred ["the Bridge"]. Upon contact the Williams confirmed they had hired a contractor, Travis Skoreyko, to operate the gravel pit and to construct the Bridge. The Bridge's abutments were constructed far into the bed and shore of the river significantly narrowing the active channel of the river which has caused or may cause an adverse effect on the aquatic environment including fish and fish habitat and property upstream or downstream of the Bridge. Immediate measures to remove the Bridge are required before the restricted activity period for the Beaver River begins on April 16, 2016 in order to prevent further adverse effects to the aquatic environment or to property from occurring. The Parties shall immediately cease operation or any activity or works related to the Bridge and contain and isolate any gravel and debris on the river under the Bridge; submit a Remedial Plan prepared and signed by a Qualified Wetland Science Practitioner; implement all the work described in the Remedial Plan; provide 3 days written notice prior to commencing the remedial work and; within 30 days of completing the requirements of this Order, submit a Final Report. 12 of 12
