1 March 1, 2017 Ms. Katie Robertson Oregon Department of Environmental Quality 800 SE Emigrant, Suite 330 Pendleton, Oregon Re: Monitoring Well MW 25 Abandonment Fashion Cleaners Site (ECSI #1004) 623 Klamath Avenue Klamath Falls, Oregon /Task 2 Dear Ms. Robertson: This letter summarizes the abandonment of monitoring well MW 25 at the Fashion Cleaners site near the intersection of Klamath Avenue and 6th Street in Klamath Falls, Oregon (Figure 1). Abandonment activities were conducted on September 26, 2016, and included abandoning MW 25 by overdrilling in anticipation of building upgrades at the Kern Building adjacent to the former dry cleaner property (Figure 2). All work was completed for the Oregon Department of Environmental Quality (DEQ) under Task Order Preparatory Activities Hart Crowser conducted a competitive bid solicitation for private utility locating, well abandoning, and for picking up and disposing of investigation derived waste (IDW). Based on cost and the ability to complete work within the required timeframe, the following subcontractors were selected: APS Locates of Portland, Oregon, to perform private utility locates at the abandonment location; Stratus Corporation (Stratus) of Gaston, Oregon, for abandonment of MW 25; and Waste Management of Portland, Oregon, to pick up and dispose of the IDW. Access to the well location was granted by an access agreement that DEQ has with the affected property owner. Abandonment Activities Well abandonment activities, including mobilization to the site, occurred on September 26, 2016, and were completed within one day. Monitoring well MW 25 was abandoned by overdrilling by Stratus, an Oregon licensed driller, in accordance with OWRD regulations (Oregon Administrative Rule ). 2 Oregon Department of Environmental Quality /Task 2 March 1, 2017 Page 2 Hart Crowser arrived on site the morning of September 26th with APS Locates to perform underground utility location in the area around MW 25 prior to abandonment activities. Abandonment by overdrilling consisted of removing the flush mounted monument and pulling the well riser casing and well screen from the ground (Photographs 1 and 2). Upon casing and screen removal, the well bore was overdrilled using a push probe drill rig with an auger head attachment to 27 feet below ground surface (bgs; Photograph 3). Hydrated bentonite chips were used to backfill the hole to approximately 2 feet bgs. Concrete was then placed on over the bentonite as a surface seal from 2 feet bgs to approximately 4 inches bgs, where asphalt patch was used to complete the backfilling to match the surrounding surfaces (Photograph 4). Investigation derived waste (IDW), including well casing, concrete, steel monument, bentonite/concrete bags, and personal protective equipment, was disposed of by Stratus as solid waste. Additional IDW generated during abandonment activities included approximately 7 cubic feet of soil cuttings generated during overdrilling and approximately 50 gallons of decontamination water. This IDW was containerized in two separate, labelled 55 gallon drums and stored on the site. Because tetrachloroethylene was detected at the site, a review of analytical data from the IDW samples was conducted. Based on the results, the DEQ issued a No Longer Contained In Determination on November 14, 2016, allowing the soil and decontamination water to be disposed of as non hazardous waste. The IDW was picked up on January 5, 2017, and delivered to Waste Management s disposal facility on January 31, The No Longer Contained In Determination and final disposal receipt are provided in Attachment C.
