1 Dear 4 H Member, We are excited about your desire to become a member of a State 4 H Leadership Board. This State 4 H Youth Board application is the beginning opportunity providing leadership, advocacy and youth voice. As part of the State 4 H Youth Development program you will gain new or enhanced leadership skills, new friends and connections that will last a lifetime. While the Executive Board and Shootings Sports Ambassadors have different applications, this application packet is for the Citizenship, Fashion, Food and Fitness, the Performing Arts Troupe, and the SET (Science, Engineering and Technology) Boards. As you prepare your application, here are a few reminders and tips to help you be successful. It s important that you work with your Parish 4 H Agent regarding the application and submission process and deadline. Application must be signed by your Parish 4 H Agent. There are two separate parts to this application: General and Board Specific. Read carefully through the board information. Check your calendar against required attendance dates for board activities. Make sure that you can attend the meetings before making a commitment. Read the questions and respond appropriately. Align experiences and topics with the focus of the board for which you are applying. Check spelling and grammar prior to submission. As you identify individuals to write your letters of recommendation, you will want to select individuals other than family members and 4 H Agents. Examples but not limited to leader, employer, teacher, advisor, clergy, etc. Be respectful and allow time for these individuals to write and submit the letter of recommendation. Communication will be done primarily through e mail so it s important to check often. You are responsible for providing your own transportation to board activities. Be aware that there are nominal fees associated with membership. These fees may include but are not limited to membership dues, camp fees, and meeting fees. Camp counselor training is required. Practice your interview skills We are so glad you are taking the time and effort to apply for a State 4 H Leadership Board, and whether you are selected or not, you are already a winner for going through this process. This experience is helpful both now and later in life when applying for scholarships and jobs. Good luck with your quest to serve on a State 4 H Leadership Board. Any adult volunteers and agents interested in serving in a volunteer capacity on the board will need to check with the corresponding board advisors for additional details. GOOD LUCK!! 2 LOUISIANA STATE 4-H BOARD INFORMATION Youth Application for Citizenship, Food and Fitness, SET, Performing Arts and Fashion Boards Deadline: May 5 th State 4-H Leadership Board: 4-H members will serve on the Leadership Board for a one year period. They will promote the Louisiana 4-H Leadership Board Program and serve as its advisory group. The State 4-H Leadership Board Coordinator and selected 4-H agents will serve as advisors to the specific Board. Objective: The objective of the State 4-H Leadership Program is to assist with the development of educational programs that provide opportunities for advocacy, leadership, and youth voice. Eligibility for Participation: The applicant must be currently enrolled in 4-H and classified in the 9 th - 12 th grade the year they serve. (Please see board supplemental sheet for board specific requirements) Membership, Tenure and Selection: Tenure is one year. Board Members may serve unlimited terms, but must reapply each year. May have up to 25 members (discretion of the advisors). Alternates (depending on the number of applicants) may be selected. Each parish may nominate up to three members (per board). Selection will be from written application and interview. The application will be reviewed by a committee. Written applications are due prior to May 5 th by 4:30 p.m. Deliver to the State 4-H Office. Members may not serve on more than one board. If applying for more than one board, please indicate so on the application. If necessary, members are required to choose on which they will serve for the upcoming year. Members will be required to: Attend regular Board meetings to include but not limited to Summer and Winter State 4-H Leadership Board meetings Attend, help plan, and conduct Board events as specified by each board Serve as state liaison in the region and parish they represent Acquire certification as camp counselor prior to working with youth as a State 4-H Leadership Board Member Check for board announcements and updates Correspond promptly with advisors and officers Provide their own transportation and all or some funds for meetings and other scheduled events Members will be required to inform parish agents on board member's proposed plans relative to the board Criteria for Evaluation Application & Experiences 25% Essay 25% Interview and Recommendations 50% Application Process: Submit complete application by May 5 th 4:30pm 4-H members submit application form, essay, and 3 recommendation letters. 7 4 H Food and Fitness Board Supplemental Sheet Additional information for those applying for the State Food and Fitness Board: 4 H Food & Fitness Board: 4 H members will serve on the Food and Fitness Board promoting the Louisiana Healthy Living Program and serving as its advisory group. The State 4 H Healthy Living Mandate Coordinator(s) and assigned 4 H agents will serve as advisors to the Food and Fitness Board. Objectives: Members will: 1. Work with board advisors and/or Nutrition and Fitness Specialist to plan and implement healthy living related programs that provide for life skill development related to food, nutrition, fitness, and overall health (including food prep, food safety, basic nutrition, healthy choices, physical activity, etc.). In addition, opportunities for the development of decision making, communication, planning and organization, problemsolving, leadership and service will be promoted. 2. Gain skills necessary to make and teach healthy food choices in a variety of settings; mastery of skills which lead to a healthy lifestyle. 3. Gain knowledge about the sciences of food/nutrition, exercise/fitness, and understand the scientific methods used in research. 4. Explore careers in dietetics and nutrition, food science, fitness/kinesiology, and other health related fields. Membership, Tenure, and Selection: Members will be required to attend 4 H U and participate in the interview process (the interviews take place on Tuesday afternoon of 4 H U as scheduled.), summer board meeting, fall board meeting, winter board meeting, and the 4 H Food and Fitness camp. To remain in good standing as a board member, attendance is required at these meetings and/or events. Absence from any of the above mentioned meetings will be reviewed and membership status will be determined by advisor and parish 4 H agent of member. See application for additional criteria. Members may be asked to: Plan and conduct a training session at JLC (Junior Leadership Conference), 4 H U and other state/district 4 H events Implement a parish level Food and Fitness Board, if possible Develop an educational program focusing on healthy lifestyles for Louisiana 4 H Plan and implement community service learning projects as requested Participate in other 4 H events such as the Governor s Mansion dinner and the LSU football game. 8 Louisiana State 4-H Leadership Program Submit prior to May deadline Fashion Board Member Youth and Adult Volunteer Application Textile and Apparel Program Objectives: To assist with the development of educational programs that provide opportunities to explore careers in fashion: textile science, apparel design, merchandising and related industries; develop leadership abilities and character; build self-confidence and poise by improving ability to make wise consumer purchases and financial decisions regarding apparel; recognize quality and predict performance of textile and apparel products; expand and share textile and apparel knowledge, including technical and creative skills; and improve public speaking and presentation skills. Application Process: 1. Complete the application stating interest in the 4-H Fashion Board and submit complete application packet with appropriate signatures to state office prior to deadline. 2. 9 4 H Performing Arts Troupe Supplemental Sheet Additional information for those applying for the State Performing Arts Board Objectives: Market 4 H through performing Develop leadership and communication skills Develop mastery in the area of performing arts Application Process: 1. Complete the application stating interest in the 4 H Performing Arts Board and submit to your parish 4 H Agent for signature. Submit complete application packet to state office prior to deadline. 2. Prepare and send in a CD with what you plan to perform during the audition. Label CD with your name and parish. 3. Participate in an audition on the Tuesday afternoon at 4 H University, June 17, 2014 at 1:00 p.m. from 3:30 p.m. Once applications are reviewed you will be given a specific time for your audition. You can check with your agent to find out the time of your audition. 4. You must bring your music and/or instrument. Piano or keyboard will be provided if you are auditioning on the piano. 5. You need only to prepare for a 2 minute audition. 6. Be prepared to answer questions about your craft and yourself during the audition, but, after your performance. Members will be required to: Participate in both summer and Winter Performance Meetings. July 2014 and January 2015 Help plan or conduct Talent Show at 4 H U.
