5 Page 5 of Description of Incident Disruption of any portion of a terminal or interline baggage handling system (BHS), including belts, bar code readers, in-line security systems, doors, computer monitors, printers, or the Baggage Information Display System (BIDS) that interferes with the delivery of passenger luggage or baggage to or from aircraft or the display of baggage-related information to arriving passengers. (Excludes any criminal or nefarious intent) 2. Overall airport mitigation measures include: Daily system health checks to verify status and stability of servers and interfaces Multi-agency joint vulnerability assessments of critical infrastructure Hardware and software redundancy to ensure high availability of system services Airport Operations participation in terminal manager meetings to discuss the logistics and coordination of BHS/BIDS efficiencies and operational issues that may affect airport operations (e.g., Inline Service, BHS/BIDS systems interruptions, and back-up system processes) Page 5 of 18 Template V.2.1 Revision Date:

6 Page 6 of 18 Airport Operations conducting routine inspections of all terminals and providing a list of BHS/BIDS service issues for immediate repairs 6. Preparedness Measures a. Information Management Technology Group (IMTG): The IMTG Airport Response Coordination Center (ARCC) Management Division is responsible for the software interface that transmits bag sortation messages (BSMs) into the BHS systems at TBIT and Terminal 3. Response times are immediate for BSM-related issues. On-site staff is available 16 hours a day; on-call staff is available around the clock. Participation in planning and drills with IMTG, FMCD/CUP and the contract vendor to ensure back-up and other BHS/BIDS system failure procedures are managed effectively during BHS/BIDS system failures Development of forms and utilization of software programs by the ARCC to ensure staff is adequately prepared to support responders with information and resources, as needed Manage outcome of BHS issues (system orientation, training, and maintenance are overseen by contracted vendor) o Response protocols vary based upon the terminal affected by a BHS/BIDS system failure o Terminal staff training (e.g., airline representatives) involves the appropriate notification process to airport operations and other affected stakeholders Airport Operations ensures that BHS/BIDS contractors receive orientation and training concerning construction area(s) and repair situations that would prevent the ACAMS system from functioning properly (for BHS equipment secured behind ACAMS-protected portals) Construction coordination meetings are held to ensure a clear understanding of existing protocols and protection measures that are in place that prevent faulty ACAMS doors from being used for extended periods c. Airport Police Division (APD): Participation in hazard-specific planning, training, and exercises d. Facilities Maintenance and Construction Division (FMCD), Maintenance Services Division (MSD), Central Utilities Plant (CUP): Participation in hazard-specific planning, training, and exercises (also manage the BHS contractor at Terminals 1, 3 and a portion of 6) e. Media and Public Relations: Develops and implements programs and procedures, including all necessary staff training in responding to media and public inquiries regarding any incident or events that may occur within the airport f. Los Angeles Police Department (LAPD): Participation in multi-agency planning and training exercises g. Customs and Border Protection (CBP): Maintains procedures for the control of passengers and employees within CBP facilities for any incident or event that may occur within CBP controlled areas h. Transportation Security Administration (TSA): Conducts ongoing emergency drills and other training as necessary to support airport operations 7. Detection and Warning Source / Means a. IMTG: Phone call to IT Service Desk during regular business hours, or from IMTG or ARCC Duty Manager after regular business hours Notified by the affected airline, BHS/BIDS service provider, observant Airport Operations staff, or APD Communications Unit. Notifications are also received by the contract vendor that is managing the system, which varies from terminal to terminal Page 6 of 18 Template V.2.1 Revision Date:

7 Page 7 of 18 ARCC: Airport Duty Manager (DM) will ensure that the initial and subsequent ARCC notifications are sent to key stakeholders per established procedures c. APD: Notification may occur via observation by APD personnel or by telephone to APD APD staff shall notify APD Communications Unit Supervisors shall notify ARCC personnel and provide an ongoing status report on the incident d. FMCD/MSD/CUP: Support would be requested through the Facilities Maintenance Representative (FMR) located in the ARCC e. LAWA ADA Coordinator s Office: Notified through the ARCC Notification System f. CSRs/VIPs/ART: CSR Manager notified through the ARCC Notification System; determines which group(s) to notify next through the ARCC Notification System g. LAPD: Radio call or telephone h. FBI: APD contacts the FBI LAX duty agent directly if necessary Agents also have APD radios and can listen to events as they occur i. CBP: Immediate CBP chain of command notifications j. TSA: Terminal management and/or the ARCC k. Consider adding in affected airlines information to include who to call for information concerning gates changes etc. l. Consider vender specific actions and activities (information to be provided by Tibit Tech vendor) 8. Activation Criteria and Initial Alerting Activation may begin with an internal call from the affected terminal staff, BHS/BIDS contract vendor, other LAWA staff or by an external notification (e.g., a call, or radio report from an airport unit) a. IMTG: Notified by on-site LAWA and/or contract vendor staff that are monitoring the BHS and BIDS data exchange information; monitor BSM data exchange DM: o Notified by on-site LAWA or vendor staff and then utilizing the ARCC notification system to provide additional notification as necessary o May activate the Department Operations Center (DOC) as necessary to ensure deployment of staff to the Incident Command Post (ICP) when established, and to recall or call in additional staff o Ensures that the initial and subsequent ARCC notifications are sent to key stakeholders per established procedures DOC: o Activated at the discretion of the DM, or at the request of the ICP c. APD: Initial law enforcement resources are dispatched to the incident as necessary d. LAFD: Respond to alarms per dispatch matrix or as directed from the Metropolitan Fire Communication (MFC) The 24-hour civilian duty officer notifies Media and Public Relations Managing Director, LAWA Public Relations (PR) Director and PR Division employees to respond during off-hours and weekends, if necessary e. FMCD/MSD/CUP: Notified through ARCC f. Media and Public Relations: ARCC notification system notifies civilian and APD Public Information Officers (PIOs) of severity of any incident and the impact on airport operations Page 7 of 18 Template V.2.1 Revision Date:

8 Page 8 of 18 If necessary, the 24-hour civilian and one APD PIO respond during off-hours and weekends to the ICP The 24-hour civilian duty officer notifies Media and Public Relations Managing Director, LAWA Public Relations (PR) Director and PR Division employees to respond during off-hours and weekends, if necessary g. LAWA ADA Coordinator s Office: Contact City Department on Disabilities (DOD) as needed for sign language (ASL) interpreter(s) and other necessary mutual aid assistance to support persons with disabilities h. LAPD: Respond in support as requested or as necessary i. CBP: Implementation of the Continuity of Operations Plan (COOP) based on the assessment of any situation; then access, secure, isolate, notify and determine the activation of CBP emergency plans as necessary j. FBI: Activation and mobilization of FBI resources and assets as requested or necessary i. TSA Notified as necessary by the IC to support the continued screening of baggage for security issues 9. Lead Department(s) a. IMTG (responsible for the BSM interface) b. Airport Operations (overall non-public-safety coordination) c. APD d. FMCD/MSD (responsible for physical hardware components of the BHS, at Terminals 1, 3 and a portion of 6, with support from CDG and LAX 6 as needed) e. TSA f. CBP 10. External Notifications a. IMTG: IT Service Desk Notification to stakeholders, or from ARCC Duty Manager notification via ARCC notification system DM: o Utilize the ARCC Notification System to notify both airport stakeholders and internal Los Angeles World Airports (LAWA) personnel o Notify Air Traffic Control (ATC) via landline or other communications pathways prior to the issuance of a Notice to Airmen (NOTAMs) to inform the airlines and their crews of airfield impacts, if any o For a High Level impact the ATC will be notified to ensure they are aware of any significant delays (e.g. gate changes) o Coordinate with Media and Public Relations Duty Officer regarding statements to the public o Notify the LA City Emergency Management Department (EMD) Duty Officer as necessary ARCC: o Notify external entities via telephone, or utilizing the ARCC Notification System c. APD: When an unusual occurrence is reported to the APD Communications Unit, the communications supervisor, or designee, shall make appropriate notifications by radio, phone, text messaging, and d. FMCD/MSD/ADG/CPPG/CUP: Notified as requested or as necessary e. LAPD: The Watch Commander will respond to the ICP, LAX Field Services Division (LAPD-FSD) Station and make notifications to the LAPD Communications Division as necessary or requested In addition, Real Time Analysis and Critical Response (RACR) Division will be notified f. 9 Page 9 of 18 g. CBP: As necessary or requested h. TSA: Terminal management will notify the Coordination Center 11. Incident Command / Unified Command a. IMTG: Within the UC, provide resources and assets as necessary to manage BHS/BIDS system failures as soon as possible Within the UC, coordinate all airside and landside operational activities as needed to support mitigation of the BHS/BIDS failure and any related or anticipated impacts For a Low Impact Activation, the Incident Command Post (ICP) occurs at the affected site, with coordination and support provided by the ARCC, and unified command (UC) established as needed For a Medium or High Impact Activation, the DOC is activated An overall ICP may be established at a central location, for field-level command and control, with individual branches (or additional field command posts) established at various impacted terminals at the request of the incident commander/unified command DM: o Ensures timely and appropriate Operations supervisory response to the ICP to participate in the UC o In coordination with Landside, establishes initial incident command for the airport o Activate the DOC based on the level of the incident and ongoing conditions, including assessments of systems and areas affected and resulting threats/hazards. Participants may include IMTG, FMG, APD, LAPD, and LAFD. ARCC o Supports the ICP by monitoring the situation, maintaining situational awareness among participants, making notifications, and coordinating resource mobilization and deployment, as needed DOC: o Activated at the discretion of the DM or at the request of the ICP, provides the coordination of communications and resource management between the ICP, ARCC, senior airport leadership, vendors, and others c. LAFD: Respond as necessary or requested d. APD: Coordinate law enforcement and security operations Assign representatives to UC and coordinate with the IC as necessary e. FMCD/MSD/ADG/CPPG: Coordinate other necessary support such as from CDG,LAX 6 (T6),TBIT-TECc. Support would be requested though the FMR in the ARCC. f. LAPD: Will send representatives to the ICP as conditions warrant g. CBP: Will send representatives to the ICP, ARCC, and DOC as conditions warrant h. TSA: May send representatives to the ICP and DOC as directed by the Federal Security Director (FSD) i. FBI: Will send representatives to the ICP if conditions warrant 12. Roles and Responsibilities a. IMTG: Participant in the UC Verify and monitor BHS/BIDS data exchange into BHS sort controllers Communicate with FMCD/MSD group to monitor BHS status Page 9 of 18 Template V.2.1 Revision Date:

10 Page 10 of 18 Provide updates to ARCC Duty Manager Participant in the UC Responsible for the coordination of non-public safety divisions, agencies, jurisdictions, airlines and/or other stakeholders that would be responding to the airport Expedite restoration of impacted airport operations through the UC Manage all requests for LAWA resources and assets necessary to mitigate the impact of the BHS/BIDS incident on airport operations DOC: o Provides the coordination of communications and resource management between the ICP and the ARCC, senior airport leadership, and external agencies, including any logistical resources and assets o requested by responding agencies Supports the UC by monitoring the situation, maintaining situational awareness among participants, making notifications, and coordinating resource mobilization and deployment c. LAFD: Provide support as necessary or requested d. APD: Coordinate response with public safety agencies including law enforcement, fire, emergency medical service (EMS), and others through the ICP Provide crowd and traffic control as needed Support terminal evacuation and repopulation efforts e. FMCD: Provide building information and other technical information as requested or as necessary f. MSD: Provide necessary and appropriate staff and equipment to carry out activities as directed g. Media and Public Relations: Monitor, receive, and respond to media, social media, and public inquiries regarding the BHS/BIDS system incident and its impact on airport operations h. LAWA ADA Coordinator: Communicate with Ops and DOD (as needed) to facilitate adequate care, communication, transportation, needs and conditions for those with disabilities in coordination with the responsible airline(s) i. CSRs/VIPs/ART: As paid staff, the CSRs will already be in terminals and the first line of support to customers, for their care and to assist any persons with disabilities that cannot be sufficiently aided by the service providers; ART members will be called in case of a large event requiring supplemental staffing j. LAPD: Respond to the ICP and establish a presence at the UC as necessary or requested Provide traffic mitigation surrounding LAX k. CBP: Ensure international border security, safety and control of passengers, baggage, and cargo Coordinate with LAWA Operations for TBIT tech and air carriers for baggage handling support Implementation of the COOP based on the assessment of diverting passengers and baggage to an alternate relocation site for CBP processing Ensure safety of passengers and employees Coordinate emergency procedures with internal and external partners: federal, state, and local law Page 10 of 18 Template V.2.1 Revision Date:

11 Page 11 of 18 enforcement; and emergency responders Recover from the incident and resume full functions of CBP/FIS l. TSA: Maintain screening operations if the affected terminal is still operational and implement Contingency Operations Relay current incident status to TSA Coordination Center 13. Priorities a. IMTG: Ensure BSMs are being received Ensure BSMs are being transmitted to BHS sort controllers Ensure BHS sort controllers are receiving and processing BSMs Coordinate deployment of resources adequate for the restoration of affected airport services during the course of the event To the extent possible, mitigate incident impact on airport operations Provide flight information to ICP for response considerations Provide timely coordination with ATC regarding the affected movement areas, via landline or other communications pathways, prior to the issuance of NOTAMs Expedite timely resumption of full operations ARCC: o Provide timely communication to all affected stakeholders and responding agencies via ARCC Notification System DOC: o Ensure the coordinated deployment of resources during the response to, operations at, and recovery from the incident c. LAFD: Assist airport operations as requested or as necessary d. APD: Protect life and property Provide security, crowd and traffic control Support terminal evacuation and repopulation efforts e. FMCD: Provide building and other technical information to IMTG as necessary or requested, also lighting & power f. MSD: Provide incident information to all FMCD/MSD supervisors and update as necessary Provide logistical support to the incident as requested (including barriers, signs, cones, and so forth) g. Media and Public Relations: Monitor, receive, and respond to media, social media, elected leader, airport stakeholder, and public inquiries regarding the BHS/BIDS system incident impact on the airport Coordinate with other ICP PIOs the development and dissemination of accurate and timely information to media and general public/traveling public h. LAWA ADA Coordinator: Request assistance and/or coordinate with Ops, the responsible airline(s) and City DOD in identifying and offering assistance to persons with disabilities Page 11 of 18 Template V.2.1 Revision Date:

LAPD: Protect life and property Ensure the constant flow of traffic in the surrounding area Assist with any investigation of the incident if determined to be of a suspicious nature (e.g. physical damage to equipment, cyber-terrorism) Determine if this is an isolated event or part of a larger coordinated criminal effort k. CBP: Ensure international border security, safety and control of passengers, baggage and cargo Ensure separation and segregation of passengers with baggage and immediate clearance Recover from the baggage handling system incident and resume full service of CBP activities l. TSA: Ensure safety of TSA personnel and the security of TSA-controlled areas Remove unscreened bags from public area to mitigate safety concerns 14. Goals and Objectives a. IMTG: Ensure 24/7 availability of BSM transmissions into the BHS Monitor system stability and availability of services, and communicate interface status with airport stakeholders Stabilize and contain the incident as quickly as possible Restore essential airport operations as efficiently as possible in coordination with the UC In coordination with IMTG, assess the impact of the system failure on airport facilities and operations DOC: o Ensure the coordinated deployment of resources during the response to, operations at, and recovery from the incident c. APD: Provide for the safety and security of persons and property Safely direct and control vehicular and pedestrian traffic Support terminal evacuation and repopulation efforts d. LAFD: Protect and save lives and property Support airport operations as necessary or requested e. FMCD/MSD/ADG/CPPG: Provide engineering support, assessments, technical expertise and recommendations for responders and the UC as necessary or requested Contract, coordinate, and supervise activities of contractors as necessary f. LAPD: Protect life and property Support UC objectives Monitor traffic conditions outside the airport Carry out law enforcement activities h. FBI: Provide investigative resources if requested i. CBP: Ensure international border security, safety and control of passengers and their baggage Ensure all international passengers are processed and cleared that are in possession of their luggage Process international passengers with their baggage Recover from the baggage handling system incident and resume full command and control functions of CBP activities j. TSA: Assign a TSA representative to the ICP as necessary or requested Maintain mission-essential functions 15. Response Considerations / Concept of Operations a. IMTG: Ensure BSMs are being received Ensure BSMs are being transmitted to BHS sort controllers Ensure BHS sort controllers are receiving and processing BSMs Participate in the UC Initiate ARCC Notifications and other means of communication to inform the airport stakeholders the BHS/BIDS system failure Provide current and projected airport flight information relevant to the incident/event Coordinate and deploy the staff and other resources as necessary or requested by the UC (e.g., BHS Operator and/or Netversant Representative, Airport Operations staff) Determine facilities involved in the BHS/BIDS system failure and its impact on airport operations as quickly as possible Communicate the BHS/BIDS system failure location to the DM Identify and verify impact on baggage handling system, air carrier services, piers, and ticket counter areas with FMCD/MSD, systems management, and Landside Request initiation of NOTAMs, ARCC Notification alerts, and other means of communication to inform ATC and other airport stakeholders of a dynamic event within 15 minutes of the incident For BHS/BIDS systems failure of greater than four hours with associated operational issues (e.g., flight delays, diversions, cancellations) notify the LA City Emergency Management Department (EMD) of the situation Notify and brief Media and Public Relations representative Schedule conference call within 30 minutes Page 13 of 18 Template V.2.1 Revision Date:

14 Page 14 of 18 Obtain resources for the incident as requested via ICP Plan, prepare, and execute periodic briefings to operations staff and other stakeholders as approved by the UC Communicate closures with ATC, DM, and/or airport stakeholders, as appropriate Develop a viable work-around to maintain operations and flow of passengers and air traffic Keep the areas of the airport that are not impacted operational In coordination with the ICP, restore essential airport operations as quickly as possible Facilitate the safe movement of aircraft and/or passengers around the impacted or closed area Provide current and projected airport traffic information relevant to the incident/event within 45 minutes of the start of the incident Assess the forecasted arrivals and departures at each terminal for the immediate next four hours Initiate recovery efforts per established plans and procedures and reassess every 30 min the foot print area and determine if the impact to adjacent operations can be further limited Upon report of infrastructure and systems readiness, restore airport to normal operations and ensure communication to all its stakeholders DM o Consider activation of DOC; notify the Manager on Duty (MOD) of intent to activate o o Obtain resources for incident as requested Ensure that the initial and subsequent ARCC notifications are sent to key stakeholders per established procedures ARCC (in coordination with DOC): o Provide flight information to the ICP and the Terminal Incident Recovery Center (TIRC), including operations schedules for the next four hours o Coordinate with ATC to keep unaffected terminals operational for aircraft parking assignments o Obtain resources and assets for incident as requested o Reassign airline operations to other gates and/or terminals as necessary, including sort piers, baggage carousels, and ticket counters o Provide updates as situation changes including, in coordination with TIRC, reassignment of arriving/departing aircraft to other gates and/or terminals, and reassignment of sort piers, baggage carousels, and ticket counters o Communicate with ATC via landline and other communications pathways, and issue NOTAMs as necessary o Report airport status to the MOD Landside: o Respond, identify and verify the scope of incident and notify ARCC/DOC with update o Establish an ICP o For a High Impact event, establish TIRC and notify DOC of its location for dissemination to impacted air carriers and systems management o Assist FMCD/MSD and systems management with identifying impacted baggage handling system issues o Coordinate alternative baggage handling process with affected air carrier o Establish contact with the Airport Guide II (Customer Service Representative [CSR] Supervisor) and Page 14 of 18 Template V.2.1 Revision Date:

15 Page 15 of 18 have them deploy CSRs to provide information and interpretation to meeters and greeters as necessary o Coordinate with ATC to keep unaffected terminals or Air Operations Area (AOA) operational Airside: o Assume duties and responsibilities of Landside as necessary or requested o Develop action plan for inbound/outbound operations on the AOA to alleviate congestion as necessary or requested o Provide additional staff for the ICP/TIRC as necessary or requested c. APD: Monitor and provide crowd and vehicular/pedestrian traffic control Establish/conduct security to coincide with incident activities (e.g. secondary threats and hazards) Support Airport Operations in the implementation of the Terminal Evacuation and Repopulation Plan as needed d. FMCD: Provide assistance to the ARCC (e.g., asset requests) e. MSD: Assign a representative to the ICP as necessary or requested Provide support as requested f. Airport Operations Needs: Provide the facilitation of the Terminal Evacuation and Repopulation Plan, and any necessary resources and assets for displaced travelers, emergency responders, and the public (e.g., potable water, food, restroom services, and shelter), as needed and/or if applicable. Identify additional space within the airport for the relocation, portering/storage of all the additional baggage from the impacted airlines and terminal during a BHS/BIDS failure Maps and other information (Baggage handling system diagrams) to assist in providing visual information as requested or necessary (electronic versions) c. APD Needs: Interoperable communications with all response agencies and other stakeholders that may respond to incidents within the airport 19. Recovery Considerations a. IMTG: Post-incident outage report (hot-wash) and discussion with key representatives of the response organizations that assisted during the outage Coordinate with stakeholders via the DOC and TIRC Division representation within the DOC shall assist in the production of a DOC Management Plan for recovery of critical services Coordinate terminal repopulation with other responding jurisdictions and stakeholders to bring the airport back to normal operations as soon as possible c. APD: Provide support of the Recovery Plan as developed by the DOC and the TIRC for return to normal operations d. FMCD/MSD: Provide engineering and technical support as necessary or requested for the recovery effort e. Media and Public Relations: Provide information to airport stakeholders, elected leaders, the public, social media and other media outlets concerning airport operations, return of services, and other information regarding the normalizing of the airport f. LAWA ADA: Coordinating with the responsible air carrier(s) for assisting persons with disabilities Helping persons with disabilities in recovering personal items such as: o Durable medical equipment (DME) o Consumable medical supplies (CMS) g. CSRs/VIPs/ART: Support passenger repopulation as directed by IC and support ADA Coordinator recovery efforts h. CBP: Ensure international border processing, security, safety and control of passengers and baggage. ADA: This Annex meets ADA response protocols that have been developed from actual events and from airport exercises involving individuals with disabilities, including mobility, blindness, and deafness. ADA protocols and procedures are regularly evaluated and exercised by LAWA s Office of the Coordinator for Disability Services, in coordination with Airport Operations, the Los Angeles Department on Disability, and disabled volunteer participants from the community. 21.
