ABOUT THE AUSTRALASIAN COLLEGE OF COSMETIC SURGERY Established in 1999, the Australasian College of Cosmetic Surgery (ACCS) is a not-for-profit, multi-disciplinary fellowship based body of general surgeons, cosmetic surgeons, plastic surgeons, maxillofacial surgeons, dermatologists, ear, nose and throat surgeons, ophthalmologists and other doctors and health care practitioners who practice in cosmetic medicine and surgery. The primary goal of the ACCS is to ensure the safe provision of cosmetic medicine and cosmetic surgical procedures to the Australian general community through the supply of appropriately trained and certified health care practitioners. The College also seeks to work cooperatively with government and other stakeholders to improve standards and safety and to educate health care consumers. Its Fellows and spokespeople are regularly quoted in the media and consulted by federal and state health and consumer regulators. The ACCS is the only medical college which provides education and training leading to fellowship specifically in cosmetic medicine and surgery. Fellows of the College are medical doctors who have completed post-graduate education and training and demonstrated competency specifically in cosmetic medicine and surgery. To become an ACCS Fellow, doctors must typically complete a minimum of 1 years of medical and surgical education and training. 16 TERMS & CONDITIONS By returning this signed Sponsorship Agreement 1 ( Agreement ), you agree to be a non-exclusive Sponsor or Exhibitor of Cosmetex 016 held at the Hilton Sydney from Wednesday, 11 May Saturday, 14 May 016 on the following terms and conditions set out below and as attached to this agreement. 4 5 Upon receipt of your signed agreement, acknowledgment of receipt will be sent in writing with a Tax Invoice. You agree to pay Cosmetex 016 the full amount payable Sponsorship entitlements will not commence until 50% of the total amount is received with this booking form. If a sponsor wishes to cancel or change the details of their booking, advice must be made in writing to sponsorship@cosmetex.org. The cancellation fees are applicable as follows: Before Thursday, 4 February 016, 50% of the total amount After Thursday, 4 February 016, 100% of the total amount 6 Sponsors and exhibitors contracted after Wendesday, 1 April 016, will be charged a 15% surcharge. 7All cheques are to be made payable to Cosmetex 016. EFT payments may incur additional fees and charges by the respective bank and these are the sole and exclusive responsibility of the individual or Company entering into this agreement. If paying by EFT, remittance advice must be sent to sponsorship@cosmetex.org. 8All amounts stipulated in this agreement include goods and services tax (GST) of 10%. 9 Please note your requested category of sponsorship may have a limit to the number of sponsors as specified in the attached documentation and preference will be determined in order of receipt of the signed Agreement and deposit payment. The Conference Organisers will notify you if you are unable to participate in your requested category. 10 Sponsors are not permitted to enter the Conference sessions or social events, unless the relevant tickets are offered as part of the particular sponsorship package. However, should sponsors wish to attend the Conference as a delegate, the appropriate registration form must be completed and forwarded to the Conference Organisers together with the applicable registration fee. Exhibition booths contain three perimeter walls, unless 11 a corner booth is purchased. All perimeter walls of exhibition booths shown on the floor plan will remain in place if adjoining booths are not sold. Booths cannot exceed a height of.5 metres. 1 The Conference Organisers may at their discretion use your logo, symbol, trade mark or registered mark in acknowledging your sponsorship in the Conference literature where this is offered as part of your particular sponsorship package and no payment shall be made to you for such use. You will be responsible for forwarding your logo, symbol, trade mark or registered mark by the deadline. We request that all logos be supplied in an EPS format, high resolution 00 dpi.
