Harrisburg, OR CCB #64090 PH FX rex@atezinc.com Website February 8, 2016 Hazardous Building Materials Survey of the Former Willamette Industries Site in Sweet Home, Oregon CLIENT: Mr. Joey Hickey Geosyntec Consultants 621 SW Morrison Street, Suite 600 Portland, Oregon / jhickey@geosyntec.com PROJECT SITE: Former Willamette Industries Mill Site 2210 Tamarack Street Sweet Home, Oregon EXECTUIVE SUMMARY ATEZ Inc. was contracted by Geosyntec to provide a Hazardous Building Material (HBM) survey of the former Willamette Industries Mill site in Sweet Home, Oregon. The scope of the survey included asbestos-containing building materials (ACBM), lead in paint (LBP), and electronic components that may affect renovation or demolition of at the site. The survey included the Office Buildings of the former mill, the Family Pallet mill and offices, and the remaining structures on the mill site. Several of the structures have been destroyed due to fire. Although an inspection of these structures was included in the survey, no further demolition was included in the investigation. The water tower and various debris piles scattered about the mill site were not included in the scope of this investigation. Waste characterization for lead was not included in the scope of this investigation. SURVEY NARRATIVE The study areas were broken into three sections for both the investigation and reporting: The office building; the Family Pallet Mill & Offices and the remaining structure on the mill site. Both asbestos contain materials (ACM) and lead-based paint (LPB) were discovered in each of the study areas. Poly Chlorinated Biphenyl (PCB)-containing fluorescent light ballasts were identified in both the office building and the pallet mill offices. 1

4 Former Willamette Industries Mill Site, Sweet Home, Oregon SURVEY NARRATIVE cont d Methodology: All of the inspection work for both the lead and asbestos was conducted by personnel with a Housing & Urban Development (HUD) Inspector/Risk Assessor and Asbestos Hazard Reauthorization Act (AHERA) Building Inspector accreditation. Asbestos samples were shipped under a chain-of-custody to an NVLAP accredited laboratory. The lead portion of the inspection was conducted with a NITON XRF (X-ray Fluorescence) detector. Detailed survey information is presented in each section with the location, material sampled and quantities of positive materials indicated. In addition, the friability or non-friability of the positive ACM materials was noted. Office Building: Sixteen hundred square feet of asbestos-containing floor tile were identified under carpeting in this building. The underlying mastic tested negative and the tile is glued to a mix of particleboard and plywood substrates. The provided survey drawing shows the extent of this material. Forty linear feet of asbestos-containing covebase mastic and window glazing compound was identified on eight divided light windows. Although limited destructive sampling was performed to identify materials that may be concealed above ceilings, within walls and below floors, it is possible that additional suspect materials may be encountered during demolition or renovation activities. If Identified, these materials should be sampled and analyzed to determine if these materials are ACM before disturbing them. No LBP was identified on the interior of this building. On the exterior mixed results were obtained. Fascia boards, windows, doors, and various architectural trim all tested above the HUD standard for LBP of 1.0 μg/cm^2 depending upon which side of the building was tested. ATEZ recommends that all of the components be treated as lead-containing for the purposes of renovation or demolition activities in this building. Twenty-one PCB (Poly Chlorinated Bi-phenyl) containing fluorescent light ballasts were identified in this building. The determination was based on the light ballast not being labelled as PCB free. In addition many of the ballast covers showed evidence of staining characteristic of leaking ballasts. Given the age of the building and the presence of PCB-containing ballasts the stained covers should be considered contaminated when they are encountered. Family Pallet Mill & Offices: One-thousand seventy-five square feet of ACM floor tile and eight-hundred-twenty-five square feet of ACM floor tile mastic are present in the office building for Family Pallet. The floor tile is present in all rooms except one room. The tile is under carpet in two rooms and the building is constructed on a concrete slab. All the divided light windows should be considered to have ACM glazing compound even though the results are mixed since it is impossible to differentiate ACM from non-acm glazing compound by visual inspection. There are a total of fifteen windows and nine doors in the building. 2

5 Former Willamette Industries Mill Site, Sweet Home, Oregon SURVEY NARRATIVE cont d The joint-compound on the gypsum wallboard in the bathroom tested positive for asbestos, however EPA considers this a homogeneous layered material and the wallboard system tested at less than one percent asbestos (trace) overall. This means it is not an ACM, however, there are OSHA regulations that apply to the workers disturbing trace materials. LBP was encountered on windows, doors, frames and trim on both the interior and exterior of this building. Exterior architectural components like fascia boards and entry ceiling all had enough lead to be considered LPB. In the mill portion of this site asbestos was identified on twenty-one divided light windows and LBP was present on both the interior and exterior of the building. The offices and mill portion of this part of the site is in use as a pallet mill. As a result only limited destructive investigation was conducted to identify concealed materials. If additional suspect materials are encountered during renovation or demolition activities, these materials should be sampled prior to disturbing them. Fluorescent lights are present in the office building and mercury vapor lights are present in the mill. At the time of the inspection the mill was in use and an inspection for PCB content was not feasible. No other suspect building materials were observed. Mill Site: Significant amounts of asbestos-containing roofing debris were identified across the mill site, with the highest concentrations on the west end of the mill site. Although some of the roofing debris samples did test negative for asbestos, it is not possible to differentiate what debris is ACM by visual inspection. ATEZ recommends that all roofing debris on the mill site be treated as ACM. Smaller amounts of roofing debris were identified in the large burned building east of Building 5. Although the vast majority of the building debris is non-suspect such as charred wood and metal, removal of the building debris should be conducted under the supervision of personnel trained to identify suspect material and the authority to ensure the proper handling of areas which may contain ACM. Roofing of building 4, 5, & 6 all tested positive for asbestos and is the likely source of roofing debris noted above. While silver roof paint was noted and sampled during the inspection, the laboratory identified the asphalt roofing as having asbestos and not the silver roof paint. ACM floor tile was identified in a small building on the east end of the log deck. The building is 200 square feet and the tile is present throughout. In addition, vermiculite attic insulation is present in the building both in the attic and as debris on the floor and surrounding area. The vermiculite tested positive for the presence of asbestos. This building has moderate fire damage, and is identified as Building 9 on the drawings. 3

The results were not consistent by color or application. As a result all of the paint on the mill site should be treated as being lead containing. No electrical components or other building materials were identified on the mill site as being of concern for removal or disposal. Pole mounted transformers are not considered building materials and were not included in this investigation. The cost estimates assume the project will be contracted with each section (Offices, Pallet Mill & Offices, and the Mill Site) being a separate project. ACM Debris clean up in the area of the burned building is separate from the removal of the burned building remains. For the purposes of this cost estimate it is assume that the asbestos abatement contractor will work closely with the demolition contractor that is removing the building. The roof of buildings 4, 5, & 6 are not safe for workers to be on. To remove the roofing and the buildings will have to demolished in order for the abatement work to proceed. The cost estimate assumes that buildings are on the ground and does include the removal of the building remains. The cost estimates do include the cost for labor, materials, and disposal, and are based on similar projects ATEZ has completed. Use of the cost estimate is limited to the client for the purposes of project planning and should not be considered a bid by ATEZ. The cost estimates assumes the work is competitively bid at current prevailing wage rates with an adequate construction period to avoid overtime. No provision for LBP abatement is included as it is not required for demolition of the structures. Additional disposal costs for LBP cannot be calculated without conducting a waste characterization to determine whether the waste stream meets the definition of Hazardous Waste under the Resources Conservation and Recovery Act (RCRA). Waste characterization was not included in the scope of this investigation. 5
