PREVAILING WAGES The Director of Industrial Relations has determined the general prevailing rate of per diem wages in the locality in which this work is to be performed, copies of which are on online at The successful Bidder shall comply with all applicable Labor Code provisions, including but not limited to, hours of labor, employment of apprentices and debarment of contractors. BID FORM All Proposals shall be made upon the Lump Sum Bid Form, attached hereto as Exhibit B and incorporated herein by this reference. Bidder may submit supporting documents which outline the final cost of the Lump Sum Bid (e.g., line item costs, etc.). Proposals submitted without a completed Lump Sum Bid Form will be disregarded. The Bidder shall fill out all blanks in the form as therein required \

Proposals shall be valid for 90 calendar days after the time specified above. AWARD OF CONTRACT The District shall award the Contract for the Project to the Bid that is deemed most advantageous to the District. The Board of Directors of the Cosumnes Community Services District reserves the right to reject any or all proposal and to waive any informality in any Proposal. INSURANCE Contracted Bidder will be required to provide an insurance certificate naming the CSD as additionally insured. Contractor must provide certificate showing active policies of automobile liability, commercial general liability and property damage insurance from an insurance company authorized to be in business in the State of California. Each such policy shall be in an amount of not less than One Million Dollars ($1,000,000) for each occurrence. The following statement must appear on the certificate: "Additional insured endorsement names the Cosumnes Community Services District its directors, agents or employees are included as additional insured." CONTRACTOR REGISTRATION Pursuant to Labor Code sections and , all contractors and subcontractors that wish to bid on, be listed in a bid proposal, or enter into a contract to perform public work must be registered with the Department of Industrial Relations. No bid will be accepted nor any contract entered into without proof of the contractor s and subcontractors current registration with the Department of Industrial Relations to perform public work. If awarded a Contract, the Bidder and its subcontractors, of any tier, shall maintain active registration with the Department of Industrial Relations for the duration of the Project. Complete and submit Exhibit C with Proposal Package. PAYMENT The prices paid for the various items in the bid proposal shall include full compensation for furnishing all labor, materials, tools, equipment, and incidentals necessary to complete the finished product as provided in these specifications. Compensation in full for items specified as "Lump Sum" in the bid proposal shall include payment for furnishing all labor, materials, tools, equipment, and incidentals necessary to complete the Services. Whenever it is specified that the Contractor is to do work or furnish materials of any class for which no price is fixed in the bid proposal, it shall be understood that the Contractor is to do such work or furnish such materials without extra charge or allowance or direct payment of any kind. The cost of doing such work or furnishing such materials is to be included in the price bid for such other items of work, as the Contractor may consider appropriate. The Cosumnes Community Services District (CSD) is an independent local government entity and a political subdivision of the State of California, established under Government Code Sections and 56439(c). The District provides emergency medical and fire protection services to the cities of Elk Grove and Galt and to unincorporated areas in south Sacramento County, and provides parks and recreation services to the Elk Grove Community. The District encompasses roughly 157 square miles and serves an estimated population of 174,400. The CSD is dedicated to the principles of competition and fairness. The District is committed to ensuring that local taxpayers receive the maximum value for every District expenditure. Toward that goal, we encourage qualified vendors to participate in doing business with the CSD.

Protect any areas and structures which are not to be coated. 2. Courts must be cleaned using a stiff bristle broom and gas powered blower or water based pressure spray unit capable of generating 2500 psi at the nozzle tip, to remove all dirt and debris. 3. There must be an effort to eliminate all birdbaths prior to applying NOVASURFACE. CSD staff will identify which birdbaths, if any, need to be corrected using a patch mix of NOVABOND, 50-mesh sand and Portland cement, as per manufacturer s directions. Depressions must be primed with a 50% dilution of NOVABOND and water prior to patching. 4. Holes, cracks and spalled areas including outside the fence of tennis courts shall be sanded down or ground flush, cleaned of dirt, water, and other deleterious materials and corrected with proper repair materials such as NOVACAULK II or NOVABOND. Application shall proceed
4 M106 Sport Court Resurfacing ; page 4 EXHIBIT A PAGE 2 OF 8 COURT RESURFACING PROJECT Application: only if the surface is dry and clean and the temperature is at least fifty degrees (50 F) and rising and the surface temperature is not in excess of one hundred forty degrees (140 F). 1. New asphalt pavement shall cure for 14 days prior to application of any surfacing materials. 2. Contractors must notify the District Inspector of all applications, 48 hours prior to installation. The surface to be coated shall be inspected and made sure to be free of grease, oil, dust, dirt and other foreign matter before starting work. 3. The surface shall be flooded. Any ponding water remaining that is deep enough to cover the thickness of a five-cent piece shall be corrected using a patch mix consisting of NOVABOND, 50-mesh sand and Portland cement, as per manufacturer s directions. Depressions must be primed with a 50% dilution of NOVABOND and water prior to patching. 4. Application shall proceed only if the surface is dry and clean and the temperature is at least fifty degrees (50 F) and rising, and the surface temperature is not in excess of one hundred forty degrees (140 F). Do not apply coatings when rain is imminent. 5. Each coat in this system must dry completely before next application. Between each coat, inspect entire surface. Any defects should be repaired. Scrape surface to remove any lumps, and broom or blow off all loose matter. 6. Using a neoprene rubber squeegee, apply one (1) coat of NOVASURFACE acrylic resurfacer, diluted with one (1) part clean water to two (2) parts NOVASURFACE. Clean, bagged sand shall be incorporated into the diluted NOVASURFACE at the rate of five (5) to ten (10) Lbs. per gallon. Sand gradation shall be 50 to 60-mesh. Allow application to dry thoroughly. 7. Using a neoprene rubber squeegee, apply two (2) coats of NOVAPLAY (colors to match existing). Allow each application to dry thoroughly. A small (not to exceed 8 fl. oz. per gallon) quantity of water may be used in diluting these coatings, only if coatings are drying too rapidly. Permission of the owner shall be obtained before adding additional water. Line Marking: 1. Upon completion and acceptance of the surface, the contractor shall prepare and repaint lines on the court to meet the original layout and color. 2. All lines are to be applied by painting between masking tape with a paintbrush or roller. 3. Prime masked lines with Seal-A-Line. Allow application to dry. 4. Paint lines with NOVATEX textured line paint. Allow application to dry. 5. Remove masking tape immediately after lines are dry. Completion: Upon completion, contractor must remove all construction debris, surplus materials, empty containers and wash water, and must leave the site in a condition acceptable to CSD Inspector. Guarantee: Contractor must guarantee straight lines on each court.

CSD staff has already identified birdbaths (if any) on all courts. It is the responsibility of the Contractor to visit each site to include the identified birdbaths within their bid. BID PRICING: 1. Contractor shall provide a lump sum price on the Exhibit B Bid Form for each sport court site. 2. Contractor court bid price shall include: a. Prevailing wage. b. Filling all birdbaths identified by CSD staff. c. All resurfacing steps outlined in the Scope of Work. d. All materials, temporary fencing, supplies, labor, labor supervision, tools and equipment necessary to complete Scope of Work. SCHEDULE OF SERVICE: 1. Contractor will work with CSD staff for exact schedule. 2. All courts must be completed by October 31, 2016 (weather permitting). PARK SITES:
