1 SECTION INSPECTIONS AND PROCEDURES PART 1 GENERAL 1.13 INTENT AND CONDITIONS A. Intent 1. Define and coordinate structural testing and special inspection services. 2. Provide confidence that the specified Work is constructed in compliance with the contract documents and the intent of applicable codes including Sections 106 and 1704 of the 2000 International Building Code (IBC) as adopted by the current Minnesota State Building Code. 3. Structural testing and special inspection services are intended to assist in determining probable compliance of the Work with requirements specified. These services do not relieve the Contractor of responsibility for compliance with the requirements of the Contract Documents. B. Conditions 1. If inspection of a fabricator's work is required, the Owner or Architect may require testing and inspection of the Work at the plant, before shipment. Owner and Architect reserve the right to reject material not complying with the Contract Documents. 2. Refer to individual technical specification sections for specific qualifications, inspections, tests, frequency and standards required. Testing and inspection shall be performed in accordance with the referenced standard for the specific material or procedure unless other criteria are specified. In the absence of a referenced standard, tests shall be performed in accordance with generally accepted industry standards. 3. Work shall be checked as it progresses. Failure to detect any defective Work or materials shall not prevent later rejection if defective Work or materials are discovered, nor shall it obligate Owner to accept such Work. 4. Structural testing, special inspection, and periodic inspections by the Building Official do not preclude the normal field involvement and site observations by Architect or SER. 5. Structural testing, special inspection, and periodic inspections by the Building Official do not relieve the Contractor of any responsibility to complete the Work in accordance with the approved Drawings and Specifications. 6. Testing agents and/or Special Inspectors may not waive or alter Contract Document requirements, or approve, accept, or reject any portion of the Work unless specifically authorized by the Architect or SER. They may not assume any duties of the Contractor, and they have no authority to stop or reject Work RELATED REQUIREMENTS A. Drawings and general provisions of the Contract, including Division 1 Specifications apply to this Section. 07/17/

2 1.15 DEFINITIONS A. Testing: Evaluation of systems, primarily requiring physical manipulation and analysis of materials, in accordance with approved standards. B. Inspection: Evaluation of systems, primarily requiring observation and judgment. C. Structural Special Inspections: Structural special inspections include inspections of structural items required by the 2000 IBC Section 1704, as adopted by the 2003 Minnesota State Building Code, and other items, which in the professional judgment of the Structural Engineer of Record, are critical to the integrity of the building structure and are indicated to be performed under the requirements of this Section. They do not include special inspections for non-structural items such as fireproofing and smoke control systems. D. Structural Testing: Structural testing includes those tests of structural items required by the 2000 IBC, as adopted by the 2003 Minnesota State Building Code, or its referenced standards, and other tests, which in the professional judgment of the Structural Engineer of Record, are critical to the integrity of the building structure and are indicated to be performed under the requirements of this Section. E. Architect of Record: The prime consultant in charge of overall design and coordination of the Project. F. Structural Engineer of Record (SER): The licensed professional engineer in responsible charge of the structural design for the Project. G. Licensed Structural Engineer: A professional engineer with education and experience in the design of structures similar to this Project licensed to practice in the state in which the Project is located. H. Testing Agency (TA): The properly qualified firm performing testing services. I. Special Inspector (SI): A properly qualified individual or firm performing special inspections. J. Building Official: The officer or their duly authorized representative charged with the administration and enforcement of the building code for the Project REFERENCES A. 3 1.17 QUALIFICATIONS A. Testing Agency: An approved independent testing agency acceptable to the Owner, Architect, and SER and meeting the following: 1. Authorized to operate in the State in which the Project is located and experienced with the requirements and testing methods specified in the Contract Documents. 2. Meet applicable requirements of references stated in paragraph Have available testing equipment that is calibrated, at reasonable intervals, by devices of accuracy traceable to either the National Bureau of Standards, or to accepted values of natural physical constants. 4. Provide individuals performing tests and taking samples with appropriate certifications for Work performed. B. Special Inspector: Either an appropriately certified inspector or a civil/structural engineer performing under the direct supervision of a licensed civil/structural engineer (as defined earlier in this Section) and acceptable to the SER and Building Official. Unique Special Inspector requirements, for specific materials and systems, are noted in related technical Specification Sections RESPONSIBILITIES A. Special Inspectors: 1. Inspect the Work assigned for conformance with the building department approved Drawings, Specifications, and applicable material and workmanship provisions of the code. Perform inspection in a timely manner to avoid delay of Work. 2. Bring nonconforming items to the immediate attention of the Contractor for correction. If not corrected within 24 hours or if Inspector will not be on site the following day, bring to the attention of the SER by the end of the business day. If uncorrected after a reasonable period of time, bring to the attention of the Building Official, the Architect and the Owner. Notify SER immediately if non-conforming items are enclosed, embedded, or obscured prior to verification of correction. 3. Submit inspection reports to the Building Official, Contractor, the Architect, the SER, the Owner and other designated persons in accordance with the Structural Testing and Special Inspection Schedule. 4. Submit a final signed report stating whether the Work requiring special inspection was, to the best of his/her knowledge, in conformance with the approved Drawings, Specifications and the applicable workmanship provisions of the code. 5. Sign the Structural Testing and Special Inspection Schedule in conjunction with other responsible parties. 6. Attend Preconstruction meeting to review scope of special inspections. B. Testing Agency: 1. Test the Work assigned for conformance with the building department approved Drawings, Specifications, and applicable material provisions of the documents. Perform tests in a timely manner to avoid delay of Work. 07/17/

Submit test reports to the Building Official, Contractor, the Architect, the SER, the Owner, and other designated persons in accordance with the Structural Testing and Special Inspection Schedule. 3. Sign the Structural Testing and Special Inspection schedule in conjunction with other responsible parties. 4. Attend a Preconstruction meeting to review scope of structural testing. C. Contractor: 1. Attend a Preconstruction meeting to review scope of structural testing and special inspection. 2. Schedule and coordinate Preconstruction and Construction testing and inspections. 3. Post or make available the Structural Testing and Special Inspection Schedule within its office at the job site. Also, provide adequate notification to those parties designated on the Schedule so they may properly prepare for and schedule their work. 4. Provide Special Inspectors access to the approved Drawings and Specifications at the job site. 5. Review all reports issued by Special Inspectors. 6. Retain, at the job site, all reports submitted by the Special Inspectors for review on the Building Official's request. 7. Correct deficiencies identified in inspection or testing reports in a timely manner. 8. Provide safe access to the Work requiring inspection or testing. 9. Provide labor and facilities to provide access to the Work, to obtain, handle and deliver samples, to facilitate testing and inspection and for storage and curing of test samples. 10. Verify conformance of the Work with specified construction tolerances. 11. Immediately remove all rejected materials from the site. Do not incorporate any rejected materials into the Work. 12. Inspections by Building Official: Provide adequate notice for inspections performed by the building official, as required by IBC Section 109, the Minnesota State Building Code, and local ordinances. 13. Sign the Structural Testing and Special Inspection Schedule in conjunction with other responsible parties prior to commencing construction. D. Fabricator: 1. Submit a Certificate of Compliance to the Building Official, Special Inspector, and SER that the Work was performed in accordance with the approved Drawings and Specifications. 2. Sign the Structural Testing and Special Inspection Schedule in conjunction with other responsible parties prior to commencing construction. E. Owner: 07/17/

5 1. Establish direct funding to provide for cost of structural testing and special inspection services. 2. Provide Special Inspector with approved Drawings, Specifications and approved shop drawings. 3. Provide Special Inspectors and testing agencies with full access to the site at all times. 4. Sign the Structural Testing and Special Inspection Schedule in conjunction with other responsible parties PAYMENT A. The Owner will employ and pay for services of the Special Inspectors and testing agency to perform required structural testing and special inspections as mandated by code. B. The Contractor shall provide and pay for all materials, samples, mock-ups, and assemblies required for testing and inspection and shall pay for shipping costs related to delivery of such items to the Owner s testing agency testing facility. C. If items requiring testing or inspection are enclosed, embedded or obscured prior to testing or inspection or if such items are placed without tests or inspections, the Contractor shall pay for the costs of any exploratory work deemed necessary by the Architect/SER to verify compliance with the Contract Documents. D. When any testing or observations indicate the Work is non-compliant with the Contract Documents, all retesting and re-observations shall be performed by the Owner s testing or observation agencies. All costs for retesting and re-observations, including additional services of the design professionals, the design professional s consultants and the Owner s consultants are the Contractor s responsibility and shall be deducted from the Contract Sum by Change Order INSPECTION NOTICE A. Contractor shall provide minimum of 5 working days prior to starting work and 3 working days prior to continuing work for all items requiring testing or inspection. Items requiring testing and inspection services prior to or during placement shall not be placed until testing and inspection services are available. Items requiring testing and inspection services after placement shall not be enclosed or obscured until testing and inspection services are performed. If the Work is covered up prior to any required testing or observation, it shall be uncovered for review at the Contractor s expense REPORTS A. Testing agency and Special Inspectors shall submit reports for structural testing and special inspection in a timely manner to the Owner, Contractor, Building Official, SER, and Architect of Record. Provide reports of daily activities to the SER and Contractor. Submit reports to the Contractor on a daily basis and to the SER on a daily or weekly basis. Provide summary reports to the Owner, Building Official and Architect on a monthly basis unless they request otherwise. B. Provide reports for ongoing work, containing the following information: 1. Date issued. 2. Project title and number. 07/17/

6 3. Firm name and address. 4. Name and signature of tester or inspector. 5. Date and time of sampling, test, or inspection. 6. Identification of product and Specification Section. 7. Location in Project, including elevations, grid locations and details. 8. Type of test or inspection. 9. Whether test specimens, test results or observations indicate compliance with Contract Documents. Specifically state any discrepancies 10. Types and locations of discrepancies found in the Work 11. Work required to correct discrepancies and Work performed to correct previously noted discrepancies. Discrepancies corrected during an inspection need not be reported. 12. Submit certified final special inspection report stating that, to the best of the Special Inspector's knowledge, the Work requiring special inspection conformed to the Contract Documents FREQUENCY OF TESTING AND INSPECTION A. For detailed requirements, see individual technical Specification Sections and the Structural Testing and Special Inspection Schedule, copy included after this Section PROTECTION AND REPAIR A. Upon completion of testing, sample-taking, or inspection, the Contractor shall repair damaged Work and restore substrates and finishes to eliminate deficiencies, including deficiencies in the visual qualities of exposed surfaces, as judged solely by the Architect/SER. Protect Work exposed by or for testing and/or inspection and protect repaired work. Repair and protection is the Contractor's responsibility, regardless of the assignment of responsibility for testing and/or inspection TESTS TO DEMONSTRATE QUALIFICATION A. Any tests required to qualify the Contractor or the workers for any phase of the Work, shall be performed at no additional cost to the Owner. B. If the Contractor proposes a product material, method, or other system that has not been pre-qualified, the Architect/SER may require applicable tests to establish a basis for acceptance or rejection. The Contractor shall pay for these tests. C. The Architect/Engineer of Record reserves the right to require certification or other proof that the system proposed is in compliance with specified tests, criteria or standards. A representative of an independent testing agency shall sign the certificate STRUCTURAL TESTING AND SPECIAL INSPECTION SCHEDULE A. The parties involved shall complete and sign the Structural Testing and Special Inspection Schedule. Schedule to be complete at time of permit issuance. 07/17/

The completed Schedule is an element of the Contract Documents and after permit issuance, becomes part of the building department approved Drawings and Specifications PART 2 PRODUCTS Not Used PART 3 EXECUTION 3.01 QUALIFICATIONS A. Minimum category of special inspector required to perform services outlined below are noted by qualifications in parentheses EARTHWORK (SECTION ) (TECHNICAL II) A. Determine site has been prepared in accordance with approved soils report and Section Earthwork prior to placement of fill materials. B. Verify fill materials used, maximum lift thickness placed and compaction obtained comply with requirements specified in Section C. Classify materials used and encountered during construction per ASTM D2488 and ASTM D2487. D. Perform field density testing and laboratory testing of materials at frequency indicated in Section E. Observe all subgrades/excavation bases below footings and slabs and verify design bearing capacity is achieved. F. Document presence of groundwater within excavations. G. Verify cut and fill slopes as specified in Contract Documents. H. Conventional Testing and Inspections Requirements: 1. Verify footings comply with frost depth requirements and report any variances to SER in a timely manner. I. Provide reports of subgrade observations and periodic test results for general compliance with Contract Documents and Geotechnical Report CONCRETE FORM WORK (SECTION ) A. Verify formwork for all concrete, excluding slabs on grade, strip footings without transverse reinforcement and topping slabs, will result in member size, location and configuration as described on the contract documents, only as it affects the structural integrity of the concrete elements to be placed. (Technical II) 3.04 CONCRETE REINFORCEMENT (SECTION ) 07/17/

8 A. Inspect reinforcement in all cast-in-place concrete, excluding slabs on grade, footings without transverse reinforcement, and topping slabs. (Structural I) 1. Verify reinforcing bar grade. 2. Verify reinforcing bars are free of dirt, excessive rust and damage. 3. Verify reinforcing bars are adequately tied, chaired and supported to prevent displacement during concrete placement. 4. Verify proper clear distances between bars and to surfaces of concrete. 5. Verify reinforcing bar size and placement. 6. Verify bar laps for proper length and stagger and bar bends for minimum diameter, slope and length. 7. Verify mechanical splices are placed in accordance with Contract Documents and reviewed shop drawings. 8. Verify epoxy coating is present at locations noted on the Contract Documents, include tie wires, chairs, bolsters, etc. Verify coating damage is repaired in accordance with the Contract Documents CAST-IN-PLACE CONCRETE (SECTION ) A. Sample and test all cast-in-place concrete. (Technical I) 1. Make, cure and determine strength of concrete test cylinders cast in field. Perform in accordance with ASTM C172 Standard Practice for Sampling Freshly Mixed Concrete, ASTM C31 - Practice for Making and Curing Concrete Test Specimens in the Field and ASTM C39 Standard Test Method for Compressive Strength of Cylindrical Concrete Specimens. Evaluation and acceptance of concrete shall be in accordance with ACI 318 with following exceptions: a. Make 1 set of cylinders for each day's operation and each type of concrete where less than 50 cubic yards is placed, plus additional set for each 50 cubic yards (or fraction thereof) over and above first 50 cubic yards of each type. Test sets shall consist of 4 cylinders with fourth cylinder to be field-cured specimen. For slabs placed when temperatures are expected to fall below 32 degrees F within 72 hours of placement cast in place (pop out) cylinders shall be used. These cylinders shall be placed close to corners and perimeter of pour. b. Store field-cured cylinder as near as possible to location of concrete represented by sample and give cylinder, insofar as practicable, same protection and curing as adjacent concrete. Keep other 3 cylinders covered with plastic or wet burlap and in degrees F. temperature range for 24 hours, allowing no injury to cylinders. After this period, and prior to age of 48 hours, deliver 3 cylinders to laboratory for additional curing, taking care not to freeze, crack or damage specimens. c. Deliver field-cured cylinder to laboratory at 28 days of age for testing to check adequacy of curing and protection as described in ACI 318. d. Test other 3 cylinders, laboratory cured, as follows: 1 at 7 days of age for projecting probable 28 day strength and 2 cylinders at 28 days for acceptance of average strength as described in ACI 318. e. If additional field cured specimens are required to verify early strength of concrete, contractor must pay for additional testing. 2. Slump: Determine slump of concrete in accordance with ASTM C143 Standard Test Method for Slump of Hydraulic Cement Concrete. Perform 1 test for each set of test cylinders. Test at point of discharge for each truck. Test before and after high range water reducer is added on site, if applicable. 07/17/

9 3. Air Content: Determine air content of fresh concrete, when air content is specified, in accordance with ASTM C173 or ASTM C231. Where placement is by pump, air content shall be measured at location of placement. For concrete exposed to freezing and thawing, concrete from each truck shall be tested and concrete not meeting specified percentages shall not be placed. For interior concrete not exposed to freezing and thawing, such as lightweight concrete on metal decking, perform 1 test for each set of test cylinders. Concrete used in performing air content test shall not be used in fabricating test specimens. 4. Unit Weight: Make, transport, and cure specimens as required to determine unit weight of structural lightweight concrete in accordance with ASTM C567 Standard Test Method for Unit Weight of Structural Lightweight Concrete and ASTM C138 Standard Test Method for Density (Unit Weight), Yield and Air Content (Gravimetric) of Concrete. Perform one test for each day's operation where less than 50 cubic yards is placed, plus additional set for each 50 cubic yards (or fraction thereof) over and above first 50 cubic yards. 5. Concrete Temperature: Test concrete temperature hourly when air temperature is 40?F and below and when 80?F and above, and each time a set of test cylinders is made. 6. Mark each test cylinder with job name, Contractor's name, mix number, date, location of pour and measured slump. In addition, mark measured air content when air-entraining admixture is specified. 7. Submit copies of test results to Owner, Architect and Contractor as soon as practicable after they are made. B. Perform concrete mix verification. (Technical I) 1. Verify mixer truck trip ticket conforms to approved mix design. 2. Verify that total water added to mix on site does not exceed that allowed by concrete mix design. 3. Verify that concrete quality is indicative of adequate mixing time, consistency and relevant time limits (discharge within 60 minutes of batching for air entrained concrete and 90 minutes for non air-entrained concrete.) C. Batch plant inspection (Technical I) 1. Verify batching tolerance is not exceeded (random basis). D. Floor Flatness and Levelness (Technical II) 1. Test compliance with specified values per ASTM E1155. E. Inspect preparation and placement of all concrete, excluding strip footings without transverse reinforcement. (Technical I) 1. Verify acceptable general condition of concrete base prior to placement. 2. Verify that concrete conveyance and depositing avoids segregation and contamination. 3. Verify that concrete is properly consolidated. 4. Verify reinforcement remains at proper location. 07/17/

10 5. Verify underfloor vapor barrier/retarder is properly installed and not damaged during concrete placement. F. Observe protection and curing methods for all concrete, excluding strip footings without transverse reinforcement. (Technical II) 1. Verify specified curing procedures are followed. 2. Verify that specified hot and cold weather procedures are followed. G. Inspect all embedded anchors installed in concrete. (Technical II) 1. Verify specified size, type, spacing, configuration, embedment and quantity. 2. Verify proper concrete placement and consolidation around all embedded anchors. H. Testing Laboratory shall have CCRL certification at the National Bureau of Standards MASONRY (SECTION ) A. Special inspection of masonry is required during preparation of masonry wall prisms, sampling and placing of masonry units, placement of structural reinforcement, cleanout of grout space immediately prior to closing of elements and during all grouting operations. B. "Continuous" Inspection of Masonry: Inspections noted below as being continuous shall be performed uninterrupted each day while the specific task is being performed. 1. Continuous inspection shall be provided for 100% of shear walls, masonry beams and masonry columns. C. "Periodic" inspection of masonry: Inspection items noted below as being periodic shall be performed at least once per 1,000 square feet of surface but not less than once per week. D. Samples and Tests for Special Inspections: (Technical II) 1. Construction Testing Frequency: One set of tests for each 5000 sq. ft. of wall area or portion thereof, but not less than one set for the project. a. One set shall consist of testing three samples. 2. Masonry Unit Tests Perform in accordance with IBC Chapter 21. a. For each type of unit provided, verify units conform to strength, absorption, and unit weight requirements of ASTM C 55 or ASTM C 90 when tested in accordance with ASTM C Masonry Prism Tests Perform in accordance with IBC Chapter 21. a. For each type of construction provided, verify compressive strength of masonry equals or exceeds specified f m when tested in accordance with ASTM C b. Preparation, storage, handling of prism tests. (Contractor will provide labor and materials to construct all prism tests.) 07/17/

11 4. Mortar Tests for Unit Masonry Complying with ASTM C270 Proportion Specifications: a. For each mix provided, test mortar for initial consistency and board life, mortar aggregate ratio, and mortar air content in accordance with ASTM C Mortar Tests for Unit Masonry Complying with ASTM C270 Property Specifications: a. For each mix provided, test mortar for initial consistency and board life, mortar air content and compressive strength in accordance with ASTM C Flexural Bond Strength Testing: a. For each type of construction provided at the following applications, test mortar for flexural bond strength in accordance with ASTM E 518 or ASTM C Below grade masonry basement walls or retaining walls in contact with earth. 2. Exposed exterior, above grade, load-bearing and non-load-bearing walls, and parapet walls. 7. Grout Testing for each mix provided: a. Test grout for temperature in accordance with ASTM C b. Test grout for slump in accordance with ASTM C 143. c. Test grout for strength in accordance with ASTM C One set of 4 prisms for every 500 square feet of wall area or floor lever (whichever is less to give the most tests). One prism is tested at 7 days and 3 at 28 days. E. Masonry Preparation and Placement (Technical II) 1. Base Conditions: On a periodic basis, verify that masonry bearing surfaces are clean. 2. Condition of Units: On a periodic basis, verify that masonry units are clean, sound and dry. 3. Proportions of site prepared mortar: On a periodic basis, verify proportions of prepared mortar are consistent with previously submitted materials. 4. Placement: On a periodic basis, inspect laying of masonry units for the following: nominal unit widths, stack or running bond, proper thickness and tooling of mortar joints, acceptable depth of furrowing of bed joints. Note temperature at time of inspection. 5. Joints: On a periodic basis, inspect construction, expansion and contraction joints for location and continuity of steel. 6. On a continuous basis, verify hot and cold weather procedures are followed. 7. On a continuous basis, verify wall cavities are protected against entry of precipitation. F. Masonry Reinforcement (Structural I) 1. Vertical Reinforcement: On a continuous basis inspect placement and alignment of all vertical bars and dowels for size, grade and spacing. Inspect length of lap splices, clearances between bars, clearances to masonry units and outside face of walls and positioning of steel. 2. Horizontal Reinforcement: On a periodic basis, inspect horizontal joint reinforcement (HJR) steel and masonry reinforcement bars for size, length of lap splices, dowels, clearances between bars, clearance to masonry units and outside face of walls and alignment. 07/17/

12 3. Ties: On a periodic basis, inspect ties in masonry for type, straightness, embedment, spacing and size. 4. Dowels and Anchors: On a continuous basis inspect the installation of masonry anchor bolts, joist anchors, inserts, straps and dowels. G. Prior to Masonry Grouting and Capping (Technical II) 1. Grout Spaces: On a continuous basis, verify that grout spaces are correctly sized and clean, cleanouts are closed after inspection and grout barriers are in place before grouting 2. Reinforcement: On a periodic basis, verify placement of reinforcement and connectors remains consistent with Contract Documents. 3. Site Prepared Grout: On a periodic basis, verify proportions of site prepared grout are consistent with previously submitted materials. H. During Grout Placement (Technical II) 1. Grouting: a. Continuously observe proper grouting technique including consolidation to approved height of grout space, cleanouts, rebar positioning, reconsolidation and vibration. b. Grout Specimens: Observe 100% of preparation of all required grout specimens, mortar specimens, and/or prisms to be tested. 2. Dry Packing: On a periodic basis, verify proper application of dry packing STRUCTURAL STEEL (SECTION ) COMPOSITE STEEL DECKING (SECTION ) - STEEL ROOF DECKING (SECTION ) A. High Strength Bolting: (Technical II) 1. Preparation: Visually inspect mating surfaces and bolt type for all slip-critical bolted connections for general conformance with the Contract Documents prior to bolting. 2. Slip Critical Bolts and Tension Bolts: Test bolt tightening in 100% of all bolts. Verify that all plies of connected elements have been brought into contact, at 100% of connections. Verify all tips are removed from "twist"-off bolts. 3. Bearing Bolts: Visually inspect to confirm all plies of connected elements have been brought into contact, at 100% of connections. (Applies only to bolts designed for values not requiring exclusion of threads from failure plane, all other bolts require testing as for tension bolts.) 4.
